Throughout medical science today there is increasing emphasis upon understanding the familial and associated genetic interplay in various disease processes. We observed a case of Legg-Calve-Perthes disease developed in monozygotic male twins. They have some mirror image of each other, for example hair whorl and involved hip. This report of the disease which is present in mirror fashion in idenitical twins illustrates enen more vividly the genetic interaction in the disease. Therefore, those who are aware that Legg-Calve- Perthes disease has appeared in a family should keep in mind the probability of the disease developing in another member.

The heart, 500g in weight, with subacute myocarditis associated with granulomatous myocarditis may be a good model for the study on the pathophysiologic mechanism of cardiac failure. Furthermore, the clinical data of this case is enough to clarify his all clinical course from admission to death due to cardiac failure. So, we analyzed the clinical data, histologic findings, and morphometric pattern of histologically intact myocardial cells and inflammatory reaction to investigate the pathophysiologic mechanism of the cardiac failure. The results were summarized as follows. 1. Clinically, the heart showed cardiac failure of diastolic phase and abnormal conduction system related to sudden cardiac death. However, it might be adapted to the relatively stable wital signs due to pericardial positive pressure by slowlyprogressed pericardial effusion. 2. The distribution pattern of area of intact myocardial cell area and inflammtion reaction showed relatively even spread of inflammatory reaction and extremely decreased area of myocardial cells to about 21% of total heart. So, its contractility might be decreased below to the 21% of the normal cardiac contractility. 3. The mechanism of the cardiac failure in myocarditis may be sudden inflammatory involvement of conduction system and/or extremely decreased myocardial cell volume due to inflammatory destruction. 4. Morphometric analysis may be a useful objective method to grading the severity of old and recent form of myocarditis. From the above results, the cardiac failure of myocarditis is influenced by the adaptability at the inflammatory abnormality of the conduction system, contractility of injured myocardial cells, and compensation activity of pericardial effusion.

The Hahn double 15 hue test is used for social and vacational aptitude test to separate strongly and mildly affected subjects among the colour vision defective persons, detected using colour vision test. However, the assessment of colour vision defect type and severity is based on the hue confusions which are represented diagrammatically on Hahn double 15 hue score sheet, this qualitative assessment of the test results have not provide a numerical score suitable for mathematical analysis. This paper presented a new proposal for quantitatively scoring the Hahn double 15 hue test based on those hue confusions made by the subject. With this program large numbers of double 15 hue test results can be processed easily and rapidly, and program helps to compare the severity of specific type colour vision defect and monitor acquired colour vision defect which has various disease process, continuously.

Cases sharing features of both primary biliary cirrhosis and autoimmune hepatitis have been reported as a mixed type, overlap syndrome, immunocholangitis and autoimmune cholangiopathy. A primary biliary cirrhosis- autoimmune hepatitis overlap syndrome is unusual and characterized by overlapping features; cholestasis, high titer of alkaline phosphatase, bile duct damage and granulomas in the liver biopsy, high antinuclear antibody, increased IgG and IgM and intra-acinar hepatitis with piecemeal necrosis. Autoimmune mechanisms are thought to play a major role in the pathogenesis of the overlapping syndrome and the bases of immunosuppressive therapy. A 58-year-old female patient shows overlapping clinical and laboratory findings, chronic active hepatitis in initial liver biopsy which transits to primary biliary cirrhosis with cholangitis and granulomas. This is a case of hepatobiliary lesion showing overlapping features of both primary biliary cirrhosis and autoimmune hepatitis over 3-year period.

The purpose of this study was to investigate the gaps of bone-acetabular component interface, the initial mechanical stability and the effects of adjuvant screw fixation in the press-fitted two different shaped porous-coated acetabular components. Semi-hemispherical Harris-Galante II cup and full hemispherical Trilogy cup were press-fitted with or without adjuvant screw fixation in 40 bovine femur. The gaps of bone-implant interface were measured in each radiography of all specimen and axial compression tests and rotational stability tests were performed using servohydraulic tension-compression machine. There was significantly smaller gap at polar 1/3 region in the specimen implanted with Trilogy cup. But the adjuvant screw fixation did not affect the amount of gap. There was no significant statistical difference in axial load and rotational stability test in each situations of fixation. This study demonstrates that the initial mechanical stability is not influenced by the shape of semi-hemispherical Harris-Galante II cup and full hemispherical Trilogy cup and also independent of adjuvant screw fixation. But smaller gap of Trilogy cup at the polar 1/3 region indicates the possibility that less amout of wear debris of polyethylene or metal will be deposited and therefore less osteolysis or cup loosening will occur, but this hypothesis should be confirmed by the long-term clinical follow-up.
