1.Arthrographic Finding of Meniscus Tear

Double-contrast arthrography of the knee, that is roentgenographic examination of the supporting structures of the knee joint with the aid of the radiopaque dyes, has been utilized infrequently until recent years in our country. A growing appreciation of the advantages of arthrography, increased experience with the technique, and an increasing ability to interprete the arthrogram have led to more frequent use of this diagnostic aid. Double-contrast arthrography of the knee is of particular value in the examination of the patient with an atypical history of in jury and unusual physical findings, or in the individual with an acute injury who cannot tolerate a manipulative physical examination. Its use has led to greater diagnostic acumen prior to operative intervention and in many cases has aided the orthopedic surgeon in determing whether or not surgery indicated and in surgical exploration. And its accuracy of the diagnosis of the meniscus tear has been very high, greater than 90% in many series. The followings are summary of result; 1) Double-contrast arthrography of the knee is a valuable diagnostic procedure. 2) Prolonged observation and procrastination in the treatment of patients with symptomatic knees could be avoided. 3) There were no complications of arthrography and all patients returned to work immediately after the procedure. 4) In this series, 2 cases out of the 6 cases of the torn lateral meniscus were not demonstrated correctly by arthrography. From this result, it is suggested that the interpretation of arthrographic findings is somewhat difficult at present time. In future, further experience and study for arthrography is demanded for more accurate interpretation of the conditions of the structures of the knee joint and also is desired to compare with the arthroscopic findings.

2.Two-Stage Tendon Reconstruction Using Hunter Silicone Rod Prosthesis

Flexor tendon injuries of the “no man's land”(Zone2) in the hand have long been considered one of the problem areas for hand surgeons because of the frequently unsatisfactory result of treatment. Two-stage tendon reconstruction using the silicone rod prosthesis is one of the recent valuable procedures for management of severely damaged hand. From Aug. 1977 to Jun. 1978, the authors performed this procedure using “Hunter Silicone Rod” on 4 cases of flexor tendon injuries accompanied by deep cicatrix of soft tissue and stiffness of finger joints. The follow-up results were excellent in 3 cases out of the 4, and poor in the remaing case which had marked wound infection and stiffness of finger joints.

3.Thiosulfate sulfurtransferase and UDP-glucuronosyltransferase activities in cholestatic rat liver induced by common bile duct ligation.

We have investigated the effect of cholestasis on the hepatic thiosulfate sulfurtransferase (rhodanese) and UDP-glucuronosyltransferase (UDP-GT) activities in rats. Rhodanese activities in the liver cytosol, mitochondria and microsomal fractions as well as in the rat serum, and UDP-GT activity in the microsome have been investigated for a period of 42 days after common bile duct (CBD) ligation. The cytosolic rhodanese activity showed a significant decrease between the first through the 42nd day, and the mitochondrial activity showed a significant decrease between the 7th through the 42nd day after CBD ligation compared to the activities from the sham operated control, respectively. In the case of microsomal preparation, both rhodanese and UDP-GT also showed significant decrease in their activities after the ligation for the former enzyme between the 14th and the 42nd days, and for the latter enzyme between the third and 42nd days, respectively. On the other hand, the serum rhodanese activity increased markedly soon after the ligation, exhibiting the peak activity after 1 day of CBD ligation with about 4.6-fold increment. The activity subsequently decreased gradually reaching to the control level at the 42nd day post-ligation. Enzyme kinetic parameters of hepatic rhodanese and UDP-GT were analyzed using sodium thiosulfate and p-nitrophenol as substrates, respectively, with the preparations from the 28th day post-ligation. The results indicated that although the K-m values of these enzymes were about the same as the sham-operated control, the V-max values of the both enzymes decreased significantly. These results, therefore, suggest that the biosynthesis of rhodanese and UDP-GT have been reduced in response to cholestasis, and that the elevation of rhodanese activity in the serum is most likely due to leakage from the liver subsequent to CBD ligation.

