1.A Case of Torsed Voluminous Parovarian Cyst.

Parovarian cysts constitute 10% of adnexal masses. They were found in broad ligament and mostly arise from mesothelium and less commonly from paramesonephric element and rarely from mesonephric element. In most cases parovarian cysts were asymptomatic. So, they were found incidentally at surgery for other conditions. Torsion is infrequent and it is difficult to distinguish it from torsion of other adnexal masses, appendicitis, etc. Recently, we experienced a voluminous parovarian cyst undergoing torsion, 25 cm in diameter. The case is presented with a review of literature.

2.Pulmonary Embolism following Pelvic Lymphadenectomy for Staging Prostatic Carcinoma.

We reported a case of postoperative pulmonary embolism associated with pelvic lymphadenectomy for staging prostatic carcinoma. A 66-year-old man was admitted to our hospital complaining of dysuria. Transperineal prostatic biopsy under transrectal ultrasonographic guidance revealed evidence of malignancy. Abdominal CT and whole body scan demonstrated no abnormal findings. Staging pelvic lymphadenectomy was performed safely under general anesthesia. On the 6th postoperative day, the patient had sudden dyspnea and chest pain. EKG showed sinus tachycardia. Chest A-P showed some elevation of left diaphragm. Pulmonary perfusion scan revealed multiple cold area throughout both lung fields. A diagnosis of pulmonary embolism was made. Shock treatment and anticoagulant therapy were successfully performed. This case suggests that attention should be directed to pulmonary embolism as one of the postoperative complication of staging pelvic lymphadenectomy.

5.A Case of Secondary Organizing Pneumonia Occurring in Therapy for Lung Abscess.

The patient is a 62-year-old man with known diabetes mellitus who presented with a two-weeks-history of dyspnea, cough, and fever. He was diagnosed with a lung abscess in the right upper lobe and was treated with intravenous antibiotics. The patient's clinical and radiological findings improved within seven days after medical treatment. However, newly developed ground-glass opacity and infiltrations were observed in the right lower lung. Fourteen days after admission, the patient's symptoms and imaging finding became aggravated despite trestment with susceptible antibiotics for lung abscess. Trans-bronchial lung biopsy (TBLB) was performed in the lateral basal segment of the right lower lobe of the lung. A histologic photomicrograph showed organizing pneumonia, also called bronchiolitis obliterans with organizing pneumonia(BOOP), that became more definite as the terminal bronchioles and alveoli became occluded with masses of inflammatory cells and fibrotic tissue. The clinical symptoms and radiograph findings resolved quickly with prednisone treatment. We report a case of secondary organizing pneumonia diagnosed after TBLB following lung abscess treatment and provide a review of the literature.

6.Kostmann's Disease: A Case Report.

Kostmann's disease is a rare hematologic disease entity which is characterized by persistent neutropenia and recurrent bacterial infections since early in infancy. The authors experienced a 2-month-old female infant with disseminated bacterial skin infection since 3 weeks of age and persistent neutropenia. Diagnosis was made by neutropenia in the peripheral blood, maturation arrest of myeloid hemopoiesis at myelocyte stage in the finding of bone marrow aspiration and improving response of neutropenia with G-CSF administration. She had experienced 4 episodes of sepsis and one episode of acute otitis externa during follow-up until 14 months of age.

7.A Case of Tracheostomy Induced Bilateral Tension Pneumothorax.

Tracheostomy is one of the oldest surgical procedures in medical history. The indications for a tracheostomy include the relief of an upper airway obstruction, long-term mechanical ventilation, and decreased airway resistance to help wean the patient from mechanical ventilator support. Unfortunately, tracheostomy is also associated with a number of problems including, bleeding, infection, pneumothorax, and tracheal stenosis. A pneumothorax is an uncommon complication of a tracheostomy, and can result from direct injury to the pleura or positive pressure ventilation through a dislocation of the tracheostomy tube. We report an uncommon case of a tracheostomy-induced bilateral tension pneumothorax with a review of the literature.

8.A Case of Ruptured Abdominal Pregnancy Associated with Endometriosis.

Abdominal pregnancy is a rare form of extrauterine pregnancy which is account for 1.4% of all ectopic pregnancy cases. Among these omental pregnancy is extremely rare and may cause life-threatening massive hemorrhage as a complication. But the diagnosis of early abdominal pregnancy is difficult because of non specific findings about clinical history, physical examination and ultrasonography. Risk factors related to ectopic pregnancy include inflammation, infection and surgery, but endometriosis is an uncommon factor. We present a case of the ruptured primary omental pregnancy associated with endometriosis with a review of the literature.

9.Characteristics of dog-bite injuries and factors affecting hospitalization in a single-center emergency department setting in Korea

Objective:
We conducted this study to examine the characteristics of dog-bite injuries and to analyze the factors affecting hospitalization in a single-center emergency department (ED) setting in Korea.
Methods:
The current single-center, retrospective, cohort study was conducted on 202 patients with dog-bite injuries who visited our medical institution between 2011 and 2019 through a retrospective analysis of their medical records.
Results:
The Pearson’s chi-square test indicated that outpatient treatment had a significant positive correlation with the location of the bite. Also, hospitalization had a significant positive correlation with age and the depth of the bite wounds (P=0.002, P=0.031, and P=0.034, respectively). On multiple regression analysis, it was observed that hospitalization had a significant positive correlation with age (odds ratio [OR], 1.421; 95% confidence interval [CI], 1.047-1.529; P=0.011) and the depth of the bite wounds (OR, 1.077; 95% CI, 1.013-1.139; P=0.014).
Conclusion
In conclusion, our results indicate that older patients and those with deeper bite wounds are vulnerable to hospitalization. This would be helpful in an ED setting for dispensing treatment to those with dog-bite injuries.
