The prevalence of right-sided infective endocarditis is increasing with a growing number of intravenous drug abusers. A case of right-sided endocarditis is reported in a 18-year-old female heroin addict. Tricuspid valve vegetation, septic pulmonary emboli and staphyloccus aureus in blood cultures are noted. During antimicrobial therapy, she developed remittent fever and skin rash due to vancomycin. After a change of the antibiotic to teicoplanin, she was freed from fever and discharged in an improved condition.

2.14 Cases of Nondysgerminomatous Ovarian Germ Cell Tumor.

A clinicopathologic study was per formed on 14 cases of malignant nondysgerminomatous ovari an germ cell tumors(NDOGCT) treated at the department of obstetrics and gynecology in Korea Cancer Center Hospital between Jan. l987 and,Jun. 1992. The results obtained were as follows ; 1. Median age of the patients was 27.5 years (range, 5 to 54) 2.Clinical symptoms presented were abdminal pain(64.3%), abdominal palpable mass(42.9%), abdominal distension(28.5%) in order of frequency. 3. Histopathologically, 7 cases(50%) were irnmature teratoma, 4 cases(28.6%) were endoder mal sinus tumor, 1 case(7.1%) was embryonal cell carcinoma, 2 cases(14.3%) were mixed germ cell tumor. 4. Bystage, 6 casse(42.9%) were in stage I, 1 case(7.1%) in stage Il, 2 cases(14.3%) in stage III, 3 cases(21.4%) in stage IV, and 2 cases(14.3%) in recurrence. 5, Serum alphafetoprotein was elevated in all patients withendodermal sinus tumor or embryonal carcinoma, and serum CA125 was elevated in about one half of the patients with nondysgerminomatous gerrn cell tumr. 6. l0 cases were managed by salpingo-oophorectomy with poatoperative chemotherapy, 3 cases by hysterectamy and salpingo-oophorectomy with postoperative chemotherapy. 7. Durations of follow up ranged 5 to 66 months, in that period, 5 cases were died, 4 cases were alive with disease and 4 casee wero no evidence of disease. 1 case was loat follow up after operation. All 5 patients were alive who had been treated with REP regirnen. In conelusion, prognosis of malignant NDOGCT is still poor in spite of adjuvant chemotherapy following surgery, and study ineluding appropriate chemotherapeutic regimen may be needed.

4.Effects of Ginsenosides on Relaxation of Rabbit Vaginal Smooth Muscle.

PURPOSE: Ginseng has been used throughout the Far East, including Korea and China, as a tonic and restorative agent to maintain physical vitality. The main pharmacoactive molecules of ginseng are ginsenosides. The present study was designed to investigate whether ginsenosides relax rabbit vaginal smooth muscle and whether this effect is modulated by nitric oxide(NO) and the cGMP pathway. MATERIALS AND METHODS: Strips of rabbit vagina were mounted in organ chambers to measure isometric tension. The strips were contracted with phenylephrine(5 X 10(-5) M), and the responses to acetylcholine, nitric oxide inhibitor, and ginsenosides were examined. The cGMP content of the strips was measured by radioimmunoassay after various doses of ginsenosides. RESULTS: Ginsenosides(100~500microgram/mL) relaxed vaginal smooth muscle in a dose-dependent manner(5~25%). Acetylcholine-induced relaxation was significantly increased in the presence of ginsenosides(100, 200microgram/mL)(p<0.05). The relaxation effect of ginsenosides was inhibited by L-NAME(10(-4) M), but the difference was not statistically significant(p>0.05). Ginsenosides(400microgram/mL for 7 min) increased the accumulation of cGMP. CONCLUSIONS: These data suggest that ginsenosides have a relaxing effect on rabbit vaginal smooth muscle. This effect is at least in part mediated by the NO-cGMP pathway.

Duodenal duplication cyst is an uncommon congenital anomaly that is usually encountered during infancy or in early childhood. The clinical manifestation is that of duodenal obstruction or, less commonly, obstructive jaundice, acute pancreatitis, or gastrointestinal bleeding. Here, we report a case of duodenal duplication cyst on the juxtapapillary region in a 19-year-old woman with an unusual clinical manifestation of recurrent pancreatitis and peculiar endoscopic finding of the cyst.

