The perichondrium has neochondrogenic potential. Many authors have published their study for the cartilage regeneration from perichondrium and the clinical uses of perichondrial graft. How is the difference of thickness of the regenerated cartilage after crushing the free perichondrial graft, compare with uncrushed graft? In this study, the perichondrium was obtained from rabbit, ears, and sectioned 10x10 mm slices. The uncrushed and crushed perichondriums were grafted into subcutaneous layer of back. After the 2,4 and 8 weeks, authors obtained regenerated cartilage from both crushed and uncrushed free perichondrial graft from rabbit back, examined the survival rate and thickness by histologic study.The results are as follows: 1. The survival rate was no difference between the uncrushed perichondrium & crushed perichondrium (p>0.05). 2. The cartilage from the uncrushed perichondrium was thicker than the crushed perichondrium, at only 8 weeks(p<0.01). 3. The thickness of regenerated cartilage was similar to the cartilage of the donor site of free perichondrial graft at 4 weeks(p>0.05).

A retrospective, multivariate statistical analysis of 394 patients who underwent cleft palate repair was performed to document the incidence of postoperative oronasal fistula formation, and to assess possible contributing factors. Fistulas of the secondary palate were included, but nasal-alveolar fistulas and intentionally unrepaired anterior palatal fistulas were excluded. Postoperative oronasal fistulas occurred in 25 of the 394 patients(6.35%). The mean age at repair was 16.4 months, and mean follow-up period was 2.5 years. Several variables that are included sex, age, extent of clefting(as estimated by the Veau classification), type of repair, and experience level of the operating surgeon were analyzed by means of the log-rank test to determine their significance in postoperative fistula formation. Sex (p=0.077), age(p=0.538), and experience level of the operating surgeon(p=0.094) did not significantly affect the rate of fistula formation. However, extent of clefting(p=0.005) and type of repair(p=0.001) are the strongest predictor of the occurrence of a cleft palate fistula.

Objectives: Cheyne-Stokes respiration (CSR) is frequently found in critically ill patients and is associated with poor prognosis. However, CSR has not been evaluated in neurocritical patients. This study investigated the frequency and prognostic impact of CSR in neurocritical patients using biosignal big data obtained from intensive care units. Methods: This study included all patients who received neurocritical care at the tertiary hospital from January 2018 to December 2019. Clinical information and biosignal data of intensive care units were used and analyzed. The respiratory curve was visually assessed to determine whether CSR and obstructive sleep apnea (OSA) were present, and a heart rate variability (HRV) was obtained from the electrocardiogram. Results: CSR was confirmed in 166 of 406 patients (40.9%). Patients with CSR were older, had a higher frequency of cardiovascular risk factors as well as heart failure, and had a poor outcome (modified Rankin scale ≥4). As a result of multiple regression analysis adjusted for other variables, CSR was significantly associated with poor outcome with an odds ratio of 2.27 times higher (95% confidence interval 1.25–4.14, p=0.007). HRV analysis demonstrated that CSR and OSA had distinct autonomic characteristics. Conclusions This study first revealed the substantial frequency of CSR in neurocritical patients and suggests that it can be used as a predictor of poor prognosis in neurocritical care.

PURPOSE: To describe the appearance and average thickness of the internal anal sphincter with anal endosonography in healthy Korean adults. MATERIAL AND METHODS: 184 subjects(male: 96, female: 88) with no history of anorectal disease or surgery were studied with anal endosonography. The average thickness of internal sphincter was meas ured at the mid-anal canal. For the internal sphincter, which is often asymmetric, the thickness of each 4 part(12, 3, 6 and 9 o'clock direction) were measured in left lateral decubitus position. RESULTS: The anal wall was well visualized in 5 layers(mucosa, submucosa, internal anal sphincter, intersphincteric plane, external anal sphincter) with anal endosonography. The anal endosonogram showed the internal anal sphincter as a homogenous, well-defined, hypoechoic, circular band, and slightly asymmetric. The average thickness of the internal anal sphincter in the area of mid-anal portion was 2.0 0.3 mm(range: 1~3 mm). There was no sexual difference; however, a significant positive correlation with age was found in average thickness of the internal anal sphincter. The correlation with lean body mass was not found. CONCLUSION: The internal anal sphincter is well-visualized, best defined structure by anal endosonography. Average thickeness of the sphincter in Korean appeared to be the same as in the Western.

BACKGROUND: The gaitaids are helpful for the patients of gait disturbance, but there was no study about the quantitative assessment of the effectiveness of gaitaids of several different types. But proper use of gaitaids is important for the disabled who is suffered from falls with or without trauma. The appropriate prescription of gaitaids is coming from the objective assessment for gaitaids. METHODS: We evaluated the 'balance index(BI)', which is originally suggested assessment scale by author, for the five different gaitaids :mono-cane, quad-cane, forearm crutch, axillary crutch, walker We assessed the difference of B.1 with or without using each gaitaids examined by 50 healthy adults and 20 stroke patients. RESULTS: The BI in the normal adult was 34.5+/-2.7, and 21.0+/-6.9 in the stroke patients. The balance index o( walker user was 37.6+/-1.4, which was the highest score. The BI of quad-cane was 30.1 +/-4.8, which was the next highest score and the BI of mono-cane, forearm crutch, axillary crutch were 25.5+/-5.6, 25.5+/-5.7, 25.5+/-5.4, which were lower than that of quad-cane. CONCLUSION: It can be suggested that the walker is the most effective for the support balance on gait and the quad-cane is more effective than mono-cane or crutches for balance support.

Extraskeletal bone-forming lesions are myositis ossificans, fibrodysplasia(myositis) ossificans progressiva, and extraskeletal osteosarcoma. It is of paramount importance to clearly distinguish these lesion from extraskeketal osteosarcoma. Myositis ossificans, by far the most common among these lesions, is a benign, ossifying process that is generally solitary and well circumscribed. It is found most commonly in the musculature, but it may also occur in other tissue, especially in tendons and subcutaneous fat. It occur as the result of various kinds of soft tissue injury(surgical scars, burns, dislocation of joints etc.) and also observed in patients with tetanus, in paraplegics secondary to traumatic spinal injury. Although there is general agreement that myositis ossificans is a nonprogressive benign process without nerplastic potential, its pathogenesis is still poorly understood.We experienced a myositis ossificans associated with chronic, extensive pressure sore in patient with paraplegia secondary to spinal injury.

With an increase in the instances of obesity, the cases of type 2 diabetes, which is caused by obesity, have also increased significantly. Because of this, the number of obesity metabolic operations performed on diabetes patients with obesity is also accumulating, and there has been no concern for implementing approaches to psychological support for these patients. Negative psychologies include anxiety, depression, passive attitude, stress, fear and impulse control disorder, which continuously influence the patient in a vicious circle of recurrence of obesity and diabetes, even after the obesity metabolic operation was attempted. Therefore, for the success of the obesity metabolic operation and the continuation of self-management of obesity and diabetes post-operation, psychological support is very important. Post-operative psychological support approaches include a respiration method, autogenic training, self-expression training, a stress reduction program, thought-change training and communication skills.
