1.Ultrastructural localization of 28 kDa glutathione S-transferase in adult Clonorchis sinensis.

Glutathione S-transferase (28GST) with molecular mass of 28 kDa is an antioxidant enzyme abundant in Clonorchis sinensis. In adult C. sinensis, 28GST was localized in tegumental syncytium, cytons, parenchyma, and sperm tails examined by immunoelectron microscopy. C. sinensis 28GST was earlier found to neutralize bioreactive compounds and to be rich in eggs. Accordingly, it is suggested that 28GST plays important roles in phase II defense system and physiological roles in worm fecundity of C. sinensis.

2.A Study on the Effect of Topical Indomethacin on Sunburn.

It is generally recognized that several chemical mediators are involved in tbe pathogenesis of sunbum; however, the exact mechanism is still to he resolved. Recently, prostaglandin has been found to be one of the inflammatory mediators in sunbum, particularly in the delayed erythema response and several investigators have shown that the nonsteroidal antiinflammatory drugs inhibit the in vivo synthesis of prostaglandin. On the assumption that the nonsteroidal antiinflammatory drugs might exert a beneficial effect on the sunbum through the antiprostaglandin effect, several authors have reported the effect of indomethacin on sunbum. This study was undertaken to evaluate the effect of topical indomethacin on the delayed erythema response induced by artificial sunlamp and sunlight by clinical and histopathological examination. Indomethacin(2. 5% ointment, 2. 5% solution, 5% solution) was applied immediately after 3 MED irradiation of artificial sunlamp to the shaved back skin of 6 healthy albino guinea pigs weighing 550-850gm and 9 human volunteers and the delayed erythema response was observed at 2, 4, 6, 12, 24 and 48 hours after irradiation respectively. Punch biopsies of the skin were performed on 6 guinea pigs at 6, 12, 24 a.nd 48 hours after irradiation and the histologic changes were observed. Four human volunters were exposed t,o 3 MED of sunlight and indomethacin preparations were applied immediately after irradiation. The delayed erythema response was observed at 2, 4, 6, 12, 24 and 48 hours postirradiation and in one volunteer, the skin was biopsied at 24 hour for histologi.c evaluation. The results were as follows: 1. Topical indomethacin in ointment or solution forms delayed and decreased the appearance and degree of erythema induced by sunlight and artificial sunlamp in both human and animal subjects. 2. Histologically, the number of sunbum cells at 24 hour in the epidermis of one human volunteer was decreased on indomethacin applied sites. The dermis showe6 no difference in histologic changes between control and indomethacin sitea. In animal study, however, no detectable difference in epidermal or dermal changes was observed between control and indomethacin applied sites. The autors confirmed that topical indometha,cin was effective in delaying and blanching of the delayed erythema of sunbum in guinea pigs as well as in human volunteers, though there was no difference in histologic changes between indomethacin and control sites in guinea pigs. The mechanism involvel on this aspect is remained to be resolved.

3.Two Cases of Weber-Christian Disease.

Weber-Christian disease is a relapsing, febrile, nodular, nonsuppurative inflammation of the subcutaneous fat tissue. Many authors reported these cases which illustrate the great variability of the clinical manifestation, such as acute or chronic, fulminating or transient, febrile or non febrile, systemic or cutaneous. Two of our cases have had recurrent painful subeutaneous nodules without fever for several years. Biopsy showed a nonspecific inflammatory panniculitis with lymphocytes chiefly, and small amount of histiocytes and foamy cells. It should be suggested Weber-Christian disease when subcutaneous nodules appear recurrently same areas of the whole body surface except anterior aspects of tibia even if no fever as systemic manifestation. Treatment with steroid and chloroquine diphosphete produced good improvement.

4.A Case of perifolliculitis Capitis Abscedens et Suffodiens.

We presented a case of perifolliculitis capitis abscedens et suffodiens associated with acne conglobata in 40-year-old rnale. He had a sausage-shaped plaque containing many fistular tracts on right temnporal area and cicatrizing alopecia on occiput. On the middle portion of upper back he had a adult fist-sized plaque containing many fistular tracts. Histopathologically epidermis showed mild byperkeratosis and keratotic plugging and in the dermis most hair follicles were destroyed and replacel by fibrous tissue but there were moderxte cellular infiltration composed of neutrophils, lymphocytes a.nd hist.iocytes around the remained hair follicles. On serum electrophoresis alpha,-globulin was elevated and gamna globulin was the upper liviit of nornal variation. Skin tests with murnps vaccine, dinitrochloro benzene (DNCB) and old tuberculin disclosed the evidence of de reased c "ll-mediated immunity(CMI). All th se findings suggest thzt immune rriechanisni r,-ay be concerned ivith the pathogenesis in this case. He wa.s treated with th internal administration of antibiotic and corticosteroid and with surgical opening and curettage of fistular tracts

7.A case of adult Wilms' tumor.

