1.Pleomorphic Xanthoastrocytoma: A Case Report.

PURPOSE: To draw attention to the radiological findings of a benign variant of cerebral astrocytoma in a young patient. MATERIALS AND METHODS: A 24-year-old man with generalized tonic-clonic seizure of 7 years' duration and normal neurological examination was examined with plain skull series, brain CT and MRI, and cerebral angiography. MR imaging was performed with a 0.5 Tesla Toshiba MRT-50A scanner (TIWI, PDWI, T2WI, 0.1 mmol/kg of Gd- DTPA, SE). RESULTS: 1 ) Plain skull series: A radiolucent lesion with a partial radiopaque rim of about 2.5 x 3 cm size in the right anterior parietal bone.2) B rain CT scan: A cystic mass in the right frontoparietal cortex of midconvexity with pressure erosion on the adjacent skull and partial enhancement at outer and anterior portion.3) Brain MRI:A hypointense mass containing a small, intensely enhancing isointensity anterolaterally on Tl-weighted images, which was hyperintense with better delineation of bulging cortical-based appearance on T2-weithted images. No peritumoral edema.4) Cerebral anglography: An avascular mass. CONCLUSION: The diagnosis of pleomorphic xanthoastrocytoma(PXA) should be entertained in patients in whom a superticially placed enhancing intracerebral tumor containing cystic portion that seems to be in contact with the meninges develops during juvenile years.

2.A Case of Oil Granuloma Developed after Application of Squalene on Both Eyelids.

Oil granuloma develops mainly after injection of oil substances such as paraffin, silicone, etc, but there have been reports of oil granuloma after topical application of lipid substance to the body. A 22-year-old woman developed oil granuloma after the application of squalene on both lower eye-lids. Histopathologic findings ahowed acanthosis and dyskeratosis of the hair follicles. Lipid suketances staining with oil-red-O stain were found mainly on the hair follicles. and partly on the epidermis. We report a case of oil grinulorna caused by squalerie that may have penetrated through the hair follicles and partly through the epidermis resulting in chronic granulous changes in the dermis.

3.Analysis of X-Ray Findings of Osteosarcoma

The diagnosis of osteosarcoma is based on both clinical, rediological and histopathological findings, but the majority of the patients with osteosarcoma can be diagnosed with a reasonable degree of confidence from the appearance of the lesion on the plain X-ray film. The combination of soft tissue mass and calcification, increased intramedullary radiodensity, cortical bone destruction,and marked periosteal reaction such as Codmans triangle and sunburst reaction is the radiographic hallmark of osteosarcoma, Among the above mentioned radiological findings, the presence of soft tissue mass with calcification can be so strongly suspected to be nearly diagnostic on the X- ray examination alone. The author performed radiological analysis of 40 cases of osreosarcoma which was experienced at the Department of Orthopedic Surgery, College of Medicine, Korea University including other hospitals from 1975 to 1980 and particulary observed correlation between the soft tissue mass with calcification and other radiological findings. The results obtained as follow: 1. Of the 40 cases, soft tissue mass with calcification were observed in 32 cases(80%), cortical destruction in 31 cases(77.5%), periosteal reaction in 24 cases(60%), sunburst reaction in 23 cases(57%) and Codmans triangle in 21 cases were observed. 2. Of the 40 cases, 23 cases(57.5%) were osteoblastic, 12 cases(30%) of osteolytic and 5 cases(12.5%) of mixed type. 3. The shadow of soft tissue mass with calcification was observed 11 cases(91.7%) among 12 cases of osteolytic type, 28 cases(91%) among 31 cases of cortical destruction, 21 cases(87.5%) among 24 cases of periosteal reaction, 20 cases(86.9%) among 23 cases of sunburst reaction and 18 cases (85.7%) among 21 cases of Codmans triangle.

4.Two Cases of Allergic Contact Dermatitis to Betadine(R).

Antiseptics are substances that kill or prevent the growth of microorganisms when applied to living tissue. They must be effective against microorganisms but must also retain their activity in presence of body fluids without being harmful locally or systemically. Among many antiseptics, Betadine(R) has been widely used because of its low toxicity and high germicidal efficacy. We reported 2 cases of allergic contact dermatitis to Betadine(R) in surgical patients. They had eczematous eruption along the Betadine(R) applying sites. Path tests Betadine(R) confirmed the diagnosis.

5.Limb Salvage Surgery with Tumor Prosthesis for the Malignant Bone Tumors Involving the Proximal Femur.

