1.The Comparison of Rigid and Semi-rigid Rod for the Correctability of the Thoracic Hypokyphosis of the Idiopathic Scoliosis.

STUDY DESIGN: The analysis was based on the sagittal and coronal correction of 64 cases of thoracic hyphokyphosis that were due to idiopathic scoliosis. PURPOSE: We wanted to compare the three pedicle screws and rod systems (using either a semirigid and rigid rod) from the view points of the coronal and sagittal plane correction during the surgical treatment of idiopathic scoliosis. SUMMARY OF LITERATURE REVIEW: There haven't been any reports that have compared the differences and their significance between the semirigid and rigid rod systems for correcting the thoracic hypokyphosis of idiopathic scoliosis. MATERIALS AND METHODS: After a minimum follow-up of 1 year, the results of the frontal and sagittal correction of each study group were compared. There were a total of 52 females and 12 males. The mean age at the time of surgical intervention was 16.4 (age range: 12~24). CD instrumentation with a rigid stainless steel rod (7 mm in diameter and 1200 N/mm for its stiffness) was used in group I (36 patients), and 4-CI instrumentation with a titanium alloy rod (6.35 mm in diameter and 1200 N/mm for its stiffness) was used in group II (18 patients). Xia instrumentation with a semi-rigid rod (6 mm in diameter and 600 N/mm for its stiffness) was used in group III (10 patients). For all patients undergoing the segmental pedicle screw fixation procedure, pedicle screws were inserted into every vertebra on the concave side, in the end of each vertebra and then alternately in every other vertebra on the convex side. We measured the preoperative and postoperative magnitude of the major and compensatory curves, the thoracic kyphotic angle on the standing radiographs and the flexibility of the curve. We statistically compared the correctability of thoracic kyphotic angle between the rigid and semi-rigid rods. RESULTS: The differences of preoperative and postoperative major curves in groups I, II and III were 47.3 degrees/12.7 degrees, 51.6 degrees/12.3 degrees and 49.6 degrees/13.3 degrees respectively. The thoracic kyphotic angle was 22.6 degrees/26.6 degrees, 22.0 degrees/26.9 degrees and 23.8 degrees/22.8 degrees, respectively. There was no significant difference for the correction of the coronal angle in each group, but groups I and II were superior to group III for the rate of correcting the kyphotic angle. CONCLUSION: The rod should be stiff enough to pull the spine posteromedially when using the derotation technique for the surgical treatment of idiopathic scoliosis.

The Department of Dermatology at Kyungpook National University School of Medicine, initially the Dermatology Department of the Private Daegu Medical Training School in 1924, commemorated its 100th anniversary in 2023.Throughout its rich history, the department has been dedicated to advancing contemporary dermatology and enhancing the well-being of local communities through exceptional research, education, and social contributions.Initially tasked with the diagnosis, treatment, and education of patients with dermatological and urological disorders, the department evolved over time. In 1958, it separated from the Urology Department to focus solely on the diagnosis, research, and education of dermatological disorders. In 1962, Dr. Soon Bong Suh became the inaugural Chief of the Dermatology Department, marking the foundation of the Kyungpook National University School of Medicine’s Dermatology Department. Our legacy of pioneering contributions extends to dermatophytic diseases, which account for a large proportion of skin disorders in Korea, where we have excelled in basic research and treatment. These achievements have played a crucial role in the advancement of dermatophytic diseases.Subsequently, we have demonstrated expertise in blistering diseases and dermatopathology, making substantial contributions to the diagnosis and treatment of refractory skin conditions. Recently, we have been at the forefront of research and interventions for chronic inflammatory skin conditions that cause significant distress in many individuals, including atopic dermatitis and psoriasis. Our influence has also extended to severe and refractory skin disorders, including skin cancer and vascular anomalies. Striving to stay contemporary, our achievements now encompass aesthetics, including acne, pigmentation, and laser treatments. Our accomplishments have garnered widespread recognition nationwide, engendering high expectations for future advancements.
3.A Dual (Brachial and Contralateral Femoral) Approach for Subintimal Angioplasty of Long Ilio-Femoral Occlusive Disease Including the Iliac Ostium.

As techniques and device technology have improved, the success rates and long patency of ilio-femoral occlusive disease have also improved. In the case of extensive iliac occlusive disease, however, wire passage and handling remain a challenge due to the relatively weak guiding catheter backup support with the contralateral femoral approach. There has been no report on methods to overcome this problem. We performed a successful percutaneous translunimal angioplasty for long ilio-femoral occlusive disease including the iliac ostium by a dual approach including simultaneous brachial and contralateral femoral arteries for subintimal angioplasty.

