1.A Case of Idiopahic Calcinosis of the Scrotum.

Idiopathic calcinois of the scrotum which was described by Shapiro et al. in 1970 is a rare cutaneous disorder and it seems that this is tbe first case reporting in Korean literature. A 26-year old male was well until age 25 when a few, small nodule developed on the scrotal area and, thereafter, have increased in the numbers and size of the nodules progressively. The skin lessions showed multiple, yellowish, firm, round, smooth nodul ranging in size from that of the head of a pin to that of a pea. Clinica,lly they seemed to be steatoma, but histologic findings showed several islands of calcified amorphous material without: any evidence of residual cysts. He appeared to be in good general health and had an apparently normal skin except for hie scrotum.

3.A Case of Acardiac - Acephalus Twin Diagnosed by Antenatal Ultrasonography.

The acardius occurs with an incidence of one in 35,000 births and is presumed to result hom vascular anastomosis between the arterial circulation of the hemodynamically larger ""pump"" twin and that of the recipient ""perfused"" twin. The perfused twin displays severe anomalies, typically referred to as acardia, that are incompatible with life. The pump twin, because of its expanded cardiac demand, may also deveolp cardiac failure, with a mortality risk of 50-70%. We experienced one case of acardiac-acephalus twin gestation diagnosed by antenatal ultrasonography, and presented this.

4.A Case of Marjllin's Ulcer.

Marjolin is ulcer may be defined briefly as a cancer arising in a bum scar. A 41-year-old female had a Marjolin is ulcer(12x10x3cm) on her right lower leg. This ulcerated tumor has been present for two and half months and the induction period sinnc her bum was 29 years. The pathologic diagn.osis was squamous cell carcinoma, grade 2. Roentgenograms of her right lower leg revealed several soft tissue masses densities which, in part, horizontally connected with the periosteum of the tibia. So she was treated by above-knee amputation with dissection of several, unenlarged inguinal nodes which showed no evidence of metastases. She developed large, single, fixed metastatic node in right inguinal area one month after amputation and dissection was done.

5.A case report of orthodontic treatment for adult patient with upper prognathism.

A 23 year-old female with skeleto-dentoalveolar protrusion of maxilla, minor broken contact points between anterior teeth, and missing of lower 1st molars, has been treated with multibanded edgewise technique. After treatment of 14 months, she has gained functional overbite-overjet relationship and facial harmony due to the retraction of upper anterior teeth. Root resorption was slight. Especially, us ing the space of missed lower 1st molars instead of extracting lower premolars, expected and favorable results were obtained.

8.The Investigation of Epidemiological and Clinical, Laboratory Characteristics of Acute Febrile Illness of Autumn-Winter Type in Rural Communities of Korea.

BACKGROUND: Tsutsugamushi disease, Leptospirosis, and Hemorrhagic fever with renal syndrome(HFRS) are an acute febrile illness of autumn-winter type which are characterized by fever, headache, myalgia in acute stage. We reported the clinical and laboratory characteristics of 49 cases under investigation far febrile illness in rural communities of Korea in order to differentiate the diagnosis among Tsutsugamushi disease, Leptospirosis, and HFRS. METHODS: This study was done retrospectively by investigation of patient's age, occupation, symptom, laboratory findings and chest X-ray findings of Tsutsugamushi disease, Leptospirosis, and HFRS. All patients was seen in our hospital between September and November in 1997 and 1998 consecutive year. RESULTS: The most characteristic symptoms and signs of Tsutsugamushi disease were eschar(90.1%), and skin rash(70%). The epidemiological and laboratory characteristics of Tsutsugamushi disease were age 40 or more, farmer(90.1%), s-GOT(serum glutamic oxaloacetic transaminase) and s-GPT(serum glutamic pyruvic transaminase) elevation. The most characteristic symptoms and signs of Leptospirosis were sputum and cough(75%). The epidemiological and laboratory characteristics of Leptospirosis were all farmers(100%), hyperbilirubinemia(1.67 mg/dl). The most characteristic symptoms and signs of HFRS were gastrointestinal disturbance(100%), flank pain(66.7%). The epidemiological and laboratory characteristics of HFRS were leukocytosis, thrombocytopenia, azotemia, proteinuria (91.6%), and hematuria (83.3%). CONCLUSION: As the clinical feature, presence of eschar and skin rash were suggestive of Tsutsugamushi disease. As the clinical feature, presence of cough and sputum and laboratory finding such as elevation of bilirubin level were suggestive of Leptospirosis. As the clinical feature, presence of gastrointestinal disturbance, flank pain and laboratory finding of thrombocytopenia, and renal dysfunction were suggestive of HFRS.

9.Effect of Sequential Embryo Transfer in vitro Fertilization.

OBJECTIVE: The objective of this study is to nfluence of sequential embryo transfers in an invitro fertilization was examined. METHOD: After in vitro fertilization, a maximum of 6 fertilized oocytes was enrolled in this study. At day 3 after an oocytes retrieval, embryos with good quality were transferred (mean 4.9), remaining embryos (mean 2.0/cycle) were cryopreserved at blastocyst stage (Group 1). At day 5 after oocytes collection, second a embryos had reached the blastocyst stage (Group 2) using P1 supplemented with 10 SSS and 30% Follicular fluid. No statistical difference in the pregnancy rate could be seen between the group without a second embryo transfer (n=21; 28.6%) and the group with a second transfer (n=52; 28.8%). RESULTS: The incidence of multiple pregnancy rate per embryo transfer was not statistically different between both group and no high-rank multiple pregnancy (greater than triplete) were observed (o.9%, 15.4%, respectively, p=0.74, chi2). Out of 114 cycles (506 embryos) cultured embryos in group 2, 52 cycles (159 embryos, 29.8%) reached the blastocyst stage. CONCLUSION: The second transfer did not have a significant effect on the pregnancy rate. The most important factor for the pregnancy seems to be the quality of the embryos transferred on day 3 following oocyte retrieval. We recommend embryo transfer is performed only one, day 2~3 or D5.
