A study for seroprevalence of Hepatitis B virus markers (HBsAg, Anti-HBs, HBeAg and Anti-HBe) by EIA method and the Liver Function Test(SGOT and SGPT) using kinetic method was carried :out from January; to October 1995 :among male workers of a .major iron and steel manufacturing company, located,,in Pohang. The results were as follows; 1. The distribution of hepatitis B virus markers showed that the proportions of HBsAg(+)/Anti-HBs (-), HBsAg(-)/Anti-HBs (+) 'and HBsAg(+)/Anti-HBs (+) were 8.1%, 60.5% and 0.1%, respectively. The positive rate of HBsAg in all subjects was 8.2% whereas that of anti-HBs was 60.6%. 2. The proportions of HBsAg(+)/Anti-HBs (-) and HBsAg(-)/Anti-HBs (+) increased significantly with age. While the distribution of hepatitis B virus markers exhibited no significant differences among the groups classified by educational background or work type. 3. The proportion of drinkers in HBsAg (+)/Anti-HBs (-) (73; 9%) was significantly lower than those of HBsAg (-)/Anti-HBs (-) and HBsAg (-)/Anti-HBs (+) (84.6% and 85.7% respectively). The proportions of smoking for the three groups of HBsAg (-)/Anti-HBs(+),-HBsAg (+)/Anti-HBs (-) and HBsAg (-)/Anti-HBs (-) were 54.3%, 53.7% and 53.0%, respectively (p<0.01). The proportion of the subjects with abnormal liver function test in HBsAg (+)/Anti-HBs (-) was 40.1%, whereas those in HBsAg (-)/Anti-HBs (-) and HBsAg(-)/Anti-HBs (+) were 21.9% and 18.4%, respectively. 4. The distribution of e Ag markers among HBsAg (+) subjects showed that the proportions of HBeAg(+)/Anti-HBe(-), HBeAg(-)/Anti-HBe(+) and HBeAg(+)/Anti-HBe(+) were 31.3%, 55.0% and 1.6%, respectively. The proportion of HBeAg(+)/Anti-HBe(-) decreased significantly with age and those of HBeAg(-)/Anti-HBe(-) and HBeAg(-)/Anti-HBe(+) increased with age. The positive rate of HBeAg among HBsAg (+) subjects was 32.9%. In conclusion, as a screening method, EIA is. recommended to test hepatitis B virus markers, especially when examining Anti-HBs more sensitively. For the workers of the iron and steel manufacturing company studied, liver diseases caused by other than hepatitis B should be given a special care including an education avoiding alcohol drinking.

5.5-hydroxy-L-tryptophan Suppressed Food Intake in Rats Despite an Increase in the Arcuate NPY Expression.

This study was conducted to define the underlying mechanism of hypophagia induced by increased central serotonergic action. Rats received 3 daily injections of 5-hydroxy-L-tryptophan (5-HTP), a serotonin precursor, at a dose of 100 mg/kg/10 ml saline at 1 h before lights off. A significant suppression in food intake was observed shortly after the 5-HTP injection and persisted during 3 daily 5-HTP injections. Neuropeptide Y (NPY) expression in the arcuate nucleus increased after 3 days of 5-HTP treatment, as high as in the pair-fed group. Immunoreactivity of phosphorylated extracellular signal-regulated protein kinase (pERK1/2) in the hypothalamic paraventricular nucleus (PVN) was increased markedly by 3 days of 5-HTP treatment, but not by 3 days of pair-fed. mRNA expression levels of serotonin reuptake transporter (5-HTT) was increased in the dorsal raphe nucleus of the 5-HTP treated rats, but not in the pair-fed group. Results suggest that increased pERK1/2 in the PVN of 5-HTP injected rats may be a part of serotonergic anorectic signaling, perhaps blunting the orectic action of NPY; i.e., 5-HTP injected rats showed hypophagia despite of increased NPY expression in the arcuate nucleus.

6.The Prognostic Significance of Maximal Precordial ST-Segment Depression in Patients with Acute Inferior Myocardial Infarction.

BACKGROUND: Inferior myocardial infarctions account for 40-50% of all acute myocardial infarctions and are generally viewed as having a more favorable prognosis than anterior wall infarctions. However, nearly 50% of patients suffering inferior infarction will have complications such as heart block, concomitant precordial ST-segment depression and right ventricular infarction or distinguishing features associated with an increased mortality that will substantially alter an otherwise favorable prognosis. METHODS: Clinical characteristics, electrocardiograms, and angiographic findings of coronary artery were viewed in 47 patients with inferior myocardial infarction. Significant ST-segment change was defined as > or =0.1mV horizontal or down sloping depression in acute inferior myocardial infarction patients with ST-segment elevation on leads II, III, aVF measured with reference to the TP segment, 80ms after J point. A group=no precordial ST-segment depression. B group=sum of ST-segment depression in leads V1 to V3 equal to or more than the sum of ST-segment depression in leads V4 to V6. C group=maximal precordial ST-segment depression in leads V4 to V6. RESULTS: 1) Incidence of multi-vessel disease in group A and in group B were 23% and 22%, respectively, compared with 60% for those of group C(p<0.05). 2) Incidence of Killip class III and IV rates in patients with group A and B were 8% and 12%, respectivly, compared with 47% for those patients with group C(p<0.05). 3) No significant difference of sex, age, peak CK level were found among three groups(p>0.05). CONCLUSION: The patients with acute inferior myocardial infarction with the maximal ST-segment depression in leads V4 to V6 would be at high risk for congestive heart failure and multi-vessel disease.