Differentiation between benign and malignant hepatic lesions was attempted according to the shape of the lesion, internal architecture and characteristics of signal intensity on MR images. MR images. We retrospectively analyzed 20 cases of hemangiomas, 12 cases of hepatomas and 5 cases of cysts which were diagnosed by clinical, radiological and surgical methods (33 patients). On T1-weighted, proton weighted, and T2-weighted images, most hemangiomas (70%) and cysts (100%) had smooth margin and were round or oval in shape, while hepatocellular carcinomas had irregular (67%) or lobulated (33%) borders. Ninety percent of hemangiomas and 100% of cysts had homogeneous signal intensity, but 67% of hepatocellular carcinomas were inhomogeneous. Signal intensities of hepatocellular carcinomas were more variable. However, hemangionmas were isointense or hyperintense on proton weighted images, and markedly hyperintense on T2-weighted images . In conclusion, inhomogenous, irregular border, and variable signal intensity lesions had high likelihood of malignancy, while homogenous and markedly hyperintense lesions were more likely benign.

6.Clinical Results on Intraocular Lens Implantation.

A clinical study of the refractive and visual results with 104 consecutive implantations of the intraocular lens was performed between July 1984 and April 1986. Postoperative refraction of myopia was observed in 56.7% of patients, and emmetropia in 21.1%, hyperopia in 22.2%. Visual acuity of 0.5 or better at 2 month after operation was revealed in 88.9% of cases studied; 90.3% in 62 cases of posterior chamber lens implantations and 85.7% in 28 cases of anterior chamber lens implantations. There was no statistically significant difference between visual results after anterior chamber lerts and posterior chamber lens implantation.

7.Modified High Tibial Osteotomy

Treatment of unstable cervical spine fracture by skull traction is time consuming procedure, and treatment by Halo appratus has many advantages but it is very expensive. Bone cement has been applied occasionally as an adjunct to metallic internal fixation in treating patient with malignancy, i.e. limited life expectancy. Internal fixation with bone cement in addition to routine posterior fusion with bone graft is tried eliminate the need of application of complicated external immobilization in treating unstable cervical spine fracture of young patient. We experienced two cases of posterior cervical fusion using bone cement with satisfactory results. The advantages of this method are: 1. Rapid and solid fixation 2. Early ambulation 3. Reduce complicated external support 4. Reduce the date of hospitalization.

8.A Clinical Study of Ipsilateral Fracture of Femur and Tibia

Twenty-six cases of fracture of the femur and tibia on the same leg were treated in Masan Koryo General Hospital during the period 1981 1986. We studied all of these patients, divided by four groups according to the method of treatment, retrospectively with analysis of treatment and end results. The results were as follows : 1. Twenty-two patients were male and four patients were female. Of twenty-six patients, eighteen patients were at their third and fourth decades. 2. The main cause of injuries was traffic accident; 23 patients(88%). 3. The most common concomitant injury was fracture and dislocation in other site(14 cases). The fat embolism developed in one case and one patient had died due to head injury. 4. The average healing time of fracture was 17 weeks in femur and 20 weeks in tibia in group three, that was most rapid of all groups. 5. A good or excellent functional result was achieved in 87% of the patients treated with internal fixation of both fracture, 75% of the patients with internal fixstion for the femur fracture and external fixation for the tibial fracture, and 25% of the patients trested conservatively.

10.MR study of normal nasal cycle.

Magnetic resonance images frequently demonstrate asymmetrical mucosal volume in the nasal area of asymptomatic patients. To further evaluate this phenomenon, sequential MR examinations of the nasal cavity and paranasal sinuses were performed three to four times during a 3 to 8 hours period in ten normal volunteers. The study demonstrated that the mucosal volume changes alternated from side to side during this period in eight volunteers. Changes were also observed within the ethmoid sinuses, nasal septum and nasolacrimal ducts. Awareness of MR findings of nasal cycle may reduce the likehood of inflammatory disease being confused with normal physiologic changes, and also may provide another method in the study of this physiologic phenomenon.
