3.Surgical Treatment of Posterior Cruciate Ligament Injury

It is generally accepted that integrity of the posterior cruciate ligament plays a major role in knee stability. The potential disability resulting from disruption of the posterior cruciate ligament is sufficient to warrant aggressive operative management when the lesion is discovered. From August 1980 to July 1982, the authors treated the 8 cases of the posterior cruciate ligament injury at the department of orthopedic surgery, Chung Ang university hospital; primary repair(2 cases), reconstruction using the medial head of the gastrocnemius muscle(6 cases). In the operative procedure of reconstruction using gastrocnemius, we experienced the following obstacle and resolved it effectively. l. In the surgical approach, we choosed the separate incision on the anteromedial and posteromedial aspect of knee and satisfactory results were obtained. 2. In adequate length of the gastrocnemius tendon was resolved by releasing of gastrocnemius as closer to it's femoral condyle origin as possible or together with attached periosteum and bone chips, by fixation of gastrocnemius with pull through wire suture method. 3. In advancement of the gastrocnemius into the knee joint, we used No. 32 chest tube and achieved the smooth enterance. 4. Adequate position of the femoral condyle drill hole was achieved by using hip screw reamer and K-wire.

6.Complications after lumbar myelography with amipaque

Amipaque is a water soluble, non-ionic myelographic contrast media, and owing to its high diagnostic accuracy and safety, its use is gradually increasing. The authors studied the complications after Amipaque lumbermyelography in 61 patients with low back pain during the period from Jan. 1981 to Nov. 1981 in Chung Ang University Hospital. The results were as follows; 1. Total complication rate was 52%(32 of 61) and there was nosexual difference in its occurrence. 2. In total, no difference in complication rate was found between head-up positioned group with a degree of 30degrees (group I) after procedure and head-up positioned group with a degree of 70degrees (group II) but female patients had more complication rate in group I than in group II (75% vs 50%).Headache was more common in group I and nausea was more common in group II. 3. Headache was most common complicaiton (44%) and there was no sexual difference in its occurrence. 4. No significant difference in complication rate was found between patients proved to have HBP and patients to have not. 5. Complications were less common in patients with punctured level of L4-5 than in patients with L2-3 or L3-4 level puncture.

7.A Study on the Peripheral Anticholinergic Effect of Minaprine.

The authors studied ED50 of bethanechol on the contractilities of the smooth muscles isolated from various organs of rat under the presence of atropine (a classical competitive blocker of cholinergic muscarinic receptor) or minaprine (a newly developed antidepressant drug) to investigate the peripheral anticholinergic effect of minaprine. The results were as follow: 1) There was no significant difference between ED50 of bethanechol in the control group and that under the presence of minaprine 10⁻⁸M and 10⁻⁷M in the smooth muscles isolated from the duodenum. 2) There was no significant difference between ED50 of bethanechol in the control group and that under the presence of minaprine 10⁻⁸M and 10⁻⁷M in the smooth muscles isolated from the ascending colon. 3) There was significant difference between ED50 of bethanechol in the control group and that under the presence of minaprine 10⁻⁸M and 10⁻⁷M in the smooth muscles isolated from the urinary bladder (P<0.01) 4) There was significant difference between ED50 of the atropine 10⁻⁸ and minaprine (10⁻⁸M) in the smooth muscles isolated from the urinary bladder (P<0.05).

10.Vaginal Ureterolithotomy: A Case Report.

Herein we report an experience of the vaginal ureterolithotomy. The patient was a 48-year-old woman with a juxtavesical left ureteral stone, which was vaginally palpable. Although this operation has limited indications, it is a safe and easy method of removing stones from the lowermost ureter especially in the women. This method seems to have considerable value to be attempted in the carefully select patients.
