Restoration of functional and anatomic changes of kidney in UPJ stricture is one of most important subjects in urology. A clinical observation was made on the 28 cases (29 kidneys) of pyeloplasties for UPJ stricture from Jan. 1976 to Dec. 1981. Of them 20 cases (21 kidneys) were followed up for more than 6 months postoperatively. The following results were obtained; 1. The patients in this series were 22 males and 6 females, and 11 children and 17 adults. The left side was 3 times more frequently affected than the right. They symptoms on admission were mostly flank pain, palpable mass, and etc. 2. Preoperative status of UPJ was; UPJ stricture alone in 20, UPJ stricture with aberrant vessel in 4, UPJ stricture with stone in 3, and UPJ stricture with vesicoureteral reflux in 1 case. 3. Preoperative grade of hydronephrosis was closely related to each other in the functional and anatomic aspects of kidney. In general, children showed relatively more advanced hydronephrosis than adults and the degree of improvement of hydronephrosis postoperatively was similar to each other. 4. Anderson Hynes' method of pyeloplasty in most cases (20 kidneys) and simple pyeloureterostomy in 9 kidneys were performed, and results between both operations showed no difference. But Anderson Hynes' method was more effective in marked hydronephrosis such as giant hydronephrosis and simple pyeloureterostomy in mild hydronephrosis. 5. In cases in which UTI was present preoperatively, postoperative UTI tended more to persist than in those not complicated with preoperative UTI, and in both sides postoperative UTI mostly persisted for 2 months and disappeared within 6 months. 6. The period of nephrostomy and ureteral stent was not directly related to postoperative results, but nephrostomy without ureteral stent relatively showed good results. 7. Postoperative complication was UTI in 2, re-do pyeloplasty in 1 and secondary nephrectomy in 1 case. 8. Improvement Of hydronephrosis continued for about a year after operation. It is recommended that more careful attention should be paid for the first year and be continued to watch for consecutive 2 years.

Arteriovenous Fistula*

Dendritic cell (DC)s are protessional antigen presenting cells and they have been used for antitumor immunotherapy or cell vaccines. However therapy using DC is restricted because the number of DC available from tissue is very low. Flt3 ligand (FL) has been known as a hematopoietic growth factor that increases proliferation of hematopoietic stem cells and progenitor cells, and recently it showed inducibility of dendritic cell (DC)s and signiticant antitumor effects in vivo. Thus FL will be frequently used for DC induction and antitumor immunotherapy in future. Here we constructed FL plasmid and studied its in vivo effect. FL plasmids were made by cloning of partial FL cDNA into pcDNA3 plasmid, and gene expression and protein producibility of FL plasmid were confirmed in Renca cells transfected with FL plasmid. Mice were injected with FL plasmid (100ug/mouse) three times and 20 days later mouse spleens were harvested for staining and RT-PCR. There were lots of blastogenic cells in the spleen of mice treated with FL plasmid. FL plasmid also induced DEC205, IL-12 and GM-CSF gene expression in mouse splenocyte. All these data suggest FL plasmid may be used for induction of DC and antitumor therapy as DNA adjuvant.

This study was designed to investigate the mole- cular mechanism of chemokine induction by lipopoly-saccharide(LPS) of E. coli. Chemokine gene expression was evaluated by the reverse transcriptase-polymerase chain reaction(RT-PCR) assay using RNAs isolated either from kidneys of LPS-injected inice or from the mesangial cells stimulated with LPS, IFN-r or TNF-a. LPS was shown to induce interferon gamma(IFN- 7 ) inducible protein 10 (IP-10) and monokine induced by interferon gamma (MIG) in kidney. IP-10 gene expression was induced by LPS and IFN-r, but MIG gene expression was induced by IFN-r in mesangial cell. Chemokines induced by LPS increased splenocyte migration. Sodium salicylate, wortmanin and piperazine blocked LPS mediated chemokine induction suggesting the activation of nuclear factor-a B pathway. It is concluded from this study that mesangial cells are the target of LPS in the renal failure resulting from the systemic infections. LPS induces chemokines directly and/or indirectly in the mesan- gial cells, and these chemokines may associated with renal inflammation.

Systemic lupus erythematosus is a connective tissue disease which can affect every organ system. Neurologic abnormalities are common, occuring in approximately half of all patients at some time during the course of their illness. But symptoms of nervous system as the sole presenting symptoms occur in less than 1% of lupus patients. In patients initially presenting with neurologic symptoms and signs, differential diagnosis is difficult and sometimes it may be misdiagnosed. Therefore extensive laboratory investigations should be carried out in all patients with unusual neurological symptoms, since early diagnosis of lupus can help in providing effective treatment. We report a patient with systemic lupus erythematosus who presented with dysarthria and dysphagia resembling multiple sclerosis.

