2.The Treatement of ankle Fracture

The ankle stability has a vital role in normal gait and walking. An accurate and anatomic reduction with rigid fixation has been emphasized as important regimens as well as early motion and weight bearing exercises. The authors analysed 90 cases(88 patients) of ankle fractures which were admitted and treated in orthopedic department, Chung-Ang Gil General Hospital from March 1987 to May 1992. Males were affected 1.7times than female and the age group of third and fourth decades constituted 60.2%. The most common cause was vehicle accident(35 cases; 38.9%). According to Lauge-Hansen classification, supination-external rotation type was most common(32 cases; 35.6%). Accoompanyng injuries were noted on 45 cases(50%) and fractures in lower extremities were most common(17 cases; 18.9%). According to meyer's assessment of result, the clinical and radiologic results were better in operative method than conservative ones. From the evaluation of clinical results, anatomic reduction and rigid internal fixation of lateral malleoli and distal tibiofibular joint are more important factors. Complications are resulted in 19cases(21.1%) and traumatic arthritis is the most common (10 cases; 11.1%). The extent and degree of injury was an important factor in an overall results of treatment as well as the anatomic reduction of fracture alignment.

4.A Case of Rhino-orbital Mucormycosis, Treated with Intraconal Amphotericin B Irriation and Abscess Drainage.

Mucormycosis is a relatively uncommon opportunistic fungal infection that occurs more commonly in patients with immunosuppression and diabets mellitus, particularly those with ketoacidosis. Successful therapy for rhinoorbital mucormycosis requires early diagnosis, correction of predisposing conditions, prompt use of amphotericin B, and aggressive surgical debridement. We experienced a 56-years-old poorly controlled diabetic woman with rhinoorbital mucormycosis that was successfully treated with intraconal amphotericin B irrigation and packing without exenteration.

5.Leiomyosarcoma of the Pancreas: A case report.

Most of the malignant tumors of the pancreas are adenocarcinomas arising from the ductal epithelium. Primary leiomyosarcoma of the pancreas, even though it is the most common sarcoma of the pancreas, is very rare. We present a case of leiomyosarcoma of the pancreas, probably primary, with metastases to the stomach, lymph nodes, and abdominal wall. A 52-year-old woman visited the hospital with vague right upper abdominal pain and weigh loss of 6 kg for 2 months. The radiological and endoscopic examination revealed that she had a large heterogeneous mass, 11 cm in size, in the pancreatic body and tail, a 4 cm-sized mass in the paraaortic area, and a 3 cm-sized polypoid mass in the stomach. Histologically, they were all similar to one another and composed of markedly pleomorphic cells. Immunohistochemical and electron microscopic studies showed definite smooth muscle differentiation of the tumor cells. Two months later, the patient underwent an excision of a new 3 cm mass in the right lower abdominal wall, showing features of well differentiated leiomyosarcoma.

6.Studies on bovine besnoitiosis in Korea II. A survey on incidence in the enzootic region.

The 4,725 cattle in the enzootic region, 6 counties of southern Korea, were surveyed for besnoitiosis epidemiologically by examining the scleral conjunctiva for the cysts and the following results were obtained. The survey showed that 285 cattle, representing 6 per cent of a total of 4,725 cattle examined had S.C. cysts. Only 49 (14 per cent) of them showed clinical sclerodermatitis and the ratio between clinical and inapparent cases was 1:5.8. On age distribution of the S.C. positive cases the highest incidence(10-12 per cent) was seen in amimals that were from 5 to 8 years of age, but clinically apparent cases occurred much more in younger age of animal. In local incidence of S.C. cysts positive cases the prevalence was higher in the secluded districts such as Koheung and Sancheoung counties(9 per cent).

7.Clinical Analysis of Penetrating Keratoplasty in Herpes Simplex Keratitis.

