1.A Case of Epidermolysis Bullosa Dystrophica.

A case of Epidermolysis Bullosa Dystrophica is reported with review of pathogenesis with literatures. Epidermolysis Bulloaa Dystrophica is a rare genetic disease which is characterized by the formation of blister upon the minor trauma or friction to the skin. A 7-year old boy was referred to our dept. in Aug. 1976 because of a severe bullous eruption primarily on his hands and feet. On admission tense bullae were noted on the dorsa of hands and feet which is usually serous but may be hermorhagic tendency. Soon thereafter tbis bullous lesion tend to break down into painful shallow denudation which may leave depigmented scars and milia after healing. Family history was negative for skin disorders.

2.Clinical Trials of Frason on Various Dermatosis.

Esperson(R), a new topical steroid ointment containing 0. 05% 17-Desoxymethasone,. was used in several kinds of dermatoses. Framycetin which was isolated and uientified by the research organisation of Roussel Laboratories (SOFRAPEN) in 1950 from culture broths of a strain of Streptomyces species is a wide spectrum antibiotics on various bacterial infectious diseases. Total 50 patients including 10 cases of 2ndary infected Atopic dermatitis, 10 of infected Contact dermatitis, 6 of infected Seborrheic dermatitis, 7 of infected Neurodermatitis, 3 of infected Chronic dermatitis, 5 of lmpetigo, 2 of infected Nummular eczema, 2 of infected Infantile ecze;na, 1 of infected Intertrigo, 1 of Eczematoid Otitis extema, 1 of 2nd degree Bum, 1 of infected Pruritis ani, 1 of infeeted 1nsect bite. were treated with Frason(R) (0.05% 17-desoxymethasone & 0.5% Framycetin combination). Among these 50 patiants, we observed excellent effects in 7 patients (14%), good effect in 29 patients (58%), fair effect in 10 patients (20%), Totally 36 patients (72%) revealed excellent to good therapeutic effects on their skin conditione after unoccluded application of Frason(R). 3 times daily for about 14 days. There was no sid ffect in all subjects after application of Esperson-F.

6.The Preventive Effect of Pentoxifylline on the Oleic Acid-Induced Lung Injury.

BACKGROUND: Although exact mechanism of adult respiratory distress syndorme (ARDS) is not known, many evidences suggest that inflammatory mediators such as protease and oxygen radicals may be the final common pathway to the endothelial injury. It is gradually accepted that activated neutrophils play a major role in the process. Pentoxifylline is a methylated xanthine with a wide range of actions that make it theoretically useful in the treatment of ARDS. Studies show that it decreases neutrophil phagocytosis and superoxied production. The aim of this experiment is to determine whether pentoxifylline has a preventive effect on oleic acid-induced lung injury. METHODS: Oleic acid (0.08 ml/kg) was infused in 10 mongrel dogs in order to induce acute lung injury. Pentoxifylline (20 mg/kg) was administered 40 mins before oleic acid injection. The author made a comparative studies on the effects of pentoxifylline on hemodynamics and gas exchange before oleic acid injection and at 15, 30, 45, 60, 90 mins after oleic acid injection. RESULTS: There were not significant changes in mean arterial pressure, cardiac output, systemic vascular resistence, pulmonary wedge pressure intrapulmonary shunt in both groups. There were statistical significance in arterial oxygen tension, pulmonary artery pressure, pulmonary vascular resistance between both groups (p<0.05). CONCLUSIONS: Pentoxifylline is a noteworthy drug that could be a candidate as a therapy to help prevent effect in lung injuries that share a common mechanism with oleic acid-induced lung injury.

7.Heat Treatment Suppressed the Expression and Activity of Matrix Metalloproteinases in the Primary Cultured Astrocytes.

Matrix metalloproteinases (MMPs), which is a kind of Zn-dependent enzyme, are a family of proteolytic enzymes that can degrade the extracellular matrix and then play important roles in the pathophysiology of ischemia/reperfusion, especially MMP-2 and MMP-9. This study was aimed to demonstrate how heat treatment and Hsp70 regulate expression of MMP-2 and MMP-9. The expression and regulation of MMPs after ischemic-like injury with and without heat treatment were investigated in the astrocytes expressing Hsp70 or LacZ and mock infected cells. Primary astrocyte cultures were prepared from ICR mice and infected with retroviral vectors overexpressed Hsp70 or LacZ. After heat treatment, expression of MMP-2 mRNAs was not remarkably changed in heat moc cells. Otherwise, expression of MMP-9 mRNAs was significantly reduced by heat treatment. Expression of MMP-2 and MMP-9 in astrocytes, which were treated with ischemic injury after heat treatment, was obviously decreased than in untreated Moc cells. Moreover, Hsp70s were significantly synthesized in response to heat treatment. Compared to amount of protein expression of MMP-2 and MMP-9, the expression of MMP-2 in astrocytes expressing Hsp70, LacZ and mock infected were decreased after heat treatment, especially pro-form of protein expression of MMP-2. However, the expression of MMP-9 were decreased only in the astrocytes expressing Hsp70 by heat treatment and OGD injury after heat treatment, not shown a big change in the LacZ cells and mock infected cells. The results of activity study with MMPs protein were that MMP-2 protein activity was reduced but not in condition of ischemic injury after heat treatment. On the other hand, the activity of MMP-9 after heat treatment was similar to the results of activity of MMP-9 adding ischemic injury. In this study, we shown that Hsp70 overexpression following heat treatment reduced the expression of proMMP-2, proMMP-9 and processed MMP-2, especially. This findings can suggest a possible role of Hsp70 induced by heat treatment for regulation of MMPs and neuroprotection in ischemic injury.

8.Proper allergen selection for serum specific IgE test in children.

Identifying allergic sensitization is important for the proper diagnosis and management of allergic diseases. Skin prick testing and measuring serum specific IgE antibodies are standard tests to confirm allergic sensitization. Skin prick testing has many advantages, but it is difficult to perform on young children. Serum specific IgE can be easily performed on young children, but it can be performed only up to 12 antigens in patients younger than 6 years due to insurance coverage. Therefore, it is important to select appropriate antigens for the patients. Appropriate allergens may vary depending on the patient's age, accompanying disease, and residential area, so that detailed history taking is essential for proper allergen selection. In this review, we will discuss considerations for selecting appropriate allergens for clinicians when performing an allergy test on difficult-to-test children under 6 years of age.