An infant having parents of Jehovah s Witnesses was 4 months old and 5.6 kg in weight. Echocardiographic diagnosis was complete transposition of great arteries(TGA), ventricular septal defect(VSD), atrial septal defect(ASD), patent ductus arteriosus(PDA), and bilateral superior vena cava(SVC). The preoperative hemoglobin level was 14.9 g/dl. We performed an arterial switch operation(ASO) with VSD closure without transfusion and he was discharged uneventfully 16 days after the operation. At that time the hemoglobin level was 12.8 g/dl. We report that we could successfully correct the complex congenital heart disease without transfusion by the combined use of erythropoietin and aprotinin, intraoperative meti-culous hemostasis, and postbypass ultrafiltration.

BACKGROUND/AIMS: Several noninvasive methods have recently been developed for the evaluation of liver fibrosis. The accuracy of transient elastography (TE), acoustic-radiation-force impulse (ARFI) elastography, and real-time elastography (RTE) in predicting liver fibrosis were evaluated. METHODS: Seventy-four patients who had undergone a liver biopsy within the previous 6 months were submitted to evaluation with TE, ARFI, and RTE on the same day. RESULTS: There were significant correlations between fibrosis stage and liver stiffness measurement (LSM) using the three tested methods: TE, r2=0.272, P=0.0002; ARFI, r2=0.225, P=0.0017; and RTE, r2=0.228, P=0.0015. The areas under the receiver operating characteristic curves (AUROC) for the diagnosis of significant fibrosis (> or =F2, Metavir stage) by TE, ARFI, RTE, TE/platelet count (PLT), velocity of shear wave (Vs)/PLT, and elasticity score (Es)/PLT were 0.727, 0.715, 0.507, 0.876, 0.874, and 0.811, respectively. The AUROC for the diagnosis of cirrhosis by TE, ARFI, RTE, TE/PLT, Vs/PLT, and Es/PLT were 0.786, 0.807, 0.767, 0.836, 0.819, and 0.838, respectively. Comparisons of AUROC between all LSMs for predicting significant fibrosis (> or =F2) produced the following results: TE vs. RTE, P=0.0069; ARFI vs. RTE, P=0.0277; and TE vs. ARFI, P=0.8836. Applying PLT, the ability of each LSM to predict fibrosis stage significantly increased: TE/PLT vs. TE, P=0.0004; Vs/PLT vs. ARFI, P=0.0022; and Es/PLT vs. RTE, P<0.0001. However, the ability to predict cirrhosis was not enhanced, combining LSM and PLT. CONCLUSIONS: TE and ARFI may be better methods for predicting significant liver fibrosis than RTE. This predictive ability increased significantly when accounting for platelet count. However, all of the measures had comparable efficacies for predicting cirrhosis.

BACKGROUND AND PURPOSE: In a recent pooled analysis of randomized clinical trials (RCTs), intravenous tissue plasminogen activator (TPA) improves the outcome in patients aged > or =80 years. However, it is uncertain whether the findings are applicable to clinical practice in Asian populations. METHODS: From a multicenter stroke registry database of Korea, we identified patients with acute ischemic stroke who were aged > or = 80 years. Using multivariable analysis and propensity score (PS)-matched analyses, we assessed the effectiveness and safety of intravenous TPA within 4.5 hours. RESULTS: Among 2,334 patients who met the eligible criteria, 236 were treated with intravenous TPA (mean age, 83+/-5; median NIHSS, 13 [IQR, 8-17]). At discharge, the TPA group compared to the no-TPA group had a favorable shift on the modified Rankin Scale (mRS) score (multivariable analysis, OR [95% CI], 1.51 [1.17-1.96], P=0.002; PS-matched analysis, 1.54 [1.17-2.04], P=0.002) and was more likely to achieve mRS 0-1 outcome (multivariable analysis, 2.00 [1.32-3.03], P=0.001; PS-matched analysis, 1.59 [1.04-2.42], P=0.032). TPA treatment was associated with an increased risk of symptomatic intracranial hemorrhage (multivariable analysis, 5.45 [2.80-10.59], P<0.001; PS-matched analysis, 4.52 [2.24-9.13], P<0.001), but did not increase the in-hospital mortality (multivariable analysis, 0.86 [0.50-1.48], P=0.58; PS-matched analysis, 0.88 [0.52-1.47], P=0.61). CONCLUSIONS: In the setting of clinical practice, intravenous TPA within 4.5 hours improved the functional outcome despite an increased risk of symptomatic intracranial hemorrhage in very elderly Korean patients. The findings, consistent with those from pooled analysis of RCTs, strongly support the use of TPA for this population.
