1.A Clinical Study of Colorectal Cancer.

This is a retrospective clinical analysis of 156 patient with colorectal cancer who were surgically treated from January 1988 to June 1996 at the Department of Surgery, Seoul Adventist Hospital. The results are as follows: 1) The peak age incidence was in the 7th decade (31.4% of the cases), and the sex ratio of males to females was 1.03 : 1. 2) The most common location of the tumor was the rectum in 77 cases (49.4%); next were the sigmoid colon in 25 cases (16.0%) and the ascending colon in 25 cases (16.0%). 3) In the right colon, the most frequent symptoms and signs were abdominal pain, a palpable mass, weight loss; in the left colon and rectum, bloody tarry stool and bowel- habit change were the most common symptoms and signs. 4) The duration of the symptoms and signs prior to admission was most commonly less than 3 month (46.8% of the cases). 5) The diagnostic methods were digital rectal examination, sigmoidoscopy, colonofiberoscopy, barium enema, and abdominal CT. In two cases,an exploratory laparotomy was done. Also, 2.8 studies were done per patient. 6) The operations performed included an abdominoperineal resection in 36 cases (24.0%) and a right hemicolectomy (18.7%). The operability was 96.2%, and the total resectability was 79.5%. 7) The staging of the tumor was performed during the initial operation according to the Aster Coller classification and the TNM classification. Stages C2 (33.8%) and B2 (29.1%) and T3N0M0 were the most frequent stages in both classification. 8) The most common histologic type was an adenocarcinoma (96.8%). 9) The most common macroscopic finding was of the annular type (59.6%) 10) The most common distant metastasis sites were the pelvic organs and the liver. 11) The most frequent postoperative complication was wound infection (14.7%). The complication rate and perioperative mortality were 32.7% and 2%, respectively.

2.Usefulness of Serum Mast Cell Tryptase Analysis in Postmortem Diagnosis of Anaphylactic Shock.

Anaphylactic deaths are frequently observed at autopsies. Since death associated with medical practice has become social concern, the identification of anaphylactic shock is an important part of forensic medicine. However, autopsy findings of anaphylactic shock are usually non-specific; therefore, the diagnosis of anaphylactic shock must be inferred from collecting data on the past history of the deceased, circumstances of death, and negative autopsy findings. The analysis of serum mast cell tryptase level is a well-known, useful ancillary test for the diagnosis of anaphylactic shock, but is not widely used in daily practice in Korea. We recently encountered 2 autopsy cases of anaphylactic shock and confirmed that analysis of serum mast cell tryptase level was useful for the diagnosis of anaphylactic shock. In this report, we present these 2 autopsy cases of anaphylactic shock, with literature review of the usefulness and limitations of serum mast cell tryptase analysis.

3.Death Year Estimation of Skeletal Remains by Radiocarbon Dating of Femur

The identification for skeletal remains is one of roles of forensic medicine. For this purpose, dating, i.e., estimating the birth year and death year is expected as useful, however has not yet been practically applied. A dating method using radiocarbon analysis was recently introduced and related studies have been reported. In this study, we tried to confirm the applicability of radiocarbon dating for the identification of skeletal remains and to develop formulas to estimate the death year. Thirty-four autopsy cases from the National Forensic Service, from December 2014 to July 2022, with known death year were selected for inclusion. For each case, two samples were taken: the spongy bone of the femur head, and the compact bone of the femur midshaft. For each sample, radiocarbon analysis was carried out and the corresponding femur year were calculated using the bomb peak curve. The differences between the femur year and the death year were determined and analyzed on the influence of variables. A formula for estimating the death year was developed and the applicability of the formula was determined. The results showed that the difference between death year and femur head year was 14.2 years on average. In male, the difference between death year and femur head year increased with age, however, it did not show any difference according to age in female. The estimation formula of death year was as follows: (In male) Death year=0.993×(Femur head year)+0.288×(Age)+15.061, (In female) Death year=0.769×(Femur head year)-0.218×(Age)+489.676. The formula for male had relatively high explanatory power (adjusted R2=0.710), however, the formula for female had low explanatory power (adjusted R2=0.588). This study is meaningful because it is the largest single study of its kind, to date, and uses specific and identical skeleton (femur head/femur midshaft) to increase the accuracy of the death year estimation. We expect that the results of this study will be supplemented through additional research in the future.
4.Birth Year Estimation of Skeletal Remains by Radiocarbon Dating for Teeth

