1.Ultrastructural Feature of Proximal Convoluted Tubular Cells of Rat Induced by Gentamicin.

Myeloid body formation is an ultrastructural feature of gentamicin induced nephrotoxicity in human being and experimental animals. The origin of the myeloid body is not satisfactorily understood and morphological verification of the developing process of this structure is not fully accomplished. We injected 100 mg/kg/12 hour of gentamicin in 20 Spraque-Dawley rats and examined the ultrastructural feature of the proximal convoluted tubular cells of the kidney every 30 minutes in the first 4 hours, and in 5 hours, 6 hours, 12 hours, 24 hours and 48 hours after injection of gentamicin, with a TEM and a SEM. Myeloid bodies were noted as concentric layers of membranous structures of degenerated endoplasmic reticulum and mitochondria in the lysosome. The number and size of the myeloid body containing lysosomes were increased with time. We can deduce from this observation that injured cell organelles by diffusible gentamicin within the cells are autophagocytosed by lysosomes which were also injured by the drug from pinocytotic vesicles, and incompletely digested organellar remnants are retained in the lysosomes as myeloid bodies. So we think that the myeloid body formation is a result of an exaggerated and a pathologic autophagocytic process due to cell injury induced by gentamicin.

2.A Case of Superficial Basal Cell Epithelioma of the Labium Majus.

A 40-year-old woman presented with a pruritic erythematous eroded patch on the right labium majus which had been for 3 years. Histopathological findings were consistent with superficial basal cell epithelioma. Basal cell epithelioma of the labium majus is a rare neoplasm and its clinical appearance is usually nodular. In contrast to the other types of basal cell epithelioma, the superficial type occurs mainly on the trunk. We report a case of superficial basal cell epithelioma arising from the labium majus.

3.A Case of Papulonecrotic Tuberculid Combined with Erythema Nodosum.

We report an extremely rare case in which two tuberculids, papulonecrotic tuberculid and erythema nodosum, occurred together. A 16-year-old girl had two types of clinically distinct lesions; asymptomatic necrotic papules on the upper and lower extremities and painful subcutaneous nodules on the legs. A Mantoux test showed a strong positive reaction. Histopathologic findings of the necrotic papule and subcutaneous nodule were consistent with papulonecrotic tuberculid and erythema nodosum, respectively. There was no extracutaneous focus of tuberculosis. Polymerase chain reaction for tuberculous bacilli in the papulonecrotic tuberculid lesion revealed a negative result. Antituberculous therapy brought a rapid clinical improvement of both lesions.

4.Sarcoma-like Mural Nodule in Ovarian Mucinous Cystadenocarcinoma: A case report.

We describe an ovarian mucinous cystadenocarcinoma with a sarcoma-like mural nodule. In the literature, rare cases of ovarian mucinous tumors have been described which contain foci of undifferentiated carcinoma, sarcoma, and sarcoma-like nodules. The distinction between these lesions is important because of poorer prognosis of true sarcoma and anaplastic carcinoma than sarcoma-like mural nodules. This case shows different results of immunohistochemical stain for anaplastic carcinoma.

6.A Case of Granulomatous Reaction to Tattoo Pigment.

A 48-year-old woman, who had had tattoo on her eyebrows, was seen with pruitic erythematous indurated plaques on the tattooing sites. The skin react ion was begun to observe about 4 weeks after getting tattoo. Skin biopsy specimens from the indurated lesions showed granulomatous reactions. Intradermal tests by means of multiple pricks and patch test were done on her low back with the same tattoo dye and also 4 others available in Korea. Serial punch biopsy specimens were obtained from the sites of intradermal test at 2nd, 7th, 28th, and 56th day after intradermal test. Granulomatous reactions were found in the spiecimens taken at the 28th and 56th day.

7.A Case of Dysplastic Nevus.

We report herein a case of dysplastic nevus, which developed on the snterior chest and abdomen in a 48-year-old American Catholic priest. Clinical features present as brownish to pinkish patches with irreguar border fading into surrounding skin, ranging from 0.5 to 1.5cm in diameter. Histopathological findings show disordered proliferation of slightly atypical melanocytes in the epidermis and fusing of epidermal rete ridges, concentric eosinophilic fibrosis along elongated rete ridges, lymphohistiocytic infiltration in the dermis, and extension of intraepidermal melanocytic hyperplasia beyond the shoulders of dermal nevus component. One year follow-up revealed no evidence of recurrence.

9.Pityrosporum Folliculitis.

Thirty-nine patients with pityrosporum folliculitis were investigated clinically and histopathologically. On clinical observation there were numerous, chronic, moderately itchy (64.1%), dome-shaped papules (89.7%) and pustules (66.7%). The most frequent sites of the lesions were the upper portion of the chest (76.9%) and back (56.4%). In biopsy specimens, abundant round and budding yeast cells were seen in a dilated hair follicle. The reptured follicle was observed in 19 specimen (48.7%). The accumulation on inflammatory cells were observed in or around the upper part of the follicle in all specimens. The effect of antimycotic treatment was excellent. After 4 weeks of treatment, 36 patients (92%) were cured and 3 (8%) had improved significantly. KOH/Parker Ink direct smear was done in 20 patients. Blue-colored round and budding yeast cells were observed under a light mcroscope in all patients. We suggest that pityrosporum folliculitis is a common disease of young and middle-aged Koreans.

10.A case of Primary Serous Papillary Carcinoma of the Peritoneum..

Although the peritoneum is rare site for a primary neoplasm, certain malignant neoplasms may arise from it. A case of 63-year-old woman who had a serous papillary carcinoma of peritoneal origin is reported. Extraovarian peritoneal serous papillary carcinoma was characterized by ascites, malignant washings, and omental involvement with bulky infiltration and/or multiple tumor nodules. The symptoms caused by diffuse spreading of the neoplasm over the peritoneum are the most important manifestation for clinical diagnosis of malignant primary neoplasm of peritoneum, especially serous papillary carcinoma. This behaving tumor was at least partially responded to therapy. In this report, we describe a case of serous papillary carcinoma of peritoneum carring on proper management with brief review.