Wilms' tumor is malignant renal tumor, originated from undifferentiated mesonephric blastema. Adult Wilms' tumor, unlike that of childhood, is a rare disease and a total of l67 cases have been reported in the world medical literature until 1980. Diagnosis of adut Wilms` tumor is very difficult preoperatively and the pathologic hallmark is the presence of abortive or embryonal glomerulotubular structure with an immature spindle cell stroma. The therapeutic guidelines and surgical principles that govern childhood Wilms' tumor should be applied to adult Wilms' tumor. We reported a case of Wilms' tumor developed in 34 year-old male patient complaining of right flank pain with gross hematuria for 2 months.

8.ESWL for management of steinstrasse.

Early in the experience of ESWL, steinstrasse was recognized as potential complication and suggestions for management were proposed. including retrograde ureteral manipulation. placement of ureteral stent or percutaneous nephrostomy, etc. Recently ESWL has been challenged due to less invasive and safer procedure. We experienced 18 cases (2.1%) of steinstrasse in 850 patients with ESWL (EDAP LT-01) from February, 1989 to February, l99l and treated with ESWL. The results were obtained as follows. 1. The primary stones in 18 cases of steinstrasse were renal stones in l7 cases (complete staghorn calculi; 4 cases. partial staghorn calculi; 5 cases) and upper ureteral stone in 1 case: 2. The sites of steinstrasse were lower ureter in 13 cases and upper ureter in 5 cases. The lengths of steinstrasse were 1.4~8.3cm (mean: 3.19cm). The site and length of steinstrasse were not related with clinical symptoms. 3. Of l8 cases. 13 cases were type 1. 3 cases type II and 2 cases type III. Type of steinstrasse was not related with clinical symptoms. 4. Six cases were asymptomatic and 3 cases of symptomatic steinstrasse required hospitalization. 5. Fifteen cases were treated with ESWL only and 2 cases of solitary kidney were treated with ESWL, after percutaneous nephrostomy. Early experienced 1 case was treated with ureteroscopic removal of stone. 6. Of 17 cases treated with ESWL, stone fragments expelled out within 1 month in 13 cases, 2 months in 3 cases and 4 months in 1 case of solitary kidney. All symptoms of steinstrasse were relieved within 3 days after ESWL.

9.The effects of mechanical and thermal fatigue on the shear bond strength of orthodontic adhesives.

The purpose of this study was to examine the effects of mechanical and thermal fatigue on the shear bond strength(SBS) of stainless steel mesh brackets bonded to human premolar teeth with 3 no-mix adhesives. The stainless steel mesh bracket was Ormesh(Ormco, .022 slot) and three types of no-mix adhesives were Ortho-one(Bisco), Monolok2(RMO), System 1+(Ormco). The 10(6) loadcycles of 17.4x10(2)sin2piftlg . cm and the 1,000 thermocycles of 15 second dwell time in each bath of 5degrees C and 55degrees C were acturated as mechanical and thermal fatigue stress, and SBS were measured after each fatigue test. The fracture sites were analyzed by stereoscope and scanning electron microscope. The results obtained were summarized as follows; 1. Before thermocycles, Monolok2 showed the highest Knoop hardness number(KHN, 64.03kg/mm2) and System 1+ showed the lowest value(31.60kg/mm2). After thermocycling, Monolok2 also showed the highest KHN(38.03kg/mm2) and system 1+ showed the minimum(20.87kg/mm2). The KHN of Ortho-one, Monolok2, System 1+ significantly decreased after thermocycling(P<0.01).

10.Palpable Abdominal Masses in Infants and Children: 52 Cases.

The discovery of an abdominal mass in a child usually presents a challenging problem in diagnosis and therapy to the pediatrician, surgeon and urologist. In abdominal masses, the fact that the kidney is the organ which singly produced more palpable masses than any other abdominal organ or even tract was accepted universally and most frequent causes were hydronephrosis, cystic kidney and Wilms` tumor in foreign literatures. In recent retrospective study of 52 cases of palpable abdominal masses in Korean infants and children, we obtained results as follows: 1) The kidney is the most common cause of palpable abdominal mass(86.5%). 2) Of 45 cases of renal masses, hydronephrosis is the most common(60%). 3) Of 45 cases of renal masses, Wilms` tumor is common(37.8%) but cystic kidney is rare(2.2%). 4) The most common cause of solid abdominal mass is Wilms` tumor(74%). 5) The most common cause of hydronephrosis is congenital UPJ obstruction(70%). 6) Neuroblastoma is seldom encountered.
