To evaluate the predictability of visual outcome in vaculopathy patients after treatment, we compared the difference between the pre-treatment visual acuity measured by hand-held laser interferometer and the posttreatment corrected visual acuity using Han`s visual acuity chart. Disease distribution was as follows; BRVO(18 cases), CRVO(11 cases), CSCR(9 cases), posterior uveitis(4 cases). The mean up period was 3.45 months. Among 42 cases, 28 eyes showed positive disparity and 13 eyes showed negative disparity. The range of disparity was from -0.2 to +0.3(mean 0.113). The post-treatment visual acuity was predictable within 2 lines in 38 eyes(90%) We concluded that the hand-held laser interferometer is one of the reliable methods to predict the post-treatment visual acuity in maculopathy patients.

To evaluate the prediction of visual outcome in cataract patients, the authors compared the preoperatively measured vision using hand-held laser interferometry(HEINE LAMBDA 100, GERMANY) and Han's visual acuity chart with the postoperatively corrected vision. Among 40 patients, in the 12 cases with immature cataract and normal macula, the postoperative measured acuity was predicted within two lines in 7 cases(58%), and in the 27 cases with mature cataract including nuclear sclerosis, the disparity under -2 was only 6 cases(21%). In the 6 cases with macular abnormality, one patient showed positive disparity. These results indicate that the prediction of visual outcome for catarct sugery using the hand-held laser interferometer was reliable in immature catarct with normal macula.

From October 1974 to September 1984, 64 consecutive patients of bladder and urethral cancer were treated by cystectomy and urinary diversion at the Department of Urology, Pusan National University Hospital. Comparative study was made between enteric diversion and ureterocutaneostomy, and several surgical mo. dality of cystectomy. The results obtained were as follows. l. The modes of cystectomy were simple cystectomy in 5 cases (7.8%), total cystectomy in 48 cases (75.0%), radical cystectomy in 10 cases (15.6%) and pelvic exenteration in 1 cases (1.6%). 2. The causes of cystectomy with urinary diversion were primary bladder cancer in 60 cases (93.8%), urethral carcinoma in 2 cases (3.1%) and metastatic bladder cancer in 2 cases (3.1%). 3. The methods of urinary. diversion were ileal conduit in 38 cases (59.4%) , ileocecal conduit in 5 cases (7.8%), tube in double stoma ureterocutaneostomy in 7 cases (10.9%) and tubeless single stoma ureterocutaneostomy in 14 cases (21.9%). 4. Age distribution was from 4 to 79 years and average was 56.3 years. Male to female ratio was 7:l. 5. Most frequent urinary diversion was enteric in 6th decade and ureterocutaneostomy in 7th decade. 6. Average operation time was 360 minutes in enteric diversion with total cystectomy and 272 minutes in ureterocutaneostomy with total cystectomy. 7. High stage (>=pT2) was 76.5% in enteric diversion and 81.O% in ureterocutaneostomy. High grade (II or III) was 67.6% in enteric diversion and 81.0% in ureterocutaneostomy. 8. Early complication rates were 50.0% in ileal conduit, 60.0% in ileocecal conduit, 42.9% in tube in double stoma ureterocutaneostomy, and 35.7% in tubeless single stoma ureterocutaneostomy. Late complication rates were 52.4% in ileal couuit, 40.0% ileocecal conduit and 33.3% in tubeless single stoma ureterocutaneostomy. 9. Local recurrence of tumor was observed in 80.0% of simple cystectomy cases but recurrence was not in total or radical cystectomy. Conclusively, total or radical cystectomy is treatment of choice in invasive bladder tumor rather than simple cystectomy. And single stoma ureterocutaneostomy may be useful method of urinary diversion inhigh stage, high grade, aged and poor risk patient if stomal control is male carefully.

Crossed renal ectopia with fusion is an unusual congenital anomaly, probably produced by abnormal development of the ureteral bud. The deformity itself produces no symptoms but it presents possible urinary tract infection, obstuction and stone formation, and other anomaly in body. Herein we report two cases of crossed fused renal ectopia complained of abdominal mass in 28 year old female and abdominal pain after trauma in 14 year-old boy.

We evaluated the effect of nitric oxide on the corneal neovascularization in diabetic rats. The NADPH-diaphorase histochemistry was used to investigate the relationship between the nitric oxide synthase activity and the corneal neovascularzation of disbetic rats. Sprague-Dawley rats of 6 weeks of age were use in this study. Streptozotocin(65mm/kg, Sigma, USA) was injected intraperitoneally to induce diabetes. N-heptanol was applied to the right eyes of the diabetic rats to induce neovascularization and the left eyes were remained uninjured. After four weeks, 5 eyes of the non-diabetic rats, 5 eyes of the diabetic rats, and 5 eyes of the neovascularized diavetic rats were enucleated. The enucleated eyes were stained with NADPH-diaphorase histochemistry method and examined under the light microscope. Neovascularized corneas showed intense expression of NADPH-diaphorase at the whole layer of epithelium, the superficial stroma, and the walls of new vessels. Corneas of the nondiabetic rats and uninjured those of diabetic rats showed only mild expression of NADPH-diaphorase, and there were no significant difference of expression between them. According to these results, we think that the increased expression of nitric oxide synthase or nitric oxide may be related to the corneal neovascularization in diabetic rats.

OBJECTIVE: This study was performed to evaluate the feasibility, safety, and tolerance of early postoperative oral feeding in gynecologic patients who have underwent intra-abdominal surgery. Thus, we investigated the clinical efficacy of early postoperative oral feeding in gynecologic patients, prospectively. METHODS: From September 1998 to March 1999, we studied 80 gynecologic patients undergoing laparotomy at Department of Obstetrics and Gynecology, Yonsei University College of Medicine, prospectively. After surgery, the patients were grouped into two arms; the first early oral feeding group began a clear liquid diet on the first postoperative day and advanced to regular diet as tolerated. the second control group received nothing by mouth until return of bowel function. RESULTS: Although more patients in the study group developed nausea, the incidence of vomiting and abdominal distension were comparable in both groups. Time to development of bowel sound and flatus were significantly shorter in study group. Postoperative complications including pneumonia, atelectasis, wound complications, and febrile morbidity occurred insignificantly in both group. Postoperative changes in hematologic indices and electrolytes were comparable in both group. CONCLUSION: Early postoperative oral feeding in gynecologic patients undergoing intra-abdominal surgery is safe, well tolerated. We believe that re-evaluation of postoperative surgical care may increase our knowledge and better serve our patients.

Pterygium is known to cause corneal curvature changes by tractional effect on cornea. We measured central corneal curvature changes induced by pterygium and also evaluated topographic changes at the site of pterygium by difference map of corneal topography (Eyesys 2000, USA). Bedsides recurrent pterygium, we assessed keratomeric readings at central cornea(Sim K) and pterygium site in 20 cases of pterygium operated with bare sclera technique. Before operation, keratometric reading at 3mm central cornea(Sim K) was 43.71 +/- 1.12D in horizontal meridian, 44.94 +/- 1.41 D in vertical meridian and that of pterygium site was 40.92 +/- 3.28D. In the other hand, 4 weeks after pterygium excision. Sim K was 44.40 +/- 0.83D in horizantal meridian, 45.23 +/- 0.78D in vertical meridian and K reading of pterygium site was 44.45 +/- 1.51D. These results show that mean keratomeric changes of a pterygium site are greater than changes of mean Sim K(3.17 +/- 3.16D)0.61 +/- 0.43D). In conclusion, pterygium induced corneal astigmatism seems to be resulted from the peripheral flattening rather than the pterygium changes of central corneal curvature.