In this study, we conducted radiocarbon analysis on teeth and aimed to develop a formula to estimate the birth year. Fifteen autopsy cases from the National Forensic Service, from December 2014 to December 2020, with known birth year were selected for inclusion. For each case, dentin of the first molar in mandible was taken, radiocarbon analysis was carried out and the corresponding estimated birth year were calculated using the bomb peak curve. The differences between the birth year and the teeth year were determined and analyzed on the influence of variables. A formula for estimating the birth year was developed and the applicability of the formula was determined. The difference between the birth year and the teeth year was 2.6 years on average for cases born before 1963, and 5.7 years for those born after 1963. The estimation formula of birth year was as follows: (Before 1963) Birth year=0.565×(Tooth year)–0.446×(Age)+875.001, (After 1963) Birth year=Tooth year–5.7. This study is meaningful in that it reduced the error by using only the first molars of the lower jaw as a sample, and presented an estimation formula of birth year that can be applied in practice through radiocarbon analysis of teeth.
5.Mammographic and Clinical Correlation of Axillary Lymph Nodes.

BACKGROUND: The purpose of this study was to determine the cause of the axillary abnormalities seen in mammography and to evaluate the imaging characteristics of benign lymphadenopathy and lymphadenopathy associated with malignancy. METHODS: One hundred ninety-three (193) abnormalities seen mammographically were retrospectively reviewed and correlated with clinical diagnoses and pathologic results found in the medical records of patients treated from August 1997 to July 1998 at the National Medical Center. For each abnormality, the length, the margins and the presence of microcalcifications were noted. RESULTS: One hundred seventy-eight (178) patients had benigh lymphadenopathy and twelve patients had metastatic breast cancer. The mean lengths of lymph nodes in benign lymphadenopathy & metastatic breast cancer were 13.7 mm and 22 mm, respectively (p=0.0002). Fatty infiltrations of benign lymphadenopathy were seen 157 patients (88.2%) while eleven patients with metastatic breast cancers had nonfatty infiltration and one patient had fatty infiltration. Among the patient with benign lymphadenopathy circumscribed margins were observed in 167 patients (93.8%), microlobulated margins in 7 patients (3.9%), and obscured margins in 4 patients (2.2%), while among the patients with metastatic breast cancer circumscribed margins were seen in 6 patients, obscured margins in 3 patients, spiculated margins in 2 patients, and a microlobulated margin in 1 patient. No microcalcifications were found in lymph nodes. CONCLUSIONS: For benign lymphadenopathy the size of the abnormality was less than 13.7 mm and it had circumscribed margin and fatty center. On the other hand; homogenously dense (nonfatty) axillary lymph nodes were strongly associated with malignancy when axillary lymph nodes were longer than 22 mm with ill-defined or spiculated margins; therefore, a biopsy should be done to confirm malignancy in such cases.

6.The Statistical Analysis on Legal Autopsy Performed in Korea during 2012 Year.

This statistical analysis of 4,709 legal autopsies performed in Korea in 2012 was conducted to obtain primary data about and related variables. The analysis revealed the following: 1. Of the total number of deaths, men accounted for and women, 26.8%. Evidently, the number of deaths among men was more than twice that among women. 2. With respect to mode of death, 54.8% were recorded as unnatural deaths, 39.2% were natural deaths, and 6.0% had unknown causes. Of the 2,581 unnatural deaths, 44.8% were accidental deaths; 27.2%, suicidal; 17.5%, homicidal; and 10.3%, undetermined. 3. Of the total number of unnatural deaths, 43.0% were trauma-related deaths, for which falling down was the leading cause, accounting for 33.5% cases. Asphyxiation was accounted for 17.2%, among which the predominant cause was hanging (55.3%). Moreover, 12.5% of deaths were due to drowning; 11.5%, poisoning; 11.2%, thermal injuries; 2.7%, complications in medical procedures; and 0.3%, electrocution, starvation, or neglect. 4. Among 1,847 natural deaths, heart diseases accounted for 50.2% and vascular diseases accounted for 16.7%. 5. There were 207 cases of deaths among children under the age of 10, of which 94 were unnatural and 49 were homicidal deaths. Of all cases, 9.2% had an unknown cause of death, and of these 67.6%, were putrefied or skeletonized bodies.

7.Comparison of Conventional Spin Echo, Fast Spin Echo and Fast Multiplanar Spoiled Gradient Recalled Imaging for Evaluation of Focal Hepatic Lesions.

