2.The Esthetic Upper Incisor Position in Korean Adult Female.

Esthetic is one of the prime goal of the orthodontic treatment. The upper incisor position in important because it is decisive in balanced smile. This study was attempted to obtain the standards and equations on the esthetic upper incisor position. Lateral cephalometric radiographs of 66 Korean young adult females who were candidates of Miss Korea Beauty Contest were traced, measured and statistically analyzed. The results were as follows ; 1. The mean and standard deviation of the measurement for upper incisor position were obtained in the Korean Beauty. 2. Among the measurements of the axis of upper incisor, 1 to SN and 1 to FH were high1y correlated with FH-MP, ANB, FH-N Pag, Y-axis, but 1 to SN (Is-Pr) to present axial inclination of labial surface of upper Incisor didn't have any correlation with the measurements for dentofacial pattern. 3. Among the measurements for the vertical position of upper incisor, 1 to stom had low correlation with to measurements for dentofacial pattern and 1 to PP(mm) was highly correlated with the measurements for facial growth direction. 4. Sixteen multiple regression equations were established to predict upper incisor position balancing with dentofacial pattern. 5. It does not have much power of persuasion to predict esthetic upper incisor position from the measurements for dentofacial pattern.

3.Histopathologic features of squamous cell carcinoma.

BACKGROUND: Squamous cell carcinoma(SCC) is one of the most conimon forms of skin cancer that arises from the surface epidermis. Although many authors have reported on the incidence and clinical features of SCC, we could not find any reports concerning the widerange of histopathologic features of a number of cases of SCC in Korean dermatologic literature. OBJECTIVE: We examined the histopathologic features of SCC. METHOD: Twenty six biopsy specimens from 26 patients with SCC were examined microscopically including the tumors themselves and surrounding epidermis and dermis on routine histologic staining. RESULTS: Grade 1 tumor was found in 15 cases, grade 2 in 8 casese, and grade 3 in 3 cases. Eighteen cases showed keratin pearls, and 11 cases showed squamous eddies. P cantholysis of tumor cells was found in all 26 cases, but glandular structure was found only in 5 cases. The epidermis overlying the tumor showed hyperkeratosis and parakeratosis in all 26 cases, follicular plug in 13 cases, and ulceration in 18 cases. The dermis beneath the tumor showed vessel dilataticn, hemorrhage, and infiltration of inflammatory cells in all 26 cases, and basophilic degeneration in 1.3 cases, fibrosis in 20 cases, lichenoid reaction in 6 cases, and necrosis in 6 cases. Rare dermal findings were abscess, foreign body reaction calcification and Russell body. CONCLUSION: The histopathologic findings of SCC were variable not inly those of the tumors themselves, but also those of the surrounding epidermis and dermis.

4.A Clinical Study on Coenzyme Q10(Neuquinon(R)) in the Treatment of Congestive Heart Failure.

Coenzyme Q is concentrated in Golgi apparatus membranes and mitochondria, but not in other membranes. Although it is difficult to prove the metabolic action of coenzyme Q administered exogenously in clinical cases, the effect of this substance can be evaluated by criteria based on clinical findings. In an attempt to evaluate the effect of coenzyme Q for the treatment of 67 patients(male 26 cases, female 41 cases) of congestive heart failure, we administered Coenzyme Q1030mg daily for 4 to 8 weeks. Most of them were valvular heart disease(74.6%) and hypertension (14.9%). Clinical effects were evaluated at least 4 weeks later by the criteria using a scoring method of severity of congestive heart failure which was devised by Ishiyama, etc. In summary, a definite effect was found in 13 cases(19%) and a mild effect was observed in 46 cases(69%). During treatment there were no significant side effects, and also no significant changes in heart rate and blood pressure.

5.Incidence of Vesicoureteral Reflux in Asymptomatic Siblings of Patients with Reflux.

PURPOSE: Vesicoureteral reflux(VUR) is defined as a retrograde flow of urine from the bladder into the upper urinary tract. It has been shown to predispose patients to hypertension, renal scarring, and end-stage renal failure if not recognized and treated. The observation that VUR occurs in siblings of children with reflux at a significantly higher rate than the general pediatric population has been recognized for many years and VUR was detected in 26% to 51% of siblings of patients with VUR. The purpose of this study is to determine the incidence of VUR in asymptomatic siblings of children with VUR and to see if this form of screening would be practical. METHODS: We retrospectively reviewed the records of 28 patients with VUR and their siblings. The total number of asymptomatic siblings investigating VUR were 28 persons. All patients and siblings were evaluated for VUR by a voiding cystourethrography and all patients and siblings with VUR were performed 99mTc 2,3-dimercaptosuccinic acid renal scan. RESULTS: A total of 28 patients(14 boys, 14 girls) with VUR were studied; the mean patient age was 2.7 years(range 1 month to 8.4 years). The total number of asymptomatic siblings investigating VUR were 28 persons(17 boys, 11 girls) and the mean age was 3.3 years(range 2 months to 7.4 years). Renal scar was detected in 20 of 28(71.4%) patients with VUR. VUR was noted in three of 28(10.7%) siblings and renal scar was detected in one of three siblings with VUR. CONCLUSIONS: In this study, the predictive value of a positive family history alone in identifying VUR was 10.7%. This incidence suggests more investigation of asymptomatic siblings and continued study of this group of patients at risk is needed for clarifying the family screening of patients with VUR.

