1.A Case of Idiopahic Calcinosis of the Scrotum.

Idiopathic calcinois of the scrotum which was described by Shapiro et al. in 1970 is a rare cutaneous disorder and it seems that this is tbe first case reporting in Korean literature. A 26-year old male was well until age 25 when a few, small nodule developed on the scrotal area and, thereafter, have increased in the numbers and size of the nodules progressively. The skin lessions showed multiple, yellowish, firm, round, smooth nodul ranging in size from that of the head of a pin to that of a pea. Clinica,lly they seemed to be steatoma, but histologic findings showed several islands of calcified amorphous material without: any evidence of residual cysts. He appeared to be in good general health and had an apparently normal skin except for hie scrotum.

2.On the conjunctival reaction of leprosy with the Mitsuda-antigen(Lepromin).

Conjunctival and dermal injection of lepromin were made on the various lepers, 9 neurals, 7 lepromatouses, 3 mild lepromatouses, 15 neurals and 7 lepromatouses associated some ocular lesions each group. The following results were obtained. 1. In general, conjuntival reaction are shown similarly with dermal reaction. 2. Have not found out the conceivable specific ocular immunity for the antigen on this work.

3.A Case of Marjllin's Ulcer.

Marjolin is ulcer may be defined briefly as a cancer arising in a bum scar. A 41-year-old female had a Marjolin is ulcer(12x10x3cm) on her right lower leg. This ulcerated tumor has been present for two and half months and the induction period sinnc her bum was 29 years. The pathologic diagn.osis was squamous cell carcinoma, grade 2. Roentgenograms of her right lower leg revealed several soft tissue masses densities which, in part, horizontally connected with the periosteum of the tibia. So she was treated by above-knee amputation with dissection of several, unenlarged inguinal nodes which showed no evidence of metastases. She developed large, single, fixed metastatic node in right inguinal area one month after amputation and dissection was done.

9.Comparison of Therapeutic Result of Oral Corticosteroid Versus Intravenous Gammaglobulin in Childhood Acute ITP.

A clinical study was conducted to compare effectiveness of IV Globulin with that of steriod in childhood acute ITP. We carried out an experiment to 36 patients of acute ITP who recovered over 50.000/mm(3) of platelet count and not to recur in 6 months. Study group consist of 10 children who were treated with steriod only (Group A), 13 children who were treated with IV Globulin (0.4g/kg/d, 5 days, Group B), and 13 children combined with steroid (Group C) Study period was from January 1988 to December 1992, folllowing results were obtained. 1) There were no significant difference in sex ratio. age distribution, pretreatment hemoglobin level, platelet count and prolonged bleeding time among study groups. 2) The days which platelet count reached to 50.000/mm(3), 100,000/mm(3) were 3.7 days (Group A), 2.1 days (Group B), and 2.3 days (Group C), respectively (p<0.05). 3) Complete response rates within 4 weeks did not show significant difference (90% in Group A, 84.6% in Group B, 92.3% in Group C), the complete response rate within 1 week were significantly different among three group (44.4% in Group A, 90.9% in Group B, 83.3% in Group C)(p<0.05). In conclusion, it is more desirable for early treatment of a childhood ITP because IV Globulin is easy to normalize platelet count in a short time.

10.Local Control of Local Excision for T1/T2 Rectal Cancer .

PURPOSE: The aim of this study was to review the outcome of local control after the local excision for T1/T2 rectal cancers and, thus, to assess its effectiveness as an alternative to a more radical resection. METHODS: This retrospective study analyzed 23 patients with T1/T2 rectal cancer treated by local excision (LE), and their results were compared with the results for 22 patients with rectal cancer of the same stage treated by a radical resection (RR). All patients with pT2 lesions in the LE group received postoperative adjuvant chemoradiation. The outcomes were defined as 5-year local-recurrence-free survival (LRFS). The median follow-up was 72 (range, 40~92) months. RESULTS: Recurrence occurred in 4 patients (pT1, 1; pT2, 3) in the LE group and in 3 patients (all pT2) in the the RR group. One patient with vascular invasion (T2N1M0) in the RR group showed multiple liver metastases at 23 months postoperatively. The difference in 5-year LRFS was not statistically significant between the two groups. In the LE group, the 5-year LRFS for pT2 lesions was significantly less favorable than that for pT1 lesions (40% vs. 94%; P= 0.005). The 5-year LRFS for pT2 in the RR group was more favorable than that in the LE group, although the difference was not statistically significant (76.9% vs. 40%, P=0.138). CONSLUSIONS: Local excision provides a favorable local control for pT1 rectal cancers. A more radical resection, however, remains an effective surgical option for pT2 lesions because local excision, even combined with adjuvant chemoradiation, showed substantial local recurrences.
