3.Antilymphocyte Globulin Therapy for Aplastic Anemia in Children.

Immunosuppressive therapy based on the use of antilymphocyte globulin (ALG) has become standard therapy for patients with splastic anemia who are not eligible for bone marrow transplantation. In this study, T cell subsets before and after ALG therapy, hematologic responses, complications and prognostic factors were analysed. Eleven (42%) out of twenty-six patients treated with ALG showed response, but two patients showed relapse. Most of the response (9 cases) was noticed within 6 months after the initiation of ALG therapy (median: 3 months). The main complications of ALG therapy were fever (91%), thrombocytopenia (86%), neutropenia (63%), and serum sickness (56%). Four patients were died just ALG therapy because of serum sickness (2 cases), intracranial hemorrhage (1 case), and shock (1 case). Short interval from diagnosis to treatment suggested to show good response (P=0.0575), but it was not significant statistically. Lymphocyte subsets were measured in the blood of 23 patients. Helper T/suppressor T cell ratio (T4/T8 ratio) at the initiation of ALG therapy (day 0) was higher significantly in patients who were responded (P=0.0299). The patients who showed above 1.0 of T4/T8 ratio on day 0 might be speculated good response (P=0.032). More difference of T4/T8 ratio between day 14 after ALG therapy and day 0 might show good response (P=0.0673). Then the actuarial probability of survival at 3 years in patients treated with ALG was 77%. Our data suggest that ALG therapy may be used as an alternative treatment to bone marrow transplantation, and T4/T8 ration of peripheral blood at the initiation of therapy may be used as one of the prognostic factors.

7.Mature Cystic Teratoma of the Fallopian Tube: A case report.

Teratomas of the fallopian tube are extremely rare, and only about 50 cases have reported in the world literature. Most cases of mature cystic tubal teratoma are asymptomatic and are discovered as an incidental finding on physical or radiologic examination, or at the time of laparatomy. We report a case of mature cystic teratoma of the right fallopian tube which was discovered during pelvic ultrasonogram for antenatal care in a 28 year old woman. Right salpingectomy was performed during cesarean section for cephalo-pelvic distortion at IUP 39 weeks. Gross examination of the right fallopian tube reveals a distended fallopian tube, measuring 8cm in length and 2cm in diameter. On section, it is filled with several small pedunculated nodules and cysts containing sebaceous materials. Microscopically the tumor was seen in continuity with the lining epithelium of mucosal folds. The majority of the tumor was composed of well differentiated mature elements of three germ layers with skin and skin appendages, mature brain tissue, bone, breast tissue, intestinal mucosa and bronchial epithelium.

8.Two Cases of Allergic Contact Dermatitis to Betadine(R).

Antiseptics are substances that kill or prevent the growth of microorganisms when applied to living tissue. They must be effective against microorganisms but must also retain their activity in presence of body fluids without being harmful locally or systemically. Among many antiseptics, Betadine(R) has been widely used because of its low toxicity and high germicidal efficacy. We reported 2 cases of allergic contact dermatitis to Betadine(R) in surgical patients. They had eczematous eruption along the Betadine(R) applying sites. Path tests Betadine(R) confirmed the diagnosis.

9.A histopathologic study of the seborrheic keratosis.

Seborrheic keratosis is a benign epidermal lesion, that commony develops in the aged. It exhibits a remarkable spectrum of histologic variations. We evaluated one hundred and one cases of seborrheic keratoses according to the histologie typing by Lever and Schaurnburg-Lever. The most common histologic type was the acanthotic type(50.5%), and the next. most common one was hyperkeratotic type(33.7%). The rest of the caces were relatively uncommon, irritated type(6.9%), mixed type(4.9%), clonal type(3.9%), adenoidal type(2.0%), and melanoacanthoma(1.0%). In 2 cases, mixed type was found in a lesion.

10.Initial Clinical Trials of Urinary Stones with Dornier Lithotriptor MPL 9000.

Extracorporeal shock wave lithotripsy was performed in 231 patients for 13 months with Dornier MPL 9000 lithotriptor. The 231 patients had renal ( 48.9%) or ureteral (51.1%) stones varying from 0.6 to 4.4cm (mean 1.2cm) in size. Among 231 patients, 184 patients (79.7%) completed the treatment with 1 session and the number of treatment sessions increased in larger stones and in situ impacted ureteral stones, with an average of 1.3 sessions. The success rate (rate free of calculi plus that of clinically insignificant residual fragments) was 98.3 %. No general or regional anesthesia was required but parental analgesics were required in 22.9% of the treatment sessions. There were no significant complications in any patient. Therefore, extracorporeal shock wave lithotripsy with Dornier MPL 9000 lithotriptor is considered to be a safe and efficient procedure for initial treatment of urinary stones.
