1.Two Cases of Type V Hyperlipoproteinemia and Eruptive Xanthomas associated with Diabetes Mellitus.

We report two cases of type V hyperlipoproteinemia and eruptive xanthomas associated with diabetes mellitus in a 44-year-old woman and a 25-year-old woman. They had asymptomatic, diffuse, erythematous yellowish papules on the extremities and buttocks. The laboratory studies showed increased serum blood glucose, total cholesterol, triglyceride, increasing pre-β and chylomicron bands on electrophoresis of lipoprotein. Histopathologic findings of erythematous yellowish papules on the thigh and buttock revealed aggregation of foam cells in the dermis. They have been treated with diet control, hypoglycemic agent (gliclazide) and hypolipidemic drug (gemfibrozil), and are under continued supervision.

2.Incomplete AICA Syndrome Presented with Peripheral Facial Palsy; A Variant of Gasperini Syndrome.

Acute infarcts of the anterior inferior cerebellar artery (AICA) territory are unusual. Furthermore incomplete AICA infarcts are perplexing because of its variations of vascular anatomy and inconsistent clinical features. We present a case with clinical features of AICA infarction, which consist of ipsilateral peripheral-type facial palsy, vertigo, and contralateral facial and upper limb sensory changes without motor weakness. The patient had hypertension and was a current smoker. The high signal intensity on inferior pontine tegmental area was found on MRI and the R2 interneuronal dysfunction was note on Blink reflex. The angiographic findings didn't show any focal vascular lesions, which is contrary to the pathogenesis of AICA infarction published previously. On the clinical ground, the present case reserves to attention in that patients with peripheral-type facial palsy should be properly evaluated and with thorough neurological examination and we could differentiate between the incomplete AICA infarcts such as Gasperini syndrome and Bell's palsy.

4.Status Epilepticus after Mye lography with Inadve rtent Injection of Diatrizoate.

A 37-year-old male patient was transferred from a local orthopedic clinic due to status epilepticus following a myelography with diatrizoate. On his laboratory findings, metabolic acidosis was prominent. Non-enhanced brain CT scan showed a high densinty in the subarchnoid space. With careful respiratory support, hydration, correction of acid-base balance, administration of dexamethasone, and anticonvulsant therapy, he completely recovered after 3 days. Diatrizoate is an ionic, iodinated and hyperosmolar X-ray contrast medium. It must not be injected intrathecally because of the risk to the central nervous system. The possible mechanisms are anaphylactic reaction due to iodine hypersensitivity, hyperosmolar effect, and direct chemotoxicity by ionicity and chemical structure itself.

5.The Depth of Double Lumen Endobronchial Tube and the Effect of Lateral Decubitus Position to the Depth in Korean Adult Patients.

BACKGROUND: During one-lung anesthesia in thoracic surgery, accurate placement of double lumen endobronchial tube(DLT) is essential. We have only a few data regarding adequate depth of DLT, especially after position change. We measured the depth of DLT and the change of depth after position change in 50 adult patients. METHODS: After endotracheal intubation with left-sided Robertshaw type polyvinyl chloride(PVC) endobronchial tube, fiberoptic bronchoscope was inserted through tracheal lumen and we positioned the proximal end of the bronchial cuff just below the carinal bifurcation. The depth of the DLT from the upper incisor was recorded, and was rechecked after position change to lateral position. RESULTS: The depth of the DLT was 29.4 +/-1.3 cm(male: 30.2+/- 0.9, female: 28.2+/- 0.8) and this was related with height. But in female group depth of DLT was not related with height. Within the same height group, the depth of DLT differ up to 3.5 cm. The change of depth more than 0.5 cm after position change ocurred in 21 out of 50 patients. CONCLUSION: The adequate depth of DLT in Korean patients, in total patients and in male group seems to related with their height, but in female group the depth of DLT was not related with their height. Within same height group, the depth of DLT may differ greatly. So, the depth of DLT must be adjusted individually. After position change, the head and neck should be kept in neutral position and the adequacy of the depth of DLT must be rechecked.

7.A Case of Malignant Pleural Mesothelioma Combined with Asbestosis in a Boilermaker and Plumber.

Diffuse mesotheliomas of the pleura and peritoneum are considered "signal tumors" of asbestos exposure. Although asbestos use in Korea started before the national liberation, it rose abruptly with economic development in 1970's. Considering latent period of mesotheliomas as twenty years, occupational mesothelioma cases would increase rapidly. In Korea construction is a major industry which use asbestos. Because workers in construction industry are mostly composed of daily based workers, it is hard to calculate the number of workers involving asbestos related work, to manage them with organized method, to educate them about adverse effects of asbestos and protection methods, to check health status of them periodically, and to follow them up. Although a case of occupational mesothelioma which was found in a asbestos textile worker have been reported, no case related with construction workers have been reported so far in Korea. The authors experienced occupational malignant pleural mesothelioma combined with asbestosis in a boilermaker and plumber in a construction industry.

8.A Clinical Study on Intestinal Tuberculosis.

Intestinal tuberculosis is an infectious disease which is still prevalent in the developing countries including Korea. It may occur insidously, have a vague clinical marofestation, and take a chronic course. We have analyzed 101 cases of intestinal tuberculosis. proven by clinical and histopathologic diagnosis for 10yrs since 1977. The results are as follows: 1) Intestinal tbc was prevalent in the age of 20-30 and the male to female ratio was 1:1.7. 2) Among clinical manifestations, abdominal pain, and tenderness, general malaise and abdominal mass were observed in order of frequency. 3) Active pulmonary lesion was associated in 73 cases (73.3%). 4) Ileocecal region was the most frequent site of involvement in barium study and in colonoscopy. 5) The most frequent colonoscopic finding was multiple ulceration and pseudopolyposis and chronic granulomatous inflammation was noted in 58% on hitopathology. 6) Ulcerative type was the most common type 45.4% by intestinal x-ray and colonofiberscopic examination

10.Soft tissue masses of extremities:MR findings.

To evaluate MR findings of soft tissue masses in extremities and to find the helpful findings of distinguish benignity from malignancy, 28 soft tissue masses (22 benign and 6 malignant) in extremities were reviewed. T1-weighted, proton density, T2-weighted and Gd-DTPA enhanced images were obtained. MR images allowed a specific diagnosis in a large number of benign masses, such as hemangioma(8/9), lipoma(2/2), angiolipoma(1/1), epidermoid cyst(2/2), myositis ossificans(1/1), synovial chondromatosis(1/1) and pigmented villonodular synovitis (1/2). Specific diagnosis was difficult in the rest of the masses including malignancy. However, inhomogenous signal intensities with necrosis and inhomogenous enhancement may suggest malignant masses.
