1.Exercise-Induced Atrial Fibrillation.

Two cases of exercise-induced atrial fibrillation were diagnosed in 68 years old female and 47 years old male. They complained palpitation during exercise, and emotional upset for the last 5 years. There was no evidence of organic heart disease except mild hypertension in female patient. The atrial fibrillation was induced repeatedly by treadmill exercise test and intravenous infusion of isoproterenol. The beta blocker(atenolol 50mg po dialy) prevented the induction of atrial fibrillation during treadmill exercise. We report two cases of exercise-induced atrial fibrillation which is very rare and might be related to catecholamine.

2.Coronary Angioplasty in Patients with Multivessel Coronary Artery Disease.

To assess the likelihood of procedural success in patients with multivessel coronary artery disease, 46 consecutive patients (male 34, female 12, mean age 60+/-9 years) umderwent single or multiple site angioplasty. The clinical diagnosis of unstable angina was in 20(44%), stable angina in 10 and acute or old myocardial infarction in 16. Coronary angiographic findings of 2 vessel disease was in 38(83%), triple vessel disease in 8. Left ventricular function was generally well preserved (mean ejection fraction 65+/-12%, range 30-82%) and mean 2.0 stenosis per patient angic, lasty had attempted. Single vessel angioplasty (SVA) was performed in 13 and multivessel angioplasty (MVA) in 33. Procedural success was achieved in 79(86%) out of total 92 stenoses. Sixty-six(88%) out of 75 stenoses in MVA and 13(76%) out of 17 stenoses in SVA had procedural success respectively. According to angiographic morphology of lesions, procedural success of type A stenoses was 17/17(100%), type B stenoses 57/66(86%) and type C stenoses was 5/9(33%). In 13 failures included inability to pass the guide wire cross the lesion in 7, inability to guide the griding catheter in 2 and inability to dilate lesions in 4. Before and after angioplasty, treadmill test (modified Bruce protocol)could be performed in 29 patients. Total duration of exercise and maximal double product improved significantly from 8.5+/-2.3 minute 5188+/-2403 to 12.2+1.3 min., 23,062+/-4111 respectively (p<0.001). 17 out of 24 patients who had positive treadmill test before angioplasty showed negative conversion after procedure. Complications included dissection in 29, prolongd chest pain in 5, acute closure in 3, cardiac tamponade in 1 and ventricular fibrillation due to side branch oclusion in 1. Thus, coronary angioplasty in selected paients with multivessel coronary artery disease might be useful and have relatively good immediate results, but the long-term efficacies with other forms of treatment must be evaluated prospectively.

3.Evaluation for ultrasonographic findings of hepatoma

Ultrasonographic findings in 34 cases of primary hepatoma, and 18 cases of secondary hepatoma, which had been proved histopathologically or suspected clinically and ultrasonographically, were reviewed. The results were summarized as follows. 1. Among 52 cases, 40 cases were male and 12 cases were female. The male predominent than the female with the ratio of 4:1. In 52 cases, 40 cases (77%) of the patients were between 41 years and 60 years.2. The most common ultrasonographic finding of hepatoma was discrete homogeneous hyperechoic mass. 3. Primaryhepatomas were usually solitary and relatively large, while secondary hepatomas were usually multiple andrelatively small. 4. Hepatitis, liver cirrhsis, and clonorchiasis were more common in primary hepatomas than secondary hepatomas.

4.Relationship of Air Pollution and Pediatric Respiratory Diseases in Incheon City.

PURPOSE: It is generally accepted that the air pollution can cause acute respiratory diseases. This study was undertaken to investigate the relationship between the increase in the level of air pollutants and hospital visits for pediatric respiratory diseases in Incheon city. METHODS: We retrospectively reviewed the medical records of pediatric patients who visited the outpatient department(OPD) of Inha University Hospital for respiratory diseases(such as bronchial asthma, pneumonia, bronchitis, bronchiolitis and URI) from January 1 to December 31 1997. We converted the data into an average per day that included meteorologic data of air pollution(such as O3, CO, SO2, NO2, and PM10) and weather data(such as atmospheric temperature, relative humidity) in three different places in Incheon city. We used a Poisson distribution, selecting a lowess statistics model. We also used the S-PLUS statistics program. RESULTS: Ozone(O3), CO, and SO2 were all found to have significant associations with the OPD visits for pediatric respiratory diseases. After meteorologic variables(such as CO, SO2, NO2, PM10, atmospheric temperature, and relative humidity) controlled, we determined the relative risk between the increase in the OPD visits for respiratory diseases and every 0.01 ppm increase in O3. The relative risk was 1.16 and the 95% confidence intervals were 1.12-1.20. CONCLUSION: There was a significant relationship between the incidence of pediatric respiratory diseases and the increase in the level of air pollutants, especially O3, CO, and SO2 in Incheon city. Further studies on the effects of continuous daily exposure to low or ambient levels of air pollutants are needed.

RESULTS: After several variables were controlled, we determined the relative risks between the incidence of pediatric respiratory diseases and air pollutants as CO=0.92, NO2=1.12, 03=1.21, SO2= 1.04, and PM10=1.00. The 95% confidence intervals were CO=0.88-0.19, NO2=1.05-1.14, O3=1.09- 1.28, SO2=1.07-1.31, and PM10=1.01-1.05. CONCLUSION: There was a significant association between the incidence of pediatric respiratory diseases and the level of air pollutants, especially NO2, O3, SO2, and PMio in Inchon city. More studies on air pollution and its effect on pediatric respiratory diseases are needed.

