1.Renin Release by Adenosine Agosists and Antagonists in Two-Kidney One Clip Goldblatt Hypertensive Rats.

BACKGROUND: In two-kidney one clip Goldbaltt hypertensive rats(2K1C GHR), clipped kidney may be exposed to low pressure and unclipped kidney to high pressure. In addition, both kidneys may have a different amount of adenosine which is increased by ischemia and plays an important role for renin release. The aim of this study was to invstigate the responsmiveness for renin release to adenosine agonists and antagonist in clipped and unclipped kidney of 2K1C GHR. METHODS: Emplying kidney slices from both unclipped and unclipped kidney of 2K1C GHR, the alteration by adenosine agonists and antagonist of renin release was studied. RESULTS: The renal renin content and basal renin release from unclipped kidney slices were suppressed, whereas those from clipped kidney were augmented Adenosine Al receptor agonist, cyclohexyladenosne(CHA), phenylisopropyl adenosine(PIA) and adenosine caused a decrease in renin release from clipped kidney slices. Adenosine A2 receptor agonist, NECA, and nonspecific adenosine receptor aganist, 2-chloroadenosine(CA) caused an increase in renin release from clipped kidney slices. Adenosine receptor antagonist, 8-phenyltheophylline(8-PT) caused an increase in renin release from clipped kidney slices. In unclipped kidney, however, the renin release in response to NECA, CA or 8-PT was reversed and the decreasing effect of renin release to CHA and adenosine was slightly inereased. CONCLUSION: These results suggest that the responsiveness of adenosine receptors, which may participate in renin release is modified in clipped and unclipped kidney of 2K1C GHR.

2.A Study on Serum IgE Levels in Patients with Scabies.

Serum IgE levels in 48 patients with scabies were measured by enzyme-linked immunosorbent a.ssay(ELISA) to evaluate the relationship between the scabies and serum IgE. A group of 44 healthy persons was taken as a controls. The results were summarized as follows: In children group(p 15 year-old) of the scabies, serum IgE level was 446 3 +267.2 IU/ml and significantly elevated than in controls, 126, 3 175, 4 IU/rnl, In adult group(over 16 year-old), serum IgE levels of the scabies and controls were 506. 4 + 581. 8 IU/ml, 435, 5+ 548, 8 lU/rnl respetively, and there was no significant difference between them. 2) There were no statistical correlations between the serum IgE levels and intensity of pruritus, duration of illness, number of skin lesions or number of circu1ating eosinophils in patients with scabies.

3.Clinical Experience of Three-dimensional Reconstruction Using Free TRAM Flap after Total Maxillectomy with Orbital Exenteration.

PURPOSE: Extensive midface defect following total maxillectomy with orbital exenteration and cheek skin resection should be three dimensionally reconstructed with a large flap that have a sufficient volume of tissue and multiple skin islands. We describe our transverse rectus abdominis myocutaneous(TRAM) free flap with three skin islands which was successfully used in this situation. METHODS: A 58-year-old man was performed enbloc total maxillectomy including orbital contents and wide cheek skin because of invasive maxillary squamous cell carcinoma. He was immediately reconstructed with TRAM flap that was designed not vertical but transverse fashion for providing sufficient skin area. Also, deepithelialization procedure making for multiple skin islands was done in flap insetting period when appropriate modification according to the intraoperative situation was possible. Dead space was completely obliterated by bulky muscular tissue, and three skin islands were used for lining of lateral nasal wall, palatal surface, and cheek skin restoration. RESULTS: Postoperative course was satisfying. Maintaining of proper ipsilateral nasal airway, loss of rhinolalia and oronasal regurgitation of food particles, and restoration of cheek contour were successfully obtained. CONCLUSION: We report clinical experience of threedimensional reconstruction using free TRAM flap after total maxillectomy with orbital exenteration.

5.Characteristics of Heat Waves From a Disaster Perspective

In September 2018, heat waves were declared to be a type of natural disaster by the Framework Act on the Management of Disasters and Safety. The present study examined the characteristics of heat waves from the perspectives of meteorological phenomena and health damage. The government’s efforts to minimize the damages incurred by heat waves are summarized chronologically. Furthermore, various issues pertaining to heat waves that are being raised in our society despite the government’s efforts are summarized by analyzing big data derived from reported news and academic articles.
6.The prevalence of intestinal helminthes in inhabitants of Cheju Do.

The authors examined 3,169 fecal specimens from inhabitants of seven localities such as City, Eups and Myons in Cheju Do on July 1970 with cellophane thick smear technique. The results were analysed and summarized as follows: The overall egg positive rate of helminths was 82.2% and it was attributed to high rates of soil-transmitted helminths as in case of other parts in Korea. The infection rates of each species were; A. lumbricoides 44.3%, T. trichiurus 65.6%, hookworm 1.5%, T. orientalis 0.8%, C. sinensis 0.2%, P. westermani 0.4%, M. yokogawai 0.9%, Taenia sp. 12.7% and H. nana 1.4%. The infection rates of soil-transmitted helminths were relatively lower than those of mainland Korea especially in case of hookworm and T. orientalis. The proportion of unfertilized ova passers among the Ascaris infected case was 22.1%. The ova of heterophyids, most probably Metagonimus yokogawai were detected in lower percentage but discovered throughout the localities surveyed and in all age groups. It is certainly presumed that Metagonimus infection is autochthonous. The most interesting results were obtained in Taenia sp. infection and the higher rates were shown in rural area than in urbanized areas. The positive rates were within 5% in childhood and adolescence but abruptly increased up to 36.4 % in adults.