PURPOSE: To compare the detectability and conspicuity of focal hepatic lesions among conventional spin echo techniqueniques and fast scanning techniqueniques in MR imaging. MATERIALS AND METHOD: 36 patients (24 men and 12 women, mean age of 54 years) with suspected liver mass on CT scan and US underwent MR imaging at 1.5 T system. Conventional spin echo T1, T2 weighted imaging, T2 weighted fast spin echo, T1 weighted fast multiplanar spoiled gradient recalled(FMPSPGR) imaging and Gd-enhanced FMPSPGR(Gd-FMPSPGR) were performed. 16 cases of hepatocellular carcinoma, 13 cases of hemangioma, 3 cases of metastasis, 3 cases of cholangiocarcinoma, and 1 case of lymphoma were included. Lesion conspicuity and number of detected hepatic masses were compared among the MR sequences. RESULTS: Overall 63 hepatic lesions were detected in 36 patients. Lesion detection' was 63(100%) with Gd-FMPSPGR, 62(98%) with non-enhanced FMPSPGR, 58(92%) with SE T2WI, 56(89%) with SE T1WI and 54 (86%)with FSE. For solid tumors, the contrast to noise ratio was greatest on Gd-FMPSPGR which was significantly higher than any other sequences. For hemangiomas it was greatest on SE T2WI and was significantly higher than SE T1WI. In the qualitative assessment, Gd-FMPSPGR was preferred for both solid mass lesion and hemangioma. FSE was inferior to SE T2WI for lesion detection and conspicuity without statistical significance. CONCLUSIONS: For the evaluation of focal hepatic lesions, breath-hold FMPSPGR with and without contrast enhancement may replace the conventional SE T1WI. Because FSE was inferior to SE T2WI when the later is used with respiratory compensation in some cases, it is difficult to replace the SE T2WI with FSE. But FSE, FMPSPGR and Gd-FMPSPGR images, when combined, may replace SE T1WI and SE T2WI.

8.An Innovative Scalp-Dyeing Technique with Gentian Violet Solution During Follicular Unit Extraction for White-Haired Follicular Units.

There exist some restrictions and difficulties in performing follicular unit extraction (FUE) in white-haired patients, for several reasons. In this paper, we introduce a novel technique for visualizing white hair during the punching procedure and graft preparation in FUE for white-haired patients. In white-haired older male patients, we dyed the surrounding scalp skin purple with a gentian violet solution-stained toothpick. Our method has several advantages: surgeons can easily focus on the center of the follicular unit and rapidly perform punching, they can recognize the condition of the harvested follicular units during FUE, and the hair transplant team can secure a clear view for trimming and loading into the implanter. We suggest that scalp dyeing in difficult FUE procedures, especially in patients with white hair, may be a simple method that provides a good visualization for donor site harvesting and for microdissection.

9.Very late stent thrombosis of DES implanted patient during noncardiac surgery: A case report.

Very late stent thrombosis after implantation of drug eluting stent is rare, but its consequences are potentially fatal. Stent thrombosis may be occurred in perioperative period because of interruption of anticoagulation therapy and intraoperative hypercoagulability. We report a case of very late stent thrombosis in a 49-year-old male patient during total gastrectomy. ST-segment elevation in lead II occurred during the surgery and followed by cardiac arrest. After external cardiac massage and electrocardioversion, normal sinus rhythm was restored. Postoperative 12 lead ECG showed ST-segment elevation in leads II, III, aVF and serum cardiac enzymes such as CPK, CK-MB, and Troponin T were markedly elevated. Postoperative coronary angiography showed complete occlusion of the right coronary artery stent. Emergency percutaneous transluminal coronary angioplasty was performed and the patient recovered uneventfully.

10.The Statistical Analysis on Legal Autopsy Performed in Korea during 2013 Year.

Medicolegal autopsy is a vital tool for obtaining reliable injury mortality data. This study statistically analyzed data obtained from medicolegal autopsies performed in Korea in 2013. The aim of this study was to analyze various aspects of the 4,861 deaths that were categorized as unusual in Korea in 2013. A total of 4,861 deaths were analyzed by gender, age, manner of death, and cause of death. Of the 4,861 deaths, 3,542 (73.3%) were of men and 1,302 (26.7%) were of women. With respect to the manner of death, 54.4% were recorded as unnatural deaths, 38.8% were natural deaths, and 6.9% had unknown causes. Of the 2,642 unnatural deaths, 45.0% were determined to be accidental deaths, 26.3% suicidal, 16.9% homicidal, and 11.8% undetermined. Of the total number of unnatural deaths, 42.1% were trauma-related deaths, for which falling down accounted for 33.8%. Asphyxiation accounted for 16.0% of unnatural deaths, and of these, the predominant cause was hanging (58.8%). In addition, 14.4% of deaths were due to drowning, 12.9% poisoning, 11.0% thermal injuries, 1.8% complications arising from medical procedures, and 0.8% electrocution, starvation, or neglect. Among the 1,886 natural deaths, heart diseases accounted for 52.0% and vascular diseases accounted for 16.9%. Of the 196 deaths among children under the age of 10 years, 41.8% were recorded as unnatural deaths, 45.1% were natural deaths, and 1.5% had unknown causes.