6.An Experimental Model of Isolated Lung Block for Evaluation of Pulmonary Preservation after Ischemia.

During the last 30 years, major organ transplantation has become popular, even in Korea, such as kidney, liver, etc. After the successful clinical cardiac transplantation in Korea, many cases of cardiac transplantation are being performed in some centers. But lung transplantation has a lot of obstacles, especially donor shortage and decreased tolerability of the lung to ischemia-reperfusion injury. Usually it was considered that the maximum safety margin of ischemic time in lung transplantation was about 4 to 6 hours. So, many investigators have tried to develop better preservation methods and experimental model for evaluation of effectiveness in those various methods. But most of those methods had several drawbacks in clinical and experimental settings. So we developed an easily-controllable, reliable, and inexpensive experimental model of isolated rabbit lung block. Using these model, we evaluated its effectiveness and reliability for the experiment of ischemia-reperfusion injury in lung transplantation.

BACKGROUND: Automated external defibrillator(AED) represent a major breakdown to permit more widespread application of the principle of early defibrillation. Many recent efforts to improve emergency medical services(EMS) and increase survival rates are simply efforts to get defibrillation to patients as rapidly as possible. AED is major innovation for the prehospital care of ventricular fibrillation cardiac arrest patients. The purpose of this study is to evaluate the course of initial training to three different groups(nurses, firefighters, and EMT trainee) to use AED. METHOD: We studies the efficacy of education of AED to 33 nurses, 15 EMT trainee, and 16 firefighters. Training lasted 75 mins and included 45 mins an overview of defibrillation, protocols for using the AED, and operation of the AED(Laerdal Heartstart 3000),15 mins demonstraion.4 check list was used to grade the performance of cardiopulmonary resuscitation, operation of the AED, and the time required to deliver the first three defibrillations. RESULT: There were no statistically significant differences in performance and time required to deliver an electrical countershock among the groups(p=0.4). To the second test 92fo of all group completed all steps successfully. The step most often foiled was the preparing of the AED for defibrillation. CONCLUSION: In nurses, EMT trainee, and firefighters, it is both feasible and effective to train AED use irrespective of the degree of the trainee.

8.The Change of Fetal Liver Length and Liver Volume by Ultra-sonography according to Gestational Age in Normal Pregnancy.

OBJECTIVE: The measurement of liver size can be used for the diagnosis of the fetal growth abnormality (FGR, macrosomia etc.). The purpose of this study was to evaluate a mathematical relationship between the fetal liver size(liver length or volume) and the gestational age in the normal pregnancies. Brief comparisons were also tried on the base of the degree of the correlation between liver length and its volume. METHODS: We collected 54 singleton pregnancies of 20 to 36weeks of gestation for measuring fetal liver length and 57 singleton pregnancies for measuring fetal liver volume. We used Combison 530 utrasonic machine(Kreztechnik AG, Zipf, Austria). RESULTS: There was significant correlation between liver size( length and volume) and gestation age. And the liver volume better correlated with gestational age than liver length(r=0.93 : r=0.78, p<0.0001, p<0.0001). CONCLUSION: Ultrasonic measurement of fetal liver size is a reliable indicator of fetal growth, especially liver volume. Therefore these data may have a potential value for the prediction of abnormal fetal growth(FGR, macrosomia).

9.The Outcomes of 17,744 Cervicovaginal Smears in Ihha University Hospital.

OBJECTIVE: Our purpose was to investigate the distribution of the result of 17,744 cervicovaginal smears and their pathologic results in Inha University Hospital to make a reference data. METHODS: During 28 months, May 1996 to August 1998, we performed 17,774 cervicovaginal smears and arranged them by the Bethesda system and compared them with their pathologic results. RESULTS: Among 17,744 smears, there were 93.2% of benign results, 2.9% of ASCUS, 0.9% of LSIL, 1.4% of HSIL, and 0.8% of SCC. The ASCUS/SIL ratio was 1.28. Among ASCUS, the high grade histologic outcome was composed 4.0%. Among LSIL, there were 15.7%, and HSIL 48.0%, SCC 56.1%, AGUS 9.3%, and Adenocarcinoma, the results was 69.2%. CONCLUSION: Among 17,744 smears, there were 93.2% of benign results, 2.9% of ASCUS, 0.9% of LSIL, 1.4% of HSIL, and 0.8% of SCC. The ASCUS/SIL ratio was 1.28. These results were very similar to other data and we would like to add our data to them as a reference. And the worse the smear results were, the worse the pathologic results were.

10.The Distribution of CA-125 Level Among the Patients Who Underwent Hysterectomy.

OBJECTIVES: To understand the importance of the serum level of Ca-125 among pelvic mass, we performed a study. METHOD: From January to December 1998, we performed the study. Before hysterectomy, we performed a blood sampling to know the serum level of Ca-125, After hysterectomy, we weighed the uterus and measured the thickness of endometrium and other histologic characteristics. RESULTS: We performed my research to 80 peoples. The relation between uterine weight and the serum level of Ca-125 is little, if ever(R2=0.0007), and the relation between the thickness of endometrium and the serum level of Ca-125 is also little, if ever(R2=0.0353). The relation between leiomyoma, the cycle of endometrium and the serum level of Ca-125 were also little, but there was a close relationship between adenomyosis and the serum level of Ca-125. CONCLUSION: There was little relationship between uterine weight and the serum level of Ca-125.
