Congenital anomalies occur in 2-3% of neonates and have unknown and variable causes. It's occurance rate is higher in twin gestations than in singleton gestations, especially in monozygotic twins. In most cases of twin anomalies, one fetus is normal and the other fetus is not. When an anomaly is found in one fetus, various tests, such as chorionic villus sampling, amniocentesis, and umbilical cord aspiration are strongly recommended in high risk groups of chromosmal anomaly for accurate diagnosis and proper treatments. A case of congenital anomalies in both twins diagnosed in a 35 year old multiparous woman is presented with brief review of literatures.

Heterotopic pregnancy is the coexistence of intrauterine and extrauterine pregnancy. It is a rare obstetrical phenomenon with an incidence rate about 0.003%. The risk factors of heterotopic pregnancy are IUD, PID, endometriosis, tubal surgery, etc resulting in functional and anatomical injury to tubes. Recently, the wide use of ovulation induction and in vitro fertilization has increased the incidence rate. The clinical manifestation are low abdominal pain, adnexal mass, peritoneal irritation signs, vaginal bleeding, etc. Early detecion of heterotopic pregnancy is very difficult. However, recently, due to development of pelviscopy, ultrasonogram and other diagnostic procedures, the diagnostic rate has increased.Because of the high maternal morbidity and mortality resulted from intraabdominal bleeding after rupture of ectopic pregnacy, immediate termination of ectopic pregnancy is required usually by operative methods. We experienced a case of heterotopic pregnancy after tubal reanastomosis and report this case with a brief review of literatures.

Tuberculomas of the lung are rare in children and one of the more common lesions presenting a solitary pulmonary nodule roetgenorgraphically. We are reporting of a 3-year-old child with a tuberculoma in left upper lobe. The patient was initially diagnosed as the benign mediastinal tumor but in the end as tuberculoma in left uper lobe. Wedge resection including the mass was done. The tumor had brown smooth external surfaces on sectioning including the mass was done. The tumor had brown smooth external surfaces on sectioning show pale gray and soft cut surface was shown. In light electromicroscopy chronic granulomas with multinucleated giant cells and central caseous necrosis were observed which are the characteristics of tuberculoma. The postopeative course was smooth and uneventful and patient has been well for 4 months postoperatively.

Traditionally, total parenteral nutrition(TPN) has been used when patients with hypaemesis gravidarum failed to respond to conservative management, but now many physicians attempt to enteral feeding via nasogastric tube because TPN is expensive and has many complications. Recently, we experienced 2 cases of hyperemesis gravidarum successfully managed by enteral feeding via nasogastric tube, therefore we think that this method may be carefully considered to the patients with hyperemesis gravidarum failed to respond to conservative management. So we report them with brief review of the literatures.

Despite the rarity of 48, XXYY, having an incidence rate of 1 in 50,000 persons, 1 in 300 persons are reported to have a mental disorder of criminal behavior. The clinical characterics of 48, XXYY are similar to Klinefelter's syndrome such as small testis, tall stature, gynecomastia and can show mental retardation and skeletal anomaly. When the cause of elevated maternal serum AFP is not explained by USG or measurement of amniotic fluid AFP, during the second trimester of pregnancy, it is defined as unexplained elevation of maternal serum AFP. We report a case of 48, XXYY with unexplained elevation of maternal serum AFP which has been experienced in our hospital with brief review of literature.

7.Photoselective vaporization treatment on benign prostatic hyperplasia in the elderly over 70 years old

Objective To investigate the efficacy and safety of photoselective vaporization treatment on benign prostatic hyperplasia(BPH)in the elderly over 70 years old.Methods From August 2004 to December 2005,a total of 126 patients with lower urinary tract symptoms secondary to BPH underwent photoselective vaporization of the prostate(PVP)with an 80 W quasicontinuous KTP laser.The patients aged between 70 and 96 years,(median 78.0),with 46 cases above 80 years. Prostatic volume was 24~86 ml,in an average of 47.0 ml,with 46 cases above 60 ml.Results The mean operative time was 46.8 min(range from 30 to 100 min).After operation,the catheter was pulled out in 69 cases within 24 hours and the patients generally urinated well.At three-months follow-up,the mean I-PSS decreased significantly from(25.0?3.7)to(16.3?2.0)(P<0.01), while the Qmax increased from(5.8?2.7)ml/s to(16.3?4.8)ml/s(P<0.01).Complications consisted of 4 cases of hematuria,6 urinary infection and 1 acute epididymitis.Conclusions The study indicates that PVP,a minimal invasive surgical procedure,is a safe and effective procedure for the treatment of BPH in the elderly.
8.The Long Term Follow-up of Maxillary Sinus Retention Cyst.

BACKGROUND AND OBJECTIVES: Retention cyst of the maxillary sinus is a common incidental finding on Waters' view. The purpose of this study is to evaluate the fate of maxillary sinus retention cyst after at least 3-year follow-up. MATERIALS AND METHOD: Twenty-five patients diagnosed to have maxillary sinus retention cyst on Waters' view between 1997 to 1999 were asked to revisit for the follow-up Waters' view at least 3 years later. Nine patients complied with the request. Medical records, endoscopic nasal findings, and radiological data of nine patients were reviewed. Cyst size was measured by the 2-dimensional milimeter scale and the relative percentage of the total antral area. RESULTS: The cyst was located in the right maxillary sinus in 4 patients, left in 4, and bilateral in 1. Nine out of ten cysts appeared to arise from the floor of the sinus. The area of the cysts ranged in size from 232 mm2 (ratio 36.4%) to 370 mm2 (55.3%), as measured on Waters' view. Six of the cysts resolved spontaneously, three showed no change in size and only one has enlarged twice the size of initial cyst. The patient with enlarged cyst didn't complain of any specific symptoms. CONCLUSION: The majority of cysts (90%) decreased in size or spontaneously resolved after 3 years. Thus, `wait and see' policy may be appropriate unless associated with specific symptoms.

9.A Case of Leiomyosarcoma of the Gingiva.

Leiomyosarcoma is a malignant mesenchymal tumor that is relatively common in the uterus, the gastrointestinal tract, and the retroperitoneum. Only 3 to 10% of leiomyosarcomas arise in the head and neck. A review of the English-language literature since 1908 revealed 11 cases of primary and one case of metastatic leiomyosarcoma of the gingiva. We report on a case of leiomyosarcoma that arose in a 34-year old male, affecting the mandibular gingiva.

10.Endoscopic Removal of Benign Endotracheal/Endobronchial Tumor.

Endoscopic removal is acceptable for the treatment of endotracheal/endobronchial mass, because it is less invasive in high-risk patients and a conservative procedure for benign tumors. Two benign tumors in the lumen of the trachea (pure lipoma) and in the intermediate bronchus (hamartoma) were completely eradicated by our procedures, which involved diathermic snaring and residual mass removal with biopsy forceps under the guidance of fiberoptic bronchoscopy. No tumor recurrence was evident after extended follow-up (6 years for endotracheal lipoma and 2.5 years for endobroncheal hamartoma). Our method is safe and less invasive for the patient and provides the surgeon with better view during procedure.