6.Anthelmintic effects of single doses of fenbendazole and oxantel-pyrantel pamoate to the intestinal nematodes.

The present study was undertaken to evaluate the anthelmintic effects of fenbendazole against intestinal nematode; Ascaris lumbricoides, hookworm and Trichuris trichiura, and to compare the efficacy in fenbendazole, oxantel-pyrantel pamoate and placebo by means of double blind method. Out of 114 subjects harbouring Ascaris lumbricoides, hookworm and Trichuris trichiura, 36 cases were treated with single dose of fenbendazole, 38 cases with oxantel-pyrantel pamoate, and the remaining 40 cases had received the placebo. The results were as follows: In the group treated with fenbendazole (30-50 mg/kg), the cure rates were 83.9 percent in 31 subjects with Ascaris lumbricoides and 83.3 percent in 18 subjects with hookworm, and only 28.6 percent in 28 subjects with T. trichiura respectively. In the group treated with a single dose of oxantel-pyrantel pamoate (10 mg/mg), the cure rates were 96.7 percent in 30 subjects with A. lumbricoides, 95.2 percent in 21 subjects with hookworm, and 54.6 percent in 33 subjects with T. trichiura. Egg reduction rate was 85.7 percent in T. trichiura cases. On the other hand, the egg negative conversion rates in placebo group were 9.7, 8.3 and 33.3 percent in Ascaris, Trichuris and hookworm infections respectively. The above results showed that fenbendazole was highly effective against Ascaris and hookworm. However, incomparisom with oxantel-pyrantel pamoate, fenbendazole was less effective in regards of A. lumbricoides, hookworm and T. trichiura infections.

8.A Case of Glosspharyngeal Neuralgia.

The authors report a case of glossopharyngeal neuralgia which was successfully treated with intracranial section of the glossopharyngeal and upper two rootlets of the vagus nerves. Despite of the complete section of these nerves, corresponding sensory or motor deficit was not developed. And the histories of this surgery were reviewed.

9.Clinical Experience of Ureterolithotomy by Posterior Vertical Incision.

The authors obtained the following results through comparative investigation between experiences from authors clinical study of ureterolithotomy by posterior vertical incision devised by Gil-Vernet and Yoon`s clinical study of ureterolithotomy by posterior vertical incision and his lumbar incision. 1. Operation time was average 62 min. and blood loss was average about 50cc. So operation time and blood loss were similar to Yoon`s result but more or less reduced as compared with lumbar incision. 2. Postoperative urine leakage and ambulation time were 1 day and 2 day that were more or less reduced as compared with Yoon`s result. 3. Ipsilateral lower abdominal wall protrusion was noted postoperatively. We think, this is from subcostal nerve injury. 4. Postoperatively sensation change on subcostal nerve, ilohypogastric nerve and ilioinguinal nerve innervation sites were noted in half of all, but disappeared in several weeks. Considering above results, while posterior vertical incision has merits such as minimal operative invasion and early postoperative ambulation time, but it often notes inevitable nerve damage. So, we think better results would be gained by careful attention to nerve and vascular distribution.

10.Cardiovascular aspects of aconitine poisoning.

BACKGROUND AND OBJECTIVES: The Oriental herbal materials known as aconitine have long been used in oriental traditional medicine for their analgesic and antiinflammatory effects. Aconitine and its related alkaloids are known cardiotoxins with no therapeutic role in modern western medicine. We have studied the cardiovascular side effects of intoxication that took place in otherwise healthy individuals after ingestion of herbal decoctions containing aconite alkaloids. MATERIALS AND METHOD: During a six-year interval from 1990 to 1996, 9 cases of accidental herb-induced aconitine intoxication were managed in Kyung Hee university medical center. Hospital records were reviewed in detail. RESULTS: All patients developed symptoms of aconitine toxicity within 4 hours of herb ingestion. The frequency of the order in cardinal symptoms of acute aconitine poisoning was nausea or vomiting, irritability, chest discomfort, dizziness, etc. Nine patients developed arrhythmias, including multifocal APC with aberrancy, multifocal VPC, ventricular tachycardia, etc. Administration of isotonic saline, dopamine, atropine and lidocaine with supportive cares brought clinical recovery and disappearance of arrhythmias in most cases within several hours. However, one case of acute aconitine poisoning had been dead of cardiac arrest due to ventricular fibrillation. CONCLUSION: Aconitine and its related alkaloids can cause toxic effects and even fatal poisoning. These cases point to the need for strict surveillance of herbal substances with low safety margins.
