3.Intracranial calcified schwannomas: report of two cases.

Calcification is exceedingly rare in schwannomas. In the literatures, we found only three reports of schwannomas with calcification. We report two cases of intracranial calcified schwannomas, one in the anterior cranial fossa and the other in the middle cranial fossa.

5.Analysis of Conservative Management of Thoracolumbar and Lumbar Fractures

In recent years, the rate of the thracolumbar and lumbar fractures rends to be increasing as the traffic accidents and industrial accidents frequently occurs. The purpose of this study is to review the results of conservative treatment for 73 patients with thoracolumbar and lumbar fractures and to review the failed 6 cases after conservative treatment, who were admitted and treated at National Medical Center during the period the period from January 1986 to June 1992. The following results were obtained: 1. Among the 65 cases of compression fracture, we performed hyperextension body cast in patient with height loss more than 30% and bed rest & brace in patient with height loss less than 30%, And in 63 cases, it showed good results. But two cases treated by bed & brace showed complaint such as radicular pain and persisdent back pain. 2. Among the 6 cases of burst fracture without neurological involvement, two cases treated by casting showed good result, but four cases treated by bed rest & brace showed complaint such as claudication and persistent back pain. 3. All cases of Chance fracture treated by casting showed good result. 4. There was no correlation between loss of correction of kyphotic angle and overall result. 5. Among the cases of poor result, we operated four cases which included two cases of instability & two cases of spinal stenosis and their results were good.

6.Effects of Growth Hormone Secretion with Insulin Induced Hypoglycemia and L-dopa in Children with Short Stature.

PURPOSE:Insulin induced hypoglycemia and L-dopa are potent for growth hormone(GH) secretion in children. We evaluated the effects of GH secretion with insulin and L-dopa in 22 children with height percentile below 3 and 11 children with height percentile between 10 to 25. METHODS:Thirty four children were performed GH secretion study after classified by height percentile and bone age according to their age and sex. Twenty two children are height percentile below 3 and bone age is delayed more than one years compare to chronologic age(group A). As a control group, twelve children took part in this study and their height percentile were between 10 to 25 but, bone age was not concerned(group B). Serum GH concentration and blood glucose level was detected on 0, 30, 60, and 90 minutes after insulin 0.1U/kg was injected intravenously. And then serum GH concentration was measured on 0, 30, 60, and 90 minutes after L-dopa 10mg/kg was administered orally. Serum GH was measured by radioimmunoassay. RESULTS:GH level in group A was below 7ng/mL in 13 children(59%) after insulin and L-dopa administration respectively but in 11 children(50%) GH level were all below 7ng/mL after insulin and L-dopa adminstration. GH deficiency(7ng/mL) was detected only one children in group B. In Group A and B, peak GH concentration was noted on 30 minutes after insulin administration, but on 60 minutes after L-dopa, peak GH concentration appeared in group B. GH concentration in zero time to 90 minutes after L-dopa was steady increased in group A. CONCLUSION: Anthropometric data such as height percentile and bone age are good for prediction of GH deficiency and if we use these data and GH secretory effects of insulin induced hypoglycemia and L-dopa, we can predict GH deficiency more accurately.

8.Factor VIII Related Antigen in Granuloma Pyogenicum.

Using Avidin-Biotin immunoperaxidase method, we investigate the distribution patterns of factor VIII related antigen in various areas of skin lesions of 14 granuloma pyogenicum. and in the skin lesions of 5 capillary hemangioma and 5 normal skin. The results are summarized as follows: 1. Factor VIII related antigen was detected in 9 of 14 cases of granuloma pyogenicum, in 4 of 5 cases of capillary hemangioma, and in 5 of 5 cases of normal skin. 2. In the lesions of granuloma pyogenicum the areas with prominent capillary endothelial cell proliferation had less positivity than the vascular areas. And the areas with relatively large vesels in the lesions of granuloma pyogenicum had similar positivity as capillary hemangioma. 3. The lesions of granuloma pyogenicum and capillary hemangioma had less positivity than the vessels of normal derrnis.

9.A Case of Angiommunoblastic Lymphadenopathy.

Herein we report a 51-year-old male with angioimmunoblastic lymphadenopathy who had developed rnaculopapular eruptions after the administration of crystalline penicillin, acetylsalicylic acid and sulpyrine. Skin biopsy showed mild lyrnphohistiocytic infiltration and extravasated RBCs around the blood vessels in the upper dermis. He had run a progressively downhill course in spite of vigorous chemotherapy and ultimately died.

10.Two Cases of Lichen Planus.

We have experienced 2 cases of lichen planus. Case 1 was a. 76-year-old male who has suffered from discrete, scaly, round to polygonal papules on the trunk and the e:xtremiities for 3 months. The VDRL test for syphilis was reactive at a titer of 1:2 and the TPHA test was positive. The skin lesions were responded to 3 weekly injections of benzathine penicillin 2.4 million units, Case 2 was a 17-year-old male who has suffered from discrete, erythematous to violaceous colored, flat papules on the right shin for several months. He has treated with oral medication of griseofulvin. After 2 weeks of the medication, the skin ]esions showed improvement with gradual reduction of pruritus and flattening of the papules. About one month of the medication, all skin lesions cliappeared with dark brownish hue and hypopigmentation. And recurreci lesions after stopping medication are disappeared after 2 months of retreatment with griseofulvin.