6.Diagnostic Process of Subcorneal Pustular Dermatosis: A Case Report

A 54-year-old man presented to our outpatient clinic with generalized pruritic millet-sized vesicles, pustules, and crusts on the whole body over the past 10 years, which were more dominant in the lower extremities. Due to the difficulty in diagnosis, a series of histopathologic examinations were conducted during the treatment course, and the findings were similar: subcorneal pustules with neutrophils, superficial perivascular and dermal infiltration of lymphocytes, neutrophils, and eosinophils, with no sign of acantholysis. The patient was treated with cyclosporine, prednisolone, doxycycline, colchicine, sulfasalazine, and acitretin; however, his condition did not improve. After showing a dramatic improvement with dapsone, he was finally diagnosed with subcorneal pustular dermatosis (SPD).Various medications commonly used in inflammatory and immunobullous skin diseases were tried but failed to improve the condition; the patient showed a dramatic response only to dapsone. Due to its rarity, careful attention and repeated biopsies are required for diagnosing SPD.
7.Cutaneous Nontuberculous Mycobacterial Infection Misdiagnosed as Rheumatoid Nodule in a Patient with Rheumatoid Arthritis

Owing to advances in diagnostics and the increase in invasive procedures, and immunocompromised patients, cutaneous nontuberculous mycobacteria (NTM) infection is rising. NTM should be suspected in patients with persistent skin lesions refractory to treatment with a history of immunosuppression or skin injury. A 59-year-old woman presented with a 4-week history of multiple erythematous tender nodules on left arm. A year prior, multiple nodules appeared on left hand dorsum, followed by recurrent suppurative nodules in left arm. She has been taking methotrexate and leflunomide for 7 years due to rheumatoid arthritis (RA). Skin biopsy revealed granulomatous inflammation, and NTM polymerase chain reaction test was positive. Furthermore, she had cut her left finger with a knife 14 months ago. Based on these findings, cutaneous NTM infection was diagnosed. Herein, we report a case of cutaneous NTM infection in an immunosuppressed patient with RA, emphasizing differentiating subcutaneous nodules from rheumatoid nodules in RA.
8.A Case of Recurrent In-Stent Restenosis with Abundant Proteoglycan Component.

A percutaneous coronary intervention (PCI) is known to be one of effective methods in the treatment of coronary artery disease. However, restenosis remains a major limitation to a PCI. Although neointimal cell proliferation is suspected to be the major cause of coronary stent restenosis, few histological characterizations of recurrent instent restenosis exist. We report a case of a 61-year-old man suffering from unstable angina due to secondary coronary in-stent restenosis in the proximal left anterior descending artery (LAD). An atherectomized tissue, obtained by a directional coronary atherectomy, showed myxoid tissue, characterized by a few stellate smooth muscle cells in the abundant extracellular matrix, which was blue-colored proteoglycan on modified Movat staining.

9.The effects of irradiation and hyperbaric oxygen therapy on microvascular anastomosis.

Malignant tumors of the head and neck frequently require treatment with both radiotherapy and surgery. Reconstruction of the defect in previously irradiated field is a challenge to surgeon, who must produce both a functional and an esthetic result. Hyperbaric oxygen therapy(HBO) has been used in an attempt to reduce the deleterious effects of radiation. But the issue of whether prior irradiation and HBO of the recipient site of a free flap affects the result of reconstruction continues to generate controversy. So, the effects of irradiation and hypergbaric oxygen therapy on microvascular anastomosis was evaluated in an experimental study in femoral vessels of rats. The experimental groups were divided into 3 groups, contorol group, irradiation group, and irradiation and HBO group. Preoperative irradiation was delivered in the left groin field with single dose corresponding 2,000cGy and total 48 hours of HBO was given 100% oxygen at 2.4 atmosphere for 4 weeks. The femoral vessels of 60 rats were anastomosed after irradiation and HBO treatment. Three days, 1 week, 2 weeks and 4 weeks after surgery, the femoral vessels were evaluated for patency and histopathologic changes. There was no notable effect of irradiation on patency of femoral vessels in rats and the radiation effects were obvious on histological examination which showed the sloughing of the endothelial cells, subintimal hyperplasia and fibrosis on the media and adventitia of femoral arteries. The histologic changes of the femoral veins were mild and not typical. But the effects of hyperbaric oxygen therapy after irradiation was seen not marked difference in irradiation group.

10.Viability of Refrigerated Human Articular Cartilage.

PURPOSE: To determine the viability of human chondrocytes within refrigerated articular cartilage stored under conditions currently used clinically. MATERIALS AND METHODS: Osteochondral sections of human ankle taluses were stored at 4 degrees C in DMEM media for 1 to 42 days. Articular cartilage was harvested and evaluated for histologic changes and proteoglycan synthesis. RESULTS: By day 7, markedly decreased proteoglycan synthesis was observed. After 21 days, synthetic activity was virtually undetectable. Histologic specimens demonstrated chondrocyte death of a half of the cells from the superficial layer at day 7. Within 21 days, significant chondrocyte death was seen. CONCLUSION: These results suggest that proteoglycan synthetic activity and chondrocyte viability are markedly decreased in articular cartilage after cold storage for longer than 7 days.