7.Metacercariae of Echinochasmus japonicus encysted in a fresh water fish, Pseudorasbora parva, and their development in experimental mice.

The echinostomatid metacercariae encysted in the gill of the fresh water fish, Pseudorasbora parva were identified through obtaining adult worms after experimental infection to mice. In addition, a brief course of worm development and maturation was observed in this experimental host. The results were as follows: The echinostomatid metacercariae were elliptical, golden yellow, 0.073-0.078 mm long and 0.054-0.065 mm wide. Their head portions were characterized by the presence of a head crown armed with collar spines of total 24 in number and interrupted at the mid-dorsal side of the oral sucker. The average rate of worm recovery from 12 mice (on the 1-21th postinfection days) was 19.4 percent and the rate revealed no decrease in accordance with the increase of infection duration. The worms were collected chiefly from the lower part of the small intestine. After the infection, their sexual maturation was attained in 5 days and their growth in size nearly completed in 7 days. The early growth curve of genital organs was S shape while that of nongenital organs was C form. In 5 day old worms, l or 2 eggs were found from their uteri and the stools of mice revealed echinostomatid eggs from the 5-6th postinfection day. The 7 day old adult worms were ovoid in shape, 0.54-0.69 mm long and 0.29-0.34 mm wide, and characterized by a well developed head crown with 24 collar spines and vitelline follicles distributed from the acetabular level down to the posterior end of body. Based on these characters they were identified to be Echinochasmus japonicus Tanabe, 1926. From these results, it is verified that P. parva is one of the second intermediate hosts of E. japonicus in Korea.

9.Tissue Reaction and Morphologic Change of Gore-Tex Implant in the Rabbit Model.

Various materials have been proposed for use in aesthetic and reconstructive augmentation of the face. Three basic types of material are currently available: autografts, homografts, and alloplasts. Alloplastic materials have been used widely for soft tissue augmentation. Many synthetic material have been developed for alloplasty; among these, the porous material, Gore-Tex(r)(ePTFE) allows minimal fibrous tissue ingrowth but the amount is sufficient to confer some stability of the implant in soft tissue over time. The purpose of this study is to evaluate the tissue reaction and size change of Gore-Tex in rabbits. A precise pockets were made in the plane below the panniculus carnosus and periosteum. The 25x5x3mm sized rectangular shape of Gore-Tex(r) was then inserted into the pocket. Tissue specimens including skin and implant were obtained in 1, 3, 6 months after implantation. The specimens were examined grossly and microscopically. Grossly, none of the implants were lost or extruded and there was no evidence of wound infection, hematoma, or seroma formation. Thickness of implants under periosteum decreased more than panniculus carnosus. Results of light microscopy revealed a mild inflammatory cell reaction in the early implant group(1 month) at the tissue-implant interface. Within the substance of the material, tissue ingrowth was not observed in the early implant group(1 month and 3 months), but was observed in the late implant group(6 months). In this study the Gore-Tex appears to be a safe and reliable substance for use in augmentation of bony and soft tissue structure of the face.

10.The Study on Reliability and Validity of Korean Version of the Barratt Impulsiveness Scale-11-Revised in Nonclinical Adult Subjects.

OBJECTIVES: This study was conducted in order to determine the validity and reliability of the Korean version of the Barratt Impulsiveness Scale-11-Revised (K-BIS-11-R) in Nonclinical Adult Subjects. METHODS: The K-BIS-11-R was tested in non-clinical (n=270) native Korean-speakers. Internal consistency reliability was calculated using Cronbach's alpha and test-retest reliability was measured using Pearson correlations. To see external validity, we performed confirmatory factor analysis for determination of whether the three-factor model, proposed by the previous report, fit the data. RESULTS: According to the results, the K-BIS-11-R had an acceptable test-retest reliability and internal consistency reliability. In addition, the K-BIS-11-R total score and second-order subscale score showed significant correlation with Conners' Adult attention deficit/hyperactivity disorder Rating Scale-Korean and Buss & Durkee Hostility Inventory. In addition, no significant difference was observed between the K-BIS-11-R total score and demographic variables. Thus, the K-BIS-11-R total score was transformed to standard score. CONCLUSION: K-BIS-11-R is a reliable and valid measure and has possible utility for assessment of impulsiveness in nonclinical adult subjects. Further research is needed in order to determine the cutoff score for administration of K-BIS-11-R to clinical adult subjects.