PURPOSE: As well as patient survival, the restoration of postoperative function such as ambulation is important in limb salvage operations for treatment of malignant bone tumors involving the proximal femur. The authors analyzed clinical outcomes of limb salvage operations using tumor prostheses for metastatic or primary malignant bone tumors in the proximal femur. MATERIALS AND METHODS: From February 2005 to January 2014, 20 cases (19 patients) with malignant bone tumor involving the proximal femur with pain or complicated pathologic fracture were treated with segmental resection and limb salvage operations with tumor prostheses. Mean age was 63.1 years (range 35-86). Fourteen patients were male and six ones were female. The mean follow-up period was 20 months (1-94 months). There were 15 cases of metastatic bone tumor, 4 cases of osteosarcoma, and 1 case of multiple myeloma. The primary tumors of the metastatic bone tumors included 4 lung cancers, 3 hepatocellular carcinomas, and 3 renal cell carcinomas. Other primary tumors were breast cancer, thyroid cancer, colon cancer, prostate cancer, and malignant spindle cell tumor, each in 1 case. Modular tumor prostheses were used in all cases; (Kotz's(R) Modular Tumor prosthesis (Howmedica, Rutherford, New Jersey) in 3 cases, MUTARS(R) proximal femur system (Implantcast, Munster, Germany) in 17 cases). Perioperative pain was assessed with Visual Analogue Scales (VAS). Postoperative functional outcome was assessed with Musculoskeletal Tumor Society (MSTS) grading system. RESULTS: Out of 20 cases (19 patients), 11 cases (10 patients) survived at the last follow-up. Average postoperative survival of the 9 deceased patients was 10.1 months (1-38 months). VAS score improved from pre-operative average of 8.40 (5-10) to 1.35 (0-3) after operation. Average postoperative MSTS function score was 19.65 (65.50%, 7-28). The associated complications were 2 local recurrences, 3 hematomas, 3 infections, 2 scrotal swellings, and 1 dislocation. There was no case of periprosthetic fracture or loosening. CONCLUSION: Limb salvage operation with tumor prosthesis is an appropriate treatment for early pain reduction and functional restoration in malignant bone tumors in the proximal femur with pain an/or complicated pathologic fractures.

Since CT scanning was introduced in the diagnosis of calcaneal fracture, we became to understand more clearly the pathoanatomy of the injury. However we have to determine if we would or would not take CT scanning by simple radiography in the acute setting and most of the surgeons still use simple radiography in evaluating postoperative result. And also we have to determine the accuracy of reduction and position of screws by simple radiography intraoperatively. The purpose of this report is to determine the accuracy of simple radiography in assessing the pathological anatomy of the intraarticular calcaneal fracture by experimental and clinical studies. This is a combined experimental and clinical studies using four below knee amputation specimens and 15 patients with 19 displaced intraarticular calcaneal fractures treated at Hanil General Hospital between Jan. 1996 and May 1997. The lateral view of the foot was the best radiographic view for visualizing the degree of depression of the posterior facet. With regard to the location of a fracture line, findings of Broden views correlated well with findings on CT in 17 of 19 fractures. With regard to gapping between fragments, the axial view was not satisfactory in the majority of cases(15 of 19 cases). There was no case which showed more than 2 mm of discrepancy between the findings on Broden view and CT. We conclude that we may obtain most of the important informations about the pathological anatomy of the intraarticular calcaneal fracture by simple radiography.

8.A Case of Metastatic Carcinoma of the Skin.

We report a case of metastatic carcinoma of the skin in 62-year-old male who showed multiple nodules on the anterior chest, scalp, face and left axilla for 2 months duration. He suffered from lower abdominal discomfortness for 6 months. Histopathologic findings revealed atypical cell nests and some tubular or glandular formation in the dermis and subcutaneous fat tissue. Material in the lumen showed diastase resistant and PAS positive, and alcian blue positive at pH 2.4, but negative pH 0.4. So it represented sialomucin. We did not find primary site of malignant tumor. In view of clinical and histopathological findings, it was considered to be originated from the large intestine.

9.Clinical Observation on the Renal Tuberculosis.

A clinical observation was made on 54 cases of renal tuberculosis who were admitted to the Department of Urology, Soon-Chun-Hyaing College Hospital during the period from January, 1977 to December, 1981. The following results were obtained. 1. The incidence of renal tuberculosis was 7.5% of all urological in-patients. 2. The most common age group was 20-49 years (68.5%) and male to female ratio was 1.8:1. 3. Both kidneys were affected in 8 cases (14.8%), while 26 cases (48.2%) in right and 20 cases (37%) were in left kidney. 4. The most frequent subjective complaints was frequency in 62.9% and hematuria in 40.7%, flank pain in 38.9%, painful urination in 29.6%, nocturia in 27.8% in order of their frequency. 5. Eleven (20.4%) of 54 cases were associated with pulmonary tuberculosis. 6. Urinary findings revealed pyuria in 68.5%, proteinuria in 66.7% and hematuria in 53.7%. 7. On the intravenous pyelogram, non-visualization of the kidney was found in 31.5%, hydronephrosis in 24.1%and destruction of calices in 20.4%. 8.Cystoscopic findings showed active lesions of cystitis in 47.9%, fibrosis with trabeculation in 20.8%, deformed ureteral orifice in 18.8% and contracted bladder in 14.6%. 9. Nephrectomy was done in 26 cases (48.1%), ureteric reimplantation in 2 cases. cutaneous ureterostomy in 1case and nephrostomy in 1 case.
