Targetoid hemosiderotic hemangioma is a benign vascular tumor with a peculiar clinical presentation and histologic features sometimes difficult to differentiate from the patch stage of Kaposi's sarcoma, progressive lymphangioma, eosinophilic hemangioma, retiform hemangioendothelioma. We report a case of targetoid hemosiderotic hemangioma on upper back and right upper arm of a 28-year-old male. He has a 5mm × 5mm sized slightly raised violaceous papule surrounded by an ecchymotic halo on upper back and right upper arm. We took a biopsy of the lesion and identified it as targetoid hemosiderotic hemangioma. This is a case of targetoid hemosiderotic hemangioma with typical clinical and histopathological findings.

Once splinter hemorrhage can be considered as a pathognomonic sign of subacute bacterial endocarditis. But it can also be associated with a variety of systemic disorders that increase capillary fragility or primary nail bed involvement in dermatologic disorders. The cause of splinter hemorrhage can usually be established by careful history and physical examination. We report a case of 33-year-old man with splinter hemorrhages, who had polycythemia vera.

Reconstruction of a injured digit needs good aesthetic result as well as functional recovery. Since Honda deported the first clinical application in 1984, the venous flap has been used increasingly for skin and soft tissue defects of the fingers and hands. From november 1992 to december 1995, 12 single-pedicled venous island flaps from hand dorsum were used for coverage of soft tissue defects of fingers over exposed bone, tendon, or joint. The pedicle of the flap included one superfitial dorsal vein of hand and its perivenous areolar tissue. Eight of twelve flaps survived completely and four necrosed partially possibly due to excessively long pedicle and poor perivenous areolar tissue. All donor sites were closed directly. Most patients were satisfied with postoperative result cosmetically and functionally. This method has several advantages : easy versatility and operability with superficial location of numerous dorsal vein, making good contour owing to proper thickness of flap, minimal donor site morbidity, and short operation time. We consider that the pedicled venous island flap is another reliable and useful method for reconstruction or finger defects as long as the pedicle length is not long and the perivenous areolar tissue is enough.

BACKGROUND: Atopic dermatitis (AD) is a chronic, relapsing inflammatory skin disease with a genetic and environmental background. Recently, a marked rise in the prevalence of AD has been observed. Current epidemiological research is largely focused on identifying environmental factors associated with AD; however, little is known about the factors affecting the disease severity in Korean childhood AD. The city of Incheon is a large harbor city, notorious for air pollution caused mainly by industry and the yellow dust that blows in from China. The environmental peculiarities of Incheon may play a critical role in AD expression. OBJECTIVE: We investigated the epidemiologic characteristics of preschool childhood AD patients and the correlation between environmental factors and its prevalence or severity in Incheon. METHODS: This study was carried out among 2523 preschool children from May to July, 2007. It was based on a questionnaire covering their clinical manifestations, environment, and lifestyle. Two dermatologists examined the skin and evaluated the severity of AD using the SCORAD index. RESULTS: The prevalence of AD among preschool children in Incheon was 18.8% (475 AD patients) which was higher than the overall prevalence in Korea (6.16~11.2%). There were statistical differences in the SCORAD index between heavily industrialized areas and residential housing areas in Incheon (p=0.012). CONCLUSION: Theses results suggest that environmental factors may affect the prevalence and severity of atopic dermatitis.

The majority of ptients with ischemic heart disease such as angina pectoris and myocardial infarction have coronary atherosclerosis as a underlying cause. However, in unusual conditions such as variant angina, aortic valvular heart disease, hypertrophic cardiomyopathy, vasculitis, hypercoagulable states, coronary atherosclerosis is not the cause. Polycythemia vesa is a myeloproliferative disease characterized by infrequent thromboembolic complications such as ischemic heart disease. We report a case of polycythemia vera presenting with angina pectoris without coronary artery stenosis with the review of the literature.

BACKGROUND: In vivo and/or in vitro diagnostic tests to identify possible allergens have been conducted for patients with chronic spontaneous urticaria (CSU) in order to identify any predisposing factors and avoid unnecessary restrictions. OBJECTIVE: The purpose of this study was to evaluate the diagnostic effectiveness of ImmunoCAP(R) ISAC (ISAC) in CSU patients. METHODS: A total of 31 patients with CSU were tested by ISAC and Multiple Allergen Simultaneous Test enzyme immunoassays (MAST-EIA). A skin prick test (SPT) was performed for 17 out of 31 patients. The results of ISAC and MAST-EIA were evaluated based on the SPT results. RESULTS: For 17 patients, with SPT used as the reference standard, ISAC compared with MAST-EIA showed lower sensitivity (22.2% vs. 26.5%), higher specificity (100% vs. 95.7%), higher positive predictive value (100% vs. 52.9%), and lower negative predictive value (86.9% vs. 87.7%). The agreement rate was more favorable for ISAC than for MAST-EIA (87.3% vs. 85.1%; kappa=0.324 vs. 0.263). For 31 patients, the agreement rate between ISAC and MAST-EIA was 83.9% for both Dermatophagoides farinae and Dermatophagoides pteronyssinus. Correlation coefficients (Rho) between ISAC and MAST-EIA for each allergen were 0.690 and 0.675, respectively. CONCLUSION: For CSU patients, specificity, positive predictive value, and the agreement rate of ISAC were similar or superior to those of MAST-EIA. In addition, ISAC provides valuable information on the possible sensitization to different allergen sources by elucidating cross-reactive components. With careful clinical correlations in interpretation, ISAC can be a useful diagnostic tool to detect allergens in CSU patients.

The routine screening of patients for hypercalcemia has increased the incidence of hyperpa-rathyroidism, But, Primary hyperparathyroidism is relatively rare disease in korea. Especially, primary hyperparathyroidism caused by parathyroid chief cell hyperplasia has not been reported. The numbers of patients with bone disease, renal stones, other severe complication has declined, but the numbers of patients with psychiatric and neuromuscular disturbance or with asymptomatic disease has increased. We experienced a case with severe bone diseases due to primary hyperparathyroidism caused by parathyroid chief cell hyprepiasia which was confirmed through the mearsurement of serum calcium and parathyroid hormone, neck CT scanning and surgical exploration and which was managed by total parathyroidectomy with autotransplantation.