We conducted a retrospective study of 20 penetrating keratoplasty (19patients) for corneal opacity due to herpes simplex keratitis from January 1992 to February 1997 at National Medical Center. We exaniined f requeuey of graft rejection and recurrence of herpetic keratitis after penetrating keratoplasty. The patients were grouped into control & treatinent, group according to the use of antivirial drug after penetrating keratoplasty and we examined the difference in rate of recurrence between two groups. The follow-up period was from 8 months to 68 months, mean follow-up period being 29.6 months. Graft rejection was seen in 3 eyes out of 20 eyes (15.0%) and recurrence was developed 3 eyes out of 20 eyes (15.0%). Recurrent herpetic keratitis developed in 15.4%(2 of 13) of the eyes in control group and 14.3% (1 of 7) of the eyes in treatment group. There was no statistically significant difference in recurrence rate between two groups (p)0.05). Postoperative prophylactic antiviral treatment is not associated with decreased rate of herpes simplex keratitis recurrence.

8.The Effect of Limbal Transplantation & Cyclosporine A for Chemically Damaged Rabbit Cornea.

The authors have evaluated the effect of topically applied 1% cyclosporine A for four weeks after limbal allograft transplantation for ocular surface stability in severely ocular surface damaged rabbits. A total eleven severely damaged rabbits were subdivided into three groups of four limbal autograft (AUTO), three limbal allograft (ALLO), four limbal allograft with topical application of 1% cyclosporine A five times daily for four weeks (ALLO-C). We examined ocular surface smoothness, clarity, corneal vascularization before surgery and three months after surgery, and classified three grade (success, partial success, failure) according to result. In AUTO, three eyes were success, one eye was failure. In ALLO, one eye showed inferior graft detachment and one eye was success, two eyes were failire. In ALLO-C, no graft detachment detected and one eye was success, two eyes were partial success, one eye was failure. We knew that. ALLO-C show more effective ocular surface stability than ALLO. These result suggest ALLO-C can be used as alternative treatment of AUTO in ocular surface reconstruction. But, rnore study for duration and concentration of cyclosporin A will be need.

9.Clinical Effects of Benoxal (Benzoyl peroxide) on Acne Vulgaris.

Benzoyl peroxide has been known to have bacteriostatic activity against Corynebacterium acnes and to have comedolytic action in patients with acne vulgaris. The present study was undertaken to evaluate the therapeutic effect of Renoxal, a 5% benzoyl peroxide lotion, in patients with acne vulgaris. A total of 35 patients entered this study at the Department of Dermatology, Seoul National University Hospital during the five months period from March to July, 1978. All patients were instructed to apply Benoxal on their affected sites once or twice daily; and the number of lesions on three fixed areas were counted before and every week for four weeks after the treatrnent. Two female patients stopped the application due to developrnent of contact dermatitis to Benoxal The results observed in the 33 patients were as follows: 1. The percentage decrease in total number of lesions in four weeks after treatment was 51.1% 2. Closed and open comedones were gradually eliminated; and their numbers were decreased in four weeks after treatment by 45. 8% and 55. 3% respectively. The papules were decreased. by 59. 1 @2 after the first week of treatment, but showed. a transient lag around tbe second week and then marked 61. 5% in four weeks after treatment. 3. Observed side effects included burning sensation,(3 cases), tightness (3 cases), itching sensation (2 cases) and scaling (1 case). All were tolerable without any specific measure or discontinuance of the application. The authors concluded througb this experiment that Benoxal (5% benzoyl peroxide lotion) is a very effective local therapeutic agent in the treatment of acne vulgaris patients.

10.A Case of Generalized Lichen Nitidus.

Lichen nitidus(LN) is an unaommon chronic eruption of numerou tiny, discrete, usually flesh colored, shiny papules most cornmonly located on the penis, arms, for arms, and abdomen. It was described by Pinkus, first in 1901. The distribution of LN is most often locaized, but in some cases it may become generalized. A 10-year-old healthy boy was seen by our department because of many flesh colored, discrete, shiny papules on the whole body. The papules are skin colored, pinhcad sized, round, and flat-topped. The lesions appeared first at the age of 9 on the forehead and then neased in the number and extended downward to the neck, trunk, abdornen and both extremities. This case was successfully treated with topical corticosteroid.
