1.A Case of Multiple Bowen' s Disease in a Leprosy Patient.

We report a case of multiple Bowen's disease associated with leprosy. A 68-year-old man was seen because of slightly pruritic multiple erythematous scaly patches and plaques on the trunk and the both extremities for 5 years. He has been treated for leprosy with many kinds of drugs consisting of the herb medications for over 40 years. He also showed decreased cell-mediated immunity in skin tests. It is considered that depressed cellular immunity may be an important predisposing factor in the development of mutiple Bowen's disease in this case, .although arsenics can not be completetly excluded.

2.Livedo Reticularis Idiopathica Associated with Mononeuropathy Multiplex Syndrome and Bilateral Ulnar-median Nerve Anastomosis.

A 31-year-old woman had a livedo reticularis of reticulated, bluish discoloration on both arms and legs for 18 months with a burning pain in the right hand and numbness in both lower legs. The findings of the electromyography were consistent with mononeuropathy multiplex and bilateral ulnar-median nerve anastomosis. A biopsy specimen from the right sural nerve showed perivascular lymphocytic infiltration and occasional myelin digestion chambers, which were consistent with vasculitic neuropathy. We present a patient with livedo reticularis idiopathica associated with mononeuropathy multiplex syndrome who also has bilateral ulnar-median nerve anastomosis.

3.The Effect of Minizide(R) in the Hypertensive Patients.

The effect of Minizide(R) (prazosine 1mg and polythiazide 0.5mg) was studied in 34 hypertensive patients (female 16 cases, male 18 cases) at the Department of Internal Medicine, Paik Hospital, In-Je Medical College between July, 1979 and August 1980 for 14 months. The patients didn't take the any drugs for one week before the clinical trial with Minizide(R), and thereafter they took minizide 1 tablet 3 times daily for 2 weeks. The blood pressure was checked before and after the trial in the supine and standing position. The results were as followings: 1) The systolic blood pressure before the treatment with Minizide in the supine position was 196+/-24mmHg and in the standing position 188+/-22mmHg. The systolic blood pressure after the treatment with minizide in the supine position was 148+/-16mmHg and in the standing position 139+/-9mmHg. 2) The diastolic blood pressure before the treatment with minizide in the supine position was 118+/-11mmHg, and in the standing position 113+/-10mmHg. The diastolic blood pressure after the treatment with minizide in the supine position was 87+/-6mmHg and in the standing position 84+/-5mmHg. 3) There was no significant orthostatic hypotension after the 34 patients. 4) There was no changes of laboratory findings after the treatment. 5) 3 of 34 hypertensive patients after the treatment complained of headache, 3 palpitation, 2 edema, 2 drowsiness, and 1 fatigability and 1 patient complained of weakness.

4.Evaluation for the sensitivity of LISS in antiglobulin test.

The association rate of antibody with antigen has been reported to be greatly increased by lowering ionic strength. Low-ionic strength salt solution(LISS) method has been used for the detection of various alloantibodies. To investigate the sensitivity of LISS in indirect antiglobulin test, a comparison study of LISS with saline and albumin methods was conducted. A total of 32 patients' samples requested for indirect antiglobulin test were evaluated. Out of 32 patients with clinical immune hemolytic anemia, 11(34.3%) were positive in 37 degrees C saline antiglobulin test, 18(56.2%) in albumin antiglobulin test, 23(71.8%) in LISS antiglobulin test respectively. These results were statistically analyzed using non parametric Page's test for ordered alternatives. LISS method is more sensitive than 37 degrees C saline method or albumin method significantly (p<0.01).

6.A Case of Cutaneous Metastasis of Male Breast Cancer.

Male breast cancer is a rare disease, representing approximately 1% of breast cancer and less than 1.5% of malignant tumors occurring in men. We report a case of cuaneous metastasis of male breast cances. a 64-year-old male who presented with multiple nodules and erythematous plaque on the right anterior chest and enlargement of the right axillary lymplinodes. The histopathologic findings of the nodule showed atypical tumor cells with a somew eat gland-like pattern. The cells showed potive reactions in PAS, DPAS and alcian blue stains, and also positive for estrogen and roiesterone receptors. Biopsy specimens taken from the plaque showed cellular arrangement in show line, referred to as Indian filing, between thiclenecin collagen bundles. He was treated not, tamoxifen, 5-fluorouracil, adriamycin, cyclophosphamide and megestrol acetate. 5 months after nitiation of the treatment, the skin lesions were improved vvith a decreased number and size or dules and decreased intensity of erythema.

7.Evaluation of Tumor Invasion in Gastric Carcinoma with CT Using Water as an Oral Contrast Agent in Prone Position.

PURPOSE: To evaluate the accuracy of CT using water as an oral contrast material in a prone position in determining the depth of tumor invasion in gastric cancer patients. MATERIALS AND METHODS: Thirty three patients(19 male, 14 female) with surgically confirmed gastric cancer were studied. We performed CT in a prone position after ingestion of 1 liter of pure water. CT findings were classified into 4 groups by the morphologic appearances of infiltrates in the perigastric fat plane :normal perigastric fat(SO), fine mottled densities(S1), irregular aggregated or linear densities(S2) and direct extension and invasion of tumor into contiguous structures(S3). Also we prospectively compared the CT staging with pathologic T staging according to the TNM systems. RESULTS: The overall accuracy of CT staging in determining the pathologic T factor was 69.6%. As we regarded T1 and T2 lesions as one group on CT, the accuracy of CT staging was increased to 80.2% because of a limitation of CT for distinguishing T1 from T2 lesions. CONCLUSION: Prone position CT using water as an oral contrast agent is quite accurate in determining the T staging of gastric carcinoma.

8.A Case of Papulonecrotic Tuberculid Combined with Erythema Nodosum.

We report an extremely rare case in which two tuberculids, papulonecrotic tuberculid and erythema nodosum, occurred together. A 16-year-old girl had two types of clinically distinct lesions; asymptomatic necrotic papules on the upper and lower extremities and painful subcutaneous nodules on the legs. A Mantoux test showed a strong positive reaction. Histopathologic findings of the necrotic papule and subcutaneous nodule were consistent with papulonecrotic tuberculid and erythema nodosum, respectively. There was no extracutaneous focus of tuberculosis. Polymerase chain reaction for tuberculous bacilli in the papulonecrotic tuberculid lesion revealed a negative result. Antituberculous therapy brought a rapid clinical improvement of both lesions.

10.Radiologic evaluation of globus symptom

The globus symptom is a condition in which a patient, often middle aged women, complains of a lump andchocking sensation in the throat, Functional disturbance of the cricopharyngeal muscle, rendering it incapable ofrelaxing during swallowing, has long been recognized as a cause of globus symptom and dysphagia. We wanted to findout how often and to what extent distrubed relaxation of the cricopharyngeal muscle can be seen in patients withglobus symptom with routine examination and video esophagogram. The results were as follows: 1. Male : femaleratio was 1:2.4. 2. Globus symptom was most frequent in the age group between 30-39 of female. 3. Organic lesionswere seen in 43.6%(24 Pts) of globus patients. 4. Cricopharyngeal muscle was visualized in 29.1%(16Pts), esophageal diverticulum in 3.6%(2 Pts) and degenerative spondylosis in 3.6%(2 Pts). 5. Incidence of visualizationof cricopharyngeal muscle were higher in male group (50%) than female one (20.5%). 6. Cricopharyngeal muscle wasmost frequently visualized on early swallowing phase(12/16 Pts).
