2.Historical Perspective of Helicobacter pylori Treatment in Korea.

Since the discovery of Helicobacter pylori in 1983 by Warren and Marshall, it has been recognized as one of the most significant risk factors for gastric cancer and has been associated with various gastrointestinal disorders such as peptic ulcer disease. In Korea, the triple therapy of proton pump inhibitor, clarithromycin, and amoxicillin has been recommended as a primary regimen since 1998. However, the eradication rate of conventional first line therapy of H. pylori infection has been decreasing progressively, primarily due to increased resistance to antibiotics. A recent meta-analysis showed that the overall eradication rates were 74.6% by intention-to-treat analysis and 82.0% by per-protocol analysis. Therefore, the need for alternative first line eradication regimens has been rising. Sequential therapy, concomitant therapy, and various combinations of new antibiotics such as quinolone and rifabutin have been introduced as new options, but they have yet to be proven as standard first line therapy. Further nation-wide surveillance regarding the antibiotic resistance rates and well-designed prospective randomized controlled multicenter trials on the empirical first line therapy are necessary to establish the appropriate treatment for H. pylori in Korea.

7.Effects of Botulinum Toxin in Spasmodic Disorders and Focal Dystonia.

Botulinum toxin A injection has been recently used in the treatment of spasmodic disorders and the focal dystonia for relieving its abnormal conditions. This treatment, however, is not popular in Korean because of difficulty in getting the drug. Se have injected Botulinum toxin A in 7 various patients;spasmodic torticollis:2, hemifacial spasm:3, blepharospasm:1, ankle spasticity:1. 3 patients improved greatly and 2 patients showed moderate, but 1 patient showed only mild improvement. No significant side effects showed except the transient ptosis in the blepharospasm and the mild focal muscle weakness in the hemifacial spasm.

8.Treatment of Hydrocephalus Following the Operation of Ruptured Intracranial Aneurysms.

Hydrocephalus, which is one of the major complications following intracranial aneurysm operations, can prevent patients from improvement of consciousness, symptoms and signs that can be expected after operation. Whenever there is no evidence of appreciable improvement after intracranial aneurysm operation, follow-up brain CT scannings will reveal the development of hydrocephalus. And also radioisotope cisternography and clinical findings could be clarified as Glasgow coma scale(GCS) and Hunt & Hess classification, which will help clinicians making a decision on shunt operation. Among 183 patients of subarachnoiod hemorrhage admitted, 92 patients were operated on due to intracranial aneurysms. Nine patients(9.8%) of them were underwent shunt operations because of hydrocephalus following intracranial aneurysm operations. Eight patients showed improvement of clinical findings and 9 patients have been improved according to GCS score.

9.New Technique of Orthotopic Ileal Neobladder: Early Experience.

PURPOSE: During the last 10 years, a multitude of different ileal neobladders have been reported. The ureterointestinal anastomotic site stricture has been shown to be one of the most common late complications. To avoid the stricture, a new orthotopic ileal neobladder was constructed, with two chimneys and two afferent ileal limbs beside the reservoir. MATERIALS AND METHODS: Twenty-six men, with invasive bladder cancer, underwent a radical cystectomy, with the creation of a two chimney ileal neobladder. The mean follow-up was 18 months. The complications were assessed and divided into those related or unrelated to the urinary tract. The continence was evaluated using patient questionnaires. RESULTS: There was no case of an ureteroileal anastomotic site stricture. Five cases of vesicoureteral reflux were noted in the 52 ureteral units. Good, or satisfactory, day- and nighttime urinary continence was reported at the 6 month follow-up by 92.3 and 69.2% of patients, respectively. CONCLUSIONS: The ileal neobladder with two chimneys would be an appropriate technique for the reduction of the ureteroileal anastomotic stricture rate and to overcome some of limitations of the ureteral length.

10.Differentiation of malignant from benign pancreatic mass by Tl-201 abdominal SPECT.

The aim of the present study was to evaluate the ability of Tl-201 abdominal SPECT to differentiate between chronic focal pancreatitis and pancreatic malignancy. Seventeen patients (12 men, 5 women; mean age, 56 years; 9 pancreatic cancer, 8 chronic pancreatitis) with pancreatic mass were prospectively investigated with Tl-201 abdominal SPECT. In all patients, CT and/or US could not clarify the nature of the pancreatic mass. Focal hot uptake was present in 8 of 9 patients with pancreatic cancer, while it was present in 2 of 8 patients with chronic pancreatitis. Therefore, the sensitivity and specificity of the present study were 89% and 75%, respectively. A significant difference of Tl-201 uptakes was noted between benign and malignant masses (p& 0.05). Therefore, we concluded that Tl-201 abdominal SPECT was a useful test in differentiation of malignant from benign pancreatic mass, especially when the differentiation could not be made by other imaging modalities
