1.Clinical Manifestations of Cerebellar Infarction Mimicking Unilateral Vestibulopathy .

BACKGROUND AND OBJECTIVES: Pseudo-labyrinthine symptom without any other accompanying neurologic symptoms or signs occur with cerebellar infarction. The prognosis and management of cerebellar infarction differ from those of vertigo associated with unilateral vestibulopathy. The objective of study was to analyze the clinical symptoms of cerebellar infarction mimicking unilateral vestibulopathy according to the infarction territory. MATERIALS AND METHOD: Among 273 patients who showed pseudo-labyrinthine symptoms or signs, 28 patients were diagnosed cerebellar infarction on MRI between January 2003 and October 2006. Out of the 28 patients, 9 patients accompanying with focal neurologic symptoms or signs in early stage were excluded, and a retrospective analysis of total 19 patients was done through chart reviews. Using diffusion-weighted imaging, we divided 19 patients into two groups, AICA and PICA territory infarction. Clinincal features, outcome of audiologic and vestibular function test were compared between the groups. RESULTS: Among the 19 subjects, 10 were males and 9 were females, the mean age was 63.1+/-.0. On MRI, 5 (26.3%) subjects showed infarction in unilateral AICA territory, 14(73.7%) in unilateral PICA territory. In the patients with AICA territory infarction, audiological tests confirmed unilateral sensorineuronal hearing loss in all 5 subjects. On V-ENG, 4/5 (80%) demonstrated horizontal or mixed horizontal torsional spontaneous nystagmus, and 4/5 (80%) had a canal paresis to caloric stimulation. In several days of hospital stay, 3/5 (60%) showed new neurologic signs, facial numbness. In the patients with PICA infarction, there was no hearing change. On V-ENG, 12/14(85.7%) demonstrated horizontal or mixed horizontal torsional spontaneous nystagmus, and 7/14 (50%) had a canal paresis to caloric stimulation. In several days of hospital stay, 10/14 (71.4%) accompanied with cerebellar ataxia. CONCLUSION: Cerebellar infarction simulating unilateral vestibulopathy is more common than previously thought. Early recognition of the cerebellar infarction showing pseudo-vestibular symptom may allow specific management. Also taking into account that clinical features differ by infarction territory will assist in determining the patient's status of the disease.

3.Clinical Experiences of Hydroxyapatite Implantation.

Hydroxyapatite which has been used as bone substitute in orthopedic and maxillofacial surgery is used as ocular implant recently. It has been shown to be completly biocompatable, nontoxic and nonallegic so less extrusion and migration develop. We performed 37 cases of hydroxyapatite implantation: 20 cases(51.4%) after evisceration, 8 cases(21.6%) after enucleation and 9 cases(24.3%) secondarily for the better prosthetic motility. Wound dehiscence developed in 6 cases(16.2%): 2 cases(10.0%) after evisceration, 2 cases(25.0%) after enucleation and 2 cases(22.2%) after secondary hydroxyapatitie implanation. Wound dehiscence was managed with dermis graft in 1 case and healed spontaneously in 1 case. Another 4 cases are under observation to expect the wound dehiscence to heal spontaneously. But no extrusion, migration and infection of implant were noted. We thought that wound dehiscence after hydroxyapatite implantation was related to conjunctival damage and atrophy by coarse surface of hydroxyapatite implant. Wound dehiscence can develop after hydroxyapatite implantation but careful patient selection and reducing tissue damage during surgery may alleviate the problems of wound dehiscence.

4.Erythrokeratodermia Variabilis with Alopecia Universalis.

We describe a 8-year-old girl with erythrokeratodermia varibilis (EKV). This diagnosis was supported by erythematous, configurate patches over the entire body. Their size, shape and location varyed with enviromental factors and emotional state. In addition, well defined brownish, hyperkeratotic plaques were scattered over the trunk and extremities. Also, this patient had alopecia universalis, which is not typical of EKV. Etretinate therapy resulted in an almost complete clinical clearing of the plaques; however, her alopecia universalis and erythematous lesions were not affected.

9.Transient Visual Loss in Peripapillary Staphyloma.

PURPOSE: To report the case of a patient with transient visual loss with a peripapillary staphyloma. CASE SUMMARY: The authors of the present study examined a 30-year-old woman who complained of transient visual loss in her right eye. The patient lost her vision for 5 seconds on average approximately 5 times a day. Her visual acuity was 20/20 in both eyes. There were no abnormalities on slit lamp examination. A deep excavation with choroidal atrophy in the peripapillary area of the right eye was found. The patient was diagnosed with peripapillary staphyloma. Her physiological blind spot in the right eye was enlarged on Humphrey visual field testing. However, she had no abnormalities on color vision testing, fluorescein angiography, or magnetic resonance imaging (MRI). There were no abnormalities in the evaluation of the brain, carotid arteries, or heart. There was no evidence of vasculitis or hypercoagulability. The symptom did not change on pressing the eyeball, performing the Valsalva maneuver or carotid massage, applying cycloplegics, or shining a strong light in the other eye. The symptom did not change after taking a calcium channel blocker for 4 weeks.

10.Giant Papillary Conjunctivitis Induced by Protruding 10-0 Prolene Sutures After Cataract Surgery.

Giant papillae of the upper tarsal conjunctiva have been found in vernal conjunctivitis and, more recently, in giant papillary conjunctivitis in patients with contact lenses. Two patients who received cataract surgery were found to have unilateratgiant papillary conjunctivitis which were supposed to be induced by protruded suture end(10-0 prolene). Eosinophil was not found in the conjunctival scraping from giant papillae of the patients. Giant papillae of tarsal conjunctiva were resolved by removal of protruded sutures.
