The clinicolaboratory findings, cardiac catheterization, and outcome of operation were analyzed in 78 patients with tetralogy of Fallot, which were diagnosed by echocardiography, cardiac catheterization, and a cardiac angiography and confirmed by operation at Dong San Hospital, Keimyung University, during a 4(1/2)-year period from January 1984 to June 1988. The following results were obtained : 1) Out of the 78 cases, 45 were male and 64 were female. 2) The electrocardiogram showed right axis deviation in 71 cases(91%) and right ventricular hypertrophy in 66 cases(84.6%). 3) Chest X-ray revealed that, the cardiothoracic ratio was normal or decreased in 68 cases(87.3%), and cardiac apex elevation was noticed in 57 cases(73%). 4) Pulmonary stenosis were chiefly valvular and infundibular type(53.8%). 5) The associated heart diseases with TOF were patent foramen ovale(64.1%), right sided aortic arch(19.2%), and secundum ASD(10.3%), in that order. 6) There was an intimate correlation between secondary polycythemia and thrombocytopenia. 7) Among the 78 Cases, 73 cases had total correction, 1 case had shunt operation, and 4cases had total correction after shunt operation. The highest mortality rate occurred in the cases of total correction after shunt operation(25%). The cases with a main pulmonary artery size of 1/3 to 2/3 against, the aorta had a higher mortality than the other group, and no cases expired in the group with a ratio of more than 2/3. The overall surgical mortality was 8.9%.

In the unconscious patient with general muscle rigidity, the natural teeth can be luxated and then bite wounds may be occurred around lip, owing to the pathologic involuntary self-biting habit. If the forceful biting is generated continuously, the teeth may be avulsed and aspirated into the aerodigestive tract with the infection of biting wound. For the prevention of pulmonary aspiration of the teeth and wound infection, the biting teeth should be cared before the fact. The authors treated the teeth as endodontic drainage with removal of the crown or iodoform gauze drainage into the socket with extraction of the teeth. The prognosis was more favorable without biting wounds.

Tsutsugamushi disease is an acute febrile disease caused by Rickettsia tsutsugamushi, and which has been reported with increasing frequency thorough the nation since 1986. We experienced 21 cases of Tsutsugamushi Disease diagnosed with serologic test occurring in Taegu city and Kyungpook province during October-November, 1989. The results of survey are as follow. 1) Of 21 cases, 12 (57%) were males and 9 (43%) were females, and the peak incidence was the 4th decade. 2) The outbreak was in October to November and the peak incidence was in October. 3) The most frequent symptoms were fever and chill (100%), myalgia (95%), headache (90%), Eschar and rash were observed in 18 patients (86%) and the eschar was detected in all over the body, especially thorax (33%) and lower extremity (22%). 4) Laboratory features were SGOT elevation (83%), SGPT elevation (61%), LDH elevation (67%), leukocytosis (38%). 5) Indirect immunofluorescent antibody test was done in 18 patients and the antibody titer was above 1:320 in all patients. 6) The chloramphenicol, tetracycline or doxycycline regimens were very effective and mean duration of defervescence from initiation of therapy was 1.3 days. 7) The complication such as meningitis or shock, was not seen.

There exist some restrictions and difficulties in performing follicular unit extraction (FUE) in white-haired patients, for several reasons. In this paper, we introduce a novel technique for visualizing white hair during the punching procedure and graft preparation in FUE for white-haired patients. In white-haired older male patients, we dyed the surrounding scalp skin purple with a gentian violet solution-stained toothpick. Our method has several advantages: surgeons can easily focus on the center of the follicular unit and rapidly perform punching, they can recognize the condition of the harvested follicular units during FUE, and the hair transplant team can secure a clear view for trimming and loading into the implanter. We suggest that scalp dyeing in difficult FUE procedures, especially in patients with white hair, may be a simple method that provides a good visualization for donor site harvesting and for microdissection.

PURPOSE: To obtain a recent epidemiological data on diabetics with diabetic retinopathy in a tertiary hospital. METHODS: We reviewed the medical charts of 1041 diabetic patients who visited on eye clinic between 1997 and 2001. We also analyzed the result of first eye examination of 325 patients diagnosed to have diabetic retinopathy. RESULTS: Diabetic retinopathy was diagnosed in 325 patients (31.2%) out of 1041 diabetic patients (41 type I and 1000 Type II patients). Among 325 patients, proliferative retinopathy was diagnosed in 109 patients (10.5%). The longer was the duration of diabetes, the higher was the prevalence of diabetic retinopathy among type II diabetics. Among type I diabetics macular edema was identified in 38.9% (7 eyes/18 eyes) of non-proliferative retinopathy patients, and 90.0% (18 eyes/20 eyes) of proliferative retinopathy patients. Among type II diabetics this prevalence was 35.0% (145 eyes/414eyes) in non-proliferative retinopathy patients, and 73.7% (146 eyes/198 eyes) in proliferative retinopathy patients. Visual acuity tended to be low in diabetics with macular edema. Only 497 patients (47.8%) were those who were referred by their primary doctors. CONCLUSIONS: The prevalence of diabetic retinopathy was 30~40% among diabetics seen in tertiary hospital, and thus the clinical importance of diabetic retinopathy increased. Education of diabetic patients and deep understanding on diabetic retinopathy by primary doctors are needed in the future.

In contrast to previous dogma, the decline in testosterone levels in postmenopausal women is thought to be age-related and not a specific function of the menopause. In addition to normal aging, many conditions affecting ovarian and adrenal function, factors leading to increased SHBG levels can decrease androgen levels below the physiologic range in women. Clinical symptoms of female androgen deficiency include a loss of libido, lack of well-being, and persistent fatigue. However, the diagnosis of female androgen deficiency, as recently provided by the Princeton consensus statement, is under debate due to several limitations. Testosterone therapy in postmenopausal women with symptoms suggestive of androgen deficiency remains controversial. The adverse effects of testosterone therapy appear to be low in incidence, but more studies are necessary to assess long-term safety. Therefore, the decision to treat or not to treat androgen deficiency in postmenopausal women must be carefully considered on an individual basis by comprehensive evaluation of potential benefits and risks.

The experienced surgeon can be surprised & challenged by the hazards of active bleeding during oral & maxillofacial surgical procedure, because of alterations in the surgical anatomy, bleeding disorders and surgical intervention of infected tissues. This is a report of two cases of active bleeding during surgical extraction of mandibular third molar, that had the pericoronitis, osteitis and adjacent neurovascular bundle in its apex. When the abrupt active bleeding was occurred during surgical extraction of mandibular third molar, pressure packing by hemostatic agent(bone wax) & wet gauze biting were applied into the extraction socket during 30 minutes. After 30 minutes, the wound was explored about the bleeding and active bleeding was then continued. In spite of repeated bleeding control method of the pressure dressing, the marked hemorrhage was generated continuously. Therefore, the author decised the bleeding as immediately uncontrollable hemorrhage and the pressure dressing was again applied for the more longer duration without wound closure. After 3 days, the pressure dressing was removed and iodoform gauze drainge was then established without the bleeding. The drain was changed as the interval of 3~5 days for prevention of infection & secondary hemorrhage and relatively good wound healing was then resulted in 6 weeks.
