1.Comparison of Intravaginal Misoprostol and Prostaglandin E2 Vaginal Tablet in Termination of Mid - Trimester Pregnancy.

OBJECTIVE: To compsre the efficiency, success rate, and abortion time of applications of intravaginal misoprostol versus prostaglandin E2 vaginal tablet for mid-trimester pregnancy termination Subjects and methods: Eighty four patients between 17-29 weeks of gestation with medical, obstetric, or genetic reasons far termination were randomized to receive either 50 ug tablets of misoprostol placed in the posterior vaginal fornix or prostaglandin E2 3mg in tables placed into the endocervix. RESULTS: Among eighty four patients recruited, fourty five patients received misoprostol and thirty nine patients received prostaglandin E2 vaginal tablets. The average interval hom start of induction to vaginal delivery was 13.35 +/- 3.34 hours in misoprostol poup and 19.14 +/- 10.64 hours in the prostaglandin E2 group. The success rate of complete termination within 12 and 24 hours in misopr-ostol group were 57.7%, 93.3%, respectively, while in prostaglandin E2 group were 20.5%, 82.1% repectively. Oxytocin augumentation was 6.7% in misoprostol group and 17.9% in the prostaglardin E2 group. No serious complication occumd. CONCLUSION: Intracervicovaginal misoprostol appears to be acceptably safe and effective agents for second trimester pregnancy termination. The abortion time is less in misoprostol group than those in the prostaglandin E2 group. Misoprostol has the advantage of being expensive, easily stored and readily available. We used 50 ug tablets of misoprostol every four hours. But, we suspect that the regimen of 100ug misopostol inserted intracervico-vaginally every eight hours will beis the proper and optimal method for pregnancy termination.

2.Muscle Transposition and Skin Graft for the Coverage of Exposed Bone

The technique of muscle transposition has gained an accepted place in treating the wide skin defect with bony exposure in which conventional palliative care has rarely been successful. The principle of this procedure, popularized by Ralph Ger and significantly contributed by Bakajian, has been approved by many and now been adapted in the care of similar lesion throughout the entire body regions. This report comprises the results of muscle transposition experienced in 28 patients. The results were as follows: 1. Twenty-four cases of antetibial skin defect with bony exposure, resulted from chronic osteomyelitis, open fracture, chronic ulcer, and burn scar, were successfully managed with the transposition of single muscle or combination of them: medial head of the gastroenemius, the soleus, the flexor digitorum longus and the abductor hallucis. The extent and the level of the defect in each case were the sole indication of selection of the appropriate muscle and the number. 2. Four patients with decubitus ulcer in the sacral region were also managed by transposing the upper half of the gluteus maximus. 3. Subsequent skin coverage over the transposed muscle were uniformly successful in all cases and this technique seemed to afford a definite favorable influence upon the healing of fracture and the eradication of infection.

6.Is Immunoglobulin Increased in Perthes' Disease?

There have been a few repots that serum immunoglobulins are increased in Perthes' disease suggesting possible immunological involvement in the pathophysiology of the disease. Joseph(1991) suggested that the raised IgM concentrations might be related to complications of Perthes' disease such as striking reduction of movement, fixed deformities and striking reduction of movement, fixed deformities and chondrolysis and that immunological mechanisms might be implicated in the mediation of some of the changes in Perthes' disease. More studies are necessary to prove the results. The purpose of this study is to detect if there is a true increase of serum immunoglobulins in Perthes' disease.

7.Heart Rate Variability in Mitral Valve Prolapse Syndrome.

BACKGROUND AND OBJECTIVES: Studies in patients with mitral valve prolapse syndrome (MVPS) have shown the coexistence of various forms of autonomic dysfunction, and so this is an area that requires further investigation. METHODS AND MATERIALS: The study group consisted of 65 patients (36 men and 29 women), aged 16 to 43 years (mean+/-SD, 28+/-8) with symptomatic, echocardiographically proven mitral valve prolapse (MVPS) who were free of other organic heart diseases and arrhythmias. In a prospective study, heart rate variability (HRV) indexes were calculated from 24-hour Holter recordings obtained during normal daily activity and plasma norepinephine was measured, and then these data were compared among the study group according to clinic heart rate (HR). RESULTS: The study group was divided into below 60 bpm (group 1: n=13), 60 - 80 bpm (group 2: n=36) and over 80 bpm (group 3: n=16) on the basis of clinic HR. These patient groups were matched with respect to age and gender. There were significant difference in SDNNIDX, rMSSD and pNN50 between these groups (p=0.005, 0.009 and 0.002 respectively), and these HRV measures correlated inversely with clinic HR (p<0.01). As clinic HR increased, there was a tendency for plasma norepinephrine concentration to also rise (p<0.05), but there was no statistical significant difference between these groups. CONCLUSION: Our data suggests that MVPS may have subgroups of HRV, reflecting cardiac autonomic modulation, according to clinic HR.
