3.Accuracy of the sphygmomanometer for measuring of blood pressure.

BACKGROUND: Recently, it is substituted automatic sphygmomanometer for mercury sphygmomanometer. But it seems to be insufficient for data of its accuracy. A sample accurate automatic sphygmomanometer could have an important role in the management of hypertension. The aim of this study is to assess the accuracy of the automatic sphygmomanometer that is used common practice and at home. METHODS: We collected 247 patients who visited the department of Famiiy Practice of Taegu medical center from April to August 1996. BP was measured sequentially same arm by standard device(mercury. sphygmomanometer), test device A(A&D TM-2650), test device B(seine SE-2000). We assessed the automatic sphygmomanometer according to the standards set out by the British Hypertension Society(BHS) protocol and the American Association for the Advancement of Medical Instrumentation(AAMI). These data were analysed using pearson' correlation and paired t-test. RESULTS: Test device A was highly correlated to mercury sphygmomanometer in systolic and diastolic BP(r=0.90, r=0.88). Also test device B was highly correlated to that(r=0.90, r=0.87). The mean difference between BP value obtained by the standard device and those obtained by the test device A were 0.59+/-7.66mmHg systole(mean+/-SD) and 3.83+/-6.43mmHg diast.ole, whereas the difference between the former and those obtained by the test device B were 1.70+/-7.99mmHg systole.and 5.58+/-6.38mmHg diastole. Comparing to standard device, there were a signifioant difference except systolic BP of test device A(P<0.05). According to the criteria of the AAMI, the diastolic BP of test device B was not enough and according to the criteria of the BHS, the diastolic BP of both test device were not enough. CONCLUSIONS: Both test device were highly correlated to mercury sphygmomanometer. But according to the criteria of the BHS and AAMI, there were not enough. Because the use of automatic sphygmomanometer was popularized, I think that further study will be required to assess of accuracy.

7.Posterior Spinal Fusion using Autogenous Bone Graft Combined with Xenograft for the Treatment of Thoracolumbar Spine Fractures.

PURPOSE: To evaluate the clinical outcome and reliability of mixed bone in the process of posterior spinal fusion for the treatment of thoracolumbar spine fractures. In order to eliminate donor site morbidity which is linked to the use of autogenous bone graft, a new surgical procedure mixing autogenous bone chips that were obtained from local decortication of the posterior element of the spine with highly purified calf bone (Lubboc) is studied. MATERIALS AND METHODS: We followed up 9 patients who had undergone autogenous bone graft and 9 patients who had undergone mixed bone graft for more than 12 months out of 33 patients who underwent posterior spinal fusion for thoracolumbar spine fractures between July 1993 and July 1997. RESULTS: Radiographic results indicated solid bony union in 17 of 18 cases regardless of the graft materials. Based on each follow-up evaluation, it was found that the increase in kyphotic angle was related to elapsed time rather than to the type of graft materials. CONCLUSIONS: It appears that the new surgical method of bone graft with highly purified calf bone can be a reliable alternative for achieving stable posterior spinal fusion as long as bone chips large enough to fill the interlamina space can be obtained.

9.Correlation between Saville's index and Bone Mineral Density Measured by the Lateral Dual x-ray Absorptiometry & Clinical Usefulness of Saville's index

For measurement of the degree of osteoporosis, various methods have been used. The Saville's index of the lumbar spine is very simple method. With the object of studying the reliability and clinical usefulness of Saville's index, authors compared the Saville's index with anteroposterior(A.P.) and lateral Dual x-ray absorptiometry, and the Singh's index with the anteroposterior D.X.A. We took the D.X.A. and simple lumbar spine lateral view in 124 patients in same time and also performed D.X.A. and both hips anteroposterior views in 112 patients, then analyzed the correlation between the A.P. and lateral D.X.A., Saville's index and D.X.A., Singh's index and D.X.A. The results were as follows. 1. The correlation coefficient of the A.P. and lateral D.X.A. was 0.46 and appeared the low degree of correlation. 2. The correlation coefficient of the A.P. and lateral D.X.A. compared with Saville's index were 0.68 and 0.83, respectively. Saville's index appeared the higher correlation in lateral D.X.A. than A.P. D.X.A. 3. The correlation coefficient of Singh's index and D.X.A. was 0.84, appeared nearly the same results of Saville's index with lateral D.X.A. 4. Intrapersonal differences were 36% in Saville's index, 28% in Singh's index. Saville's index appeared higher intrapersonal error than Singh's index. The lateral D.X.A. is more accurate and precise method than A.P. D.X.A. because lateral D.X.A. reduces the influence of soft tissue calcification and degenerative changes of spine. As Saville's index is a grading method based on lateral view of lumbar spine, we think that it should be compared with the lateral D.X.A. Regarding this results, authors suggest that Saville's index is not any accurate measurement of bone quantity, but has clinical usefulness for evaluation of osteroporosis.