Male breast cancer is a rare disease, representing approximately 1% of breast cancer and less than 1.5% of malignant tumors occurring in men. We report a case of cuaneous metastasis of male breast cances. a 64-year-old male who presented with multiple nodules and erythematous plaque on the right anterior chest and enlargement of the right axillary lymplinodes. The histopathologic findings of the nodule showed atypical tumor cells with a somew eat gland-like pattern. The cells showed potive reactions in PAS, DPAS and alcian blue stains, and also positive for estrogen and roiesterone receptors. Biopsy specimens taken from the plaque showed cellular arrangement in show line, referred to as Indian filing, between thiclenecin collagen bundles. He was treated not, tamoxifen, 5-fluorouracil, adriamycin, cyclophosphamide and megestrol acetate. 5 months after nitiation of the treatment, the skin lesions were improved vvith a decreased number and size or dules and decreased intensity of erythema.

Ferrocholinate, choline citrate iron salt is a new chelate iron which is synthesized by reacting ferric hydroxide with choline dihydrogen citrate. Ferrocholinate is used in the prevention and treatment of microcytic, hypochromic anemias due to iron deficiency. Ferrocholinate does not release high concentrations of free ionic iron in the gastrointestinal tract and clinical reports, although limited, seem to indicate that this drug is better tolerarated than is ferrous sulfate or ferrous gluconate. Ferrocholinate : syrup was administered to 19 patients of iron deficiency anemia. Ferrocholinate was administered orally. Therapeutic dosage was 1.2cc/kg (elementary iron 6mg/kg) divided three times daily. The results were obtained as follows. (Table 1) 1) In this clinical trials, the over-all effective rate was 84.2% 2) There was no effect on 3 cases who have combined with severe infection and congenital heart disease. 3) Of the 19 patients, 2 patients complained transient nausea and 1 patient mild diarrhea. These symptoms, however, disappeared along with continued therapy.

From October 1974 to September 1984, 64 consecutive patients of bladder and urethral cancer were treated by cystectomy and urinary diversion at the Department of Urology, Pusan National University Hospital. Comparative study was made between enteric diversion and ureterocutaneostomy, and several surgical mo. dality of cystectomy. The results obtained were as follows. l. The modes of cystectomy were simple cystectomy in 5 cases (7.8%), total cystectomy in 48 cases (75.0%), radical cystectomy in 10 cases (15.6%) and pelvic exenteration in 1 cases (1.6%). 2. The causes of cystectomy with urinary diversion were primary bladder cancer in 60 cases (93.8%), urethral carcinoma in 2 cases (3.1%) and metastatic bladder cancer in 2 cases (3.1%). 3. The methods of urinary. diversion were ileal conduit in 38 cases (59.4%) , ileocecal conduit in 5 cases (7.8%), tube in double stoma ureterocutaneostomy in 7 cases (10.9%) and tubeless single stoma ureterocutaneostomy in 14 cases (21.9%). 4. Age distribution was from 4 to 79 years and average was 56.3 years. Male to female ratio was 7:l. 5. Most frequent urinary diversion was enteric in 6th decade and ureterocutaneostomy in 7th decade. 6. Average operation time was 360 minutes in enteric diversion with total cystectomy and 272 minutes in ureterocutaneostomy with total cystectomy. 7. High stage (>=pT2) was 76.5% in enteric diversion and 81.O% in ureterocutaneostomy. High grade (II or III) was 67.6% in enteric diversion and 81.0% in ureterocutaneostomy. 8. Early complication rates were 50.0% in ileal conduit, 60.0% in ileocecal conduit, 42.9% in tube in double stoma ureterocutaneostomy, and 35.7% in tubeless single stoma ureterocutaneostomy. Late complication rates were 52.4% in ileal couuit, 40.0% ileocecal conduit and 33.3% in tubeless single stoma ureterocutaneostomy. 9. Local recurrence of tumor was observed in 80.0% of simple cystectomy cases but recurrence was not in total or radical cystectomy. Conclusively, total or radical cystectomy is treatment of choice in invasive bladder tumor rather than simple cystectomy. And single stoma ureterocutaneostomy may be useful method of urinary diversion inhigh stage, high grade, aged and poor risk patient if stomal control is male carefully.