7.The differences of opinion on Boramae incident between physiscians and common people.

BACKGROUND: Without available medical policy, up until now discharges against medical advice have occurred in th field of medicine in Korea. Seoul district court had convicted two physicians of crime because they had allowed a patient to discharge against medical advice, referred to as the Boramae incident in May of 1998. This shocked people in medical field as well as people and appeared in almost all newspapers and created a debate especially to physicians. The purpose of this study was to evaluate the differences of opinion of this incident between physicians and common people. METHODS: Ninety-nine physicians and ninety-nine common people were questioned from September 1 to September 30, 1998 about sociodemographic characteristics and opinion of this event. RESULTS: The mean age of the physician group, the non-physician group was 38.12+/-11.2, 38.5+/-11.1, respectively. Each group consisted of 87.9 percent, men and 12.1 percent, women. There were no differences between physicians and common people in experiences of discharges against medical advice in their families, decisions on Boramae-like incidents that could occur in the near future, and causes of discharges against medical advice. The physician group had more awareness rate of this incident than the non-physician group, and insisted more on the needs of methods to prevent recurrence. In opinion of the judgement, 5.1 percent of non-physician group agreed to conviction of Seoul district court. 22.2 percent of the non-physician group was of the opinion that this incident will affect patient care positively, about patient wifes decision, 21.2 precent of non-physician group considered that she made an imprudent decision. CONCLUSION: There were significant, differences between physicians and common people in the awareness of this incident, opinion on th judgement, needs of methods to prevent recurrence, the influence of this incident on patient care in the future, opinion of patient wifes decision, exceptional experiences of discharges against medical advice in a family, decisions on Boramae-like incidents that can occur in the near future, and causes of discharges against medical advice.

8.Studies on the Systolic Time Intervals in the Patients with COPD.

In an attempt to clarify the relationshop between left ventricular function and chronic obstructive pulmonary disease(COPD), systolic time intervals were measured in 79 control subjects and 60 patients with COPD who had no evidence of organic heart disease. The patients with COPD were divided into three groups based on the percent predicted forced expired volume in one second. Significant differences in pre-ejection period index(PEPI) and left ventricular ejection time index(LVETI) existed between the control subjects and the patients with the most severe lung disease(Group III). A highly significant difference in PEP/LVET existed between these two groups. The abnormalities of systolic time intervals demonstrated in these patients are characteristic of left ventricular dysfunction and indicate that left ventricular dysfunction is frequently present in patients with moderate obstructive lung disease.

Hemichorea-hemiballsim has been reported as a rare Implication of nonketotic hyperglycemia. Recently, Some reports revealed that brain magnetic resonance images of nonketotic hyperglycemic patients with hemichorea-hemiballism showed characteristic contralateral striatal signal abnormality, We present six patients with hemichorea-heniiballism. All of them had diabetes mellitus and performed brain MR images. The MR images of them showed high signal intensity on Tl-weighted images and low signal intensity on T2 weighted images in the striatum contralateal to the involuntary movement. In general, the striatal high signal intensity on TI weighted images are presumed to have developed following hemorrhagic transformation, osmotic myelinolysis, or mild ischemia with reversible deposition of calcium or other material occurred in association with nonketotic hyperglycemia. However the precise mechanism of this signal change is not clear yet and needs pathological confirmation.

10.The Significance of the Precordial ST: segment Depression in Acute (V1- V3) Inferior Myocardial Infarction.

BACKGROUND: Reciprocal ST-segment depression in precordial leads is a common finding in acute inferior myocardial infarction. The responsible mechanism and the significance of this finding, however, are still controversial. METHODS: From January 1991 to December 1994, 38 patients with acute inferior myocardial infarction were treated at the Department of Internal Medicine in Kyung Hee University Hospital. Clinical characteristics, serial electrocardiograms, and angiographic findings of coronary artery and left ventriculography, echocardiography were reviewed. Reciprocal ST-segment depression was defined as ST-segment depression>or=1.0mm in two or more adjacent precordial leads(V1-V3) in patients with acute inferior myocardial infarction showing ST-segment elevation in II, III, aVF. Coronary angiography and echocardiography were performed within 24 hours from admission. In this study, angiographic distribution score was used to define the perfusion territory causing inferior ischemia. RESULTS: 1) The summation of ST-segment elevation in II, III, aVF leads was significantly more higher in group H than group A (P<0.05). 2) However, the angiographic distribution score, peak CK levels, in-hospital complications and ejection fraction showed no difference between two groups (P>0.05). 3) There were no evidence of anterior wall motion abnormality in two groups. CONCLUSIONS: Our results suggest that the S'I'- segment depression on the precordial leads in acute myocardial infarction can be explained mainly by benign reciprocal electrical change.
