4.Immunohistochemical Expression of PD-L1 in Cutaneous Squamous Cell Carcinoma and Its Association with Risk of Metastasis.

BACKGROUND: Programmed cell death ligand 1 (PD-L1) plays a major role in the immune responses of a variety of cancers. Recently, several studies revealed that PD-L1 is differently expressed in some cases of non-melanoma skin cancer. The expression of PD-L1 in cutaneous squamous cell carcinoma has not yet been described in Korea. OBJECTIVE: To investigate the expression of PD-L1 in cutaneous squamous cell carcinoma and its association with variable clinicopathological factors. METHODS: We performed immunohistochemical staining of 52 cutaneous cell carcinoma cases, including 28 high-risk cases and 24 low-risk cases. Cases were selected from patients who had visited the department of dermatology of our hospital from 2001 to 2017. The expression patterns were assessed using the H-score. Cases demonstrating at least 1+ of PD-L1 in more than 1% of tumor cells were considered positive. RESULTS: PD-L1 expression of tumor cells was 19.2% (10/52) for all cases, 0.0% (0/24) for the low-risk group, and 35.7% (10/28) for the high-risk group. PD-L1 positive cutaneous squamous cell carcinoma cases showed a significantly higher proportion of large tumors and tumors with deep invasion and a higher lymphatic metastasis rate when compared to PD-L1 negative cutaneous squamous cell carcinoma cases. CONCLUSION: Our study shows that cutaneous squamous cell carcinoma exhibits PD-L1 expression in 19.2% of cases. PD-L1 positive tumors are associated with high-risk cases of cutaneous squamous cell carcinoma, which may help guide the choice of therapeutic strategy.

5.Primary Diffuse B-cell Lymphoma of Uterine Cervix: A Case Report

Primary diffuse large B-cell lymphoma (DLBCL) of the female genital tract is a rare case. It is hard to diagnose DLBCL of the uterine cervix before surgery because it is often misdiagnosed as cervical myoma or cervical squamous cell carcinoma. Here, we report a case of cervical DLBCL misdiagnosed as a cervical myoma. A 48-year-old premenopausal woman was referred to the gynecology department due to abnormal uterine bleeding with a normal Papanicolaou smear. The initial diagnosis according to ultrasound and computed tomography was a cervical myoma. She had undergone a hysterectomy with bilateral salpingectomy. The final diagnosis was cervical DLBCL and she was referred to the department of hematology for treatment with Rituximab combined chemotherapy. After six courses of chemotherapy, complete remission was reached. At a 2-year follow-up, the patient is alive without interval change.
6.A Study on Various Sizes and Volumes of the Palate among the Korean Population in Mixed Dentition

The purpose of this study was to compare the palatal dimensions (volume, width, length, and height) in different malocclusions (Class I, II, and III) in mixed dentition using a three-dimensional digital scanner. The study was performed on 30 selected casts from 1400 casts that were taken at the Department of Pediatric Dentistry at Dankook University. Casts consisted of Class I, II, and III malocclusion groups in Hellman’s dental age IIIA. The mean age was 8 years and 6 months ± 11 months. Each cast was scanned by three-dimensional digital scanner, Medit T710 (Medit, Seoul, Korea), and shaped into the three-dimensional image and calculated palatal dimensions using the Plan T program (SMD solution, Seoul, Korea). The values were statistically compared and evaluated by Kruskal-Wallis followed by the Mann-Whitney test. According to our results, subjects with Class II malocclusion showed lower palatal width and longer palatal length compared to those with Class I and Class III. For palatal height, Class III malocclusion subjects in mixed dentition exhibited a larger number than Class II and Class I. Lastly, for palatal volume, compared to other malocclusions, Class III showed higher results; however, there were no significant differences. The form of the palate differs in types of malocclusions and understanding of these differences is important in clinical significance. Based on this study, the understanding of the relationship between the shape of the palate and the skeletal pattern provides useful information about orthodontic treatment plans, early diagnosis of malocclusion, and morphological integration mechanisms. Orthopedic treatment in the maxilla should be performed during early and intermediate mixed dentition to enhance treatment efficiency.

8.Currrent Source Analysis of Interictal Spikes in a Patient With Ictal Grimacing.

Facial grimacing can be a manifestation of complex partial seizures from the temporal lobe. We observed a case of seizure with facial grimacing and partial loss of consciousness during an electroencephalography recording. The recording revealed interictal spikes on the left-sided inferior temporal electrodes and ictal discharges starting on the same electrodes. The current source appeared to be in the inferior and lateral temporal areas of the left cerebral hemisphere. These results show that it is possible to localize the current sources responsible for interictal spikes.

9.Xpert CARBA-R Assay for the Detection of Carbapenemase-Producing Organisms in Intensive Care Unit Patients of a Korean Tertiary Care Hospital.

Carbapenemase-producing organisms (CPO) are rapidly disseminating worldwide, and their presence in tertiary care hospitals poses a significant threat to the management of nosocomial infections. There is a need to control CPO, especially in intensive care unit (ICU) patients, because these organisms are resistant to most beta-lactam antibiotics and are easily transmitted. At present, the identification of CPO is time-consuming; hence, this study focused on the use of the Xpert CARBA-R assay (Cepheid, USA) to determine intestinal colonization rates of CPO in patients admitted to the ICU of a tertiary care hospital in Korea. Forty clinical stool samples were collected and inoculated both in a CARBA-R cartridge and in conventional culture plates. The CARBA-R assay required only ~one hour to screen CPO, while the time required for conventional culture was over three days. We also found that the prevalences of intestinal colonization by carbapenem-resistant organisms and Enterobacteriaceae were 17.5% (7 out of 40) and 7.5% (3 out of 40), respectively. Among the colonizing strains, three that contained carbapenemase, including Klebsiella pneumonia carbapenemase (KPC), and imipenem (IMP) and Verona integron-mediated metallo-beta-lactamase (VIM) were found. With its convenience, the Xpert CARBA-R assay can be included in CPO surveillance strategies.

10.Evaluation of the Surgical Margin Threshold for Avoiding Recurrence after Partial Nephrectomy in Patients with Renal Cell Carcinoma

Purpose:
An adequate minimal surgical margin for partial nephrectomy (PN) has not yet been conclusively established. Therefore, we aimed to compare PN recurrence rates according to surgical margin status and to establish an adequate minimal surgical margin.
Materials and Methods:
We retrospectively studied patients with clinically localized renal cell carcinoma who underwent PN between 2005 and 2014. Surgical margin width (SMW) was assessed for all surgical tissues and divided into three groups: SMW <1 mm, SMW ≥1 mm, and positive surgical margin (PSM). The data were analyzed using the Kaplan-Meier method with log-rank tests and multivariate Cox regression models.
Results:
Of 748 patients (median age, 55 years; interquartile range, 46–64 years; 220 female), 704 (94.2%) and 44 (5.8%) patients had negative and PSMs, respectively. Recurrence-free survival was significantly lower in patients with PSMs (p<0.001) and was not significantly different between SMW ≥1 mm and <1 mm groups (p=0.604). PSM was a significant predictor of recurrence (hazard ratio: 8.03, 95% confidence interval: 2.74–23.56, p<0.001), in contrast to SMW <1 mm (p=0.680).
Conclusion
A PSM after PN significantly increases the risk of recurrence. We discovered that even a submillimeter safety surgical margin may be enough to prevent recurrence. To maximize normal renal parenchyma preservation and to avoid cancer recurrence in renal parenchymal tumor patients, PN may be a safe treatment, except for those with a PSM in the final pathology.
