The femoral neck fracture having concomitant fracture of the femoral shaft occurs predaminantly in young man and have known to associate the serious complications. One of such factures is sometimes overlooked and is possibly due to a more obvious nature of the shaft fracture than that of the ipsilateral neck. Six cases of this type of combined fractures in ipsilateral femur are reported with the review of 1 iteratures. In six cases of the femoral neck fractures, three which were unrecognized at initial examiaation in early phase, resulting in serious residue as much as deformity and avascular necrosis of the femoral head, and other three were recognized in early at initial examination healed in good anatomic position and in due time. Through this observation it is stressed that early discovery and adaquate treatment of the fracture is the only solution to minimize the sequelae.

Aneurysmal bone cyst is a rare, non-neoplastic condition, which was first coined by Jaffe and Lichtenstein in 1942. Clavicle is an unusual site for aneurysmal bone cyst. Authors present a case of aneurysmal bone cyst which was found in the right clavicle of a 5 years old boy, because of its rarity of site of predilection.

In order to investigate the factors influencing QOL and life satisfaction of the physically disabled, the author interviewed, by using structuralized questionnaire, 440 individuals among the physically disabled who were participated in Health Examination from April to July, 1997. The questionnaire consisted of the general characteristics(sex, age, marital status, family number, etc), the Reintegration to Normal Living Index(RNLI) to assess QOL, and the single item of five-likert scale to evaluate life satisfaction. The means of RNLI were 16.2+/-4.8 in total score, 12.2+/-3.4 in daily functioning and 4.0+/-2.1 in perception of self. The respondents were less reintegrated toward social activities and relationships than impairments or disabilities. While the satisfied group was 47.3%, the dissatisfied group was 52.7%. As the results of multiple regression and logistic regression analysis, the significant predictors of QOL were age, education, job, grade of disability and subjective health status. The life satisfaction were related to economic status, job and subjective health status. To improve QOL and life satisfaction of the physically disabled, it is important that we improve their basic socioeconomic status by getting a job through rehabilitation education and induce them to have positive self-assessment by extending the opportunity of social participation.

Cryptosporidium, a coccidian parasite first described by Tyzzer (1907) from a laboratory mouse, has become an important human enteric pathogen causing overwhelming diarrhea especially in immunocompromised patients such as AIDS. This parasite has been reported from over 20 countries and is recognized as a cosmopolitan species. In Korea, however, there has been no report on human as well as animal cryptosporidiosis. This study was performed so as to verify the presence of Cryptosporidium in Korea by activating the parasite from laboratory mice by immunosuppression. Total 65 conventionally-bred ICR mice including a control (5 mice) and 3 experimental groups (20 each) were used for this study. Group I was immunosuppressed with prednisolone injection (1 mg IM, every other day) for 7 weeks. Group II (prednisolone injection and tetracycline administration) and Group III (prednisolone injection and trimethoprim-sulfamethoxazole administration) were prepared to observe the effect of antibacterial agents on the activation of cryptosporidiosis. In fecal examinations of mice Cryptosporidium oocysts (4-6 microns in size) were detected from 1 week after the start of immunosuppression and the mice began to die. In H-E stained tissue sections of the lower jejunum, numerous very small (2-4 microns), dense, ovoid or spherical, slightly basophilic bodies were seen attached on the free border of mucosal epithelial cells. In scanning and transmission electron microscopic observations, these organisms were identified as various developmental stages of Cryptosporidium. Peripheral arterial disease(PAD) is a major health problem in the elderly population, so prevalence of PAD is investigated using Edinburgh claudication questionnaire. Subjects were over 65 years old in the kyeongsan county. The prevalence of PAD was 9.0% in total population(8.5% in men, 9.3% in women). definite claudication was 3.9%, atypical claudication was 5.l%. and, grade 1 was 4.4%, grade 2 was 4.6%. The prevalence of PAD was increased with age but not statistically significant. There was no difference of PAD by gender and smoking. PAD was significantly associated with systolic blood pressure positively. But, unexpectedly, PAD was significantly associated with BMl negatively. PAD are important elderly health problem in the our country. so the etiology and preventive methods of PAD will be researched on our country population.

BACKGROUND: During one-lung anesthesia in thoracic surgery, accurate placement of double lumen endobronchial tube(DLT) is essential. We have only a few data regarding adequate depth of DLT, especially after position change. We measured the depth of DLT and the change of depth after position change in 50 adult patients. METHODS: After endotracheal intubation with left-sided Robertshaw type polyvinyl chloride(PVC) endobronchial tube, fiberoptic bronchoscope was inserted through tracheal lumen and we positioned the proximal end of the bronchial cuff just below the carinal bifurcation. The depth of the DLT from the upper incisor was recorded, and was rechecked after position change to lateral position. RESULTS: The depth of the DLT was 29.4 +/-1.3 cm(male: 30.2+/- 0.9, female: 28.2+/- 0.8) and this was related with height. But in female group depth of DLT was not related with height. Within the same height group, the depth of DLT differ up to 3.5 cm. The change of depth more than 0.5 cm after position change ocurred in 21 out of 50 patients. CONCLUSION: The adequate depth of DLT in Korean patients, in total patients and in male group seems to related with their height, but in female group the depth of DLT was not related with their height. Within same height group, the depth of DLT may differ greatly. So, the depth of DLT must be adjusted individually. After position change, the head and neck should be kept in neutral position and the adequacy of the depth of DLT must be rechecked.

Cellular schwannoma is a variant of schwannoma, and is diagnosed as malignant tumor in over one fourth of cases because of its cellularity, mitotic activity and the occasional presence of bone destruction. This tumor is a tumor with low malignant potential and usually occurs in peripheral nervous system, mainly in the posterior mediastinum and retroperitoneal space. But pelvic retroperitoneal cellular schwannoma is very rare. Recently, we experienced a case of pelvic retroperitoneal cellular schwannoma in a 42-year-old woman; in reporting the case a brief review of the literature is included.

Antianginal effect of Trapidil was evaluated in 30 patients (18 male and 12 females) with angina rectoris. The results were follows : 1) Antianginal effect of the drug were good in 18 cases (60%) and fair in 8 cases (37%), and there was no effect in 4 cases(13%). 2) Improvement in ECG changes was observed in 29%. 3) There were no significant change in CBC, RUA, LFT, serum electrolyte and lipid study before and after medication. 4) The adverse effects of trapidil were constipation, gastric cramp and dizziness, respectively one case. But they were not required discontinuing the medicetion. On the basis of these results, Trapidol was evaluated to be promising antianginal drug.
