Spinal meningiomas located purely in the extradural space are rare, and they may easily be confused with malignant neoplasm. We report an unusual case of a purely extradural spinal meningioma mimcking metestatic neoplasm. A 38-year-old woman had neck pain and left side weakness. MRI scan revealed extradural spinal mass. Preoperative and intraoperative diagnosis was metastatic carcinoma, but permanent diagnosis was extradural meningioma.

The authors have described a case with pure red cell aplasia (P.R.C.A.) caused by the administration of diphenylhydantoin after head injury. P.R.C.A. associated with diphenylhydantoin is very rare and easily treated with a discontinuation of diphenylhydantoin & steroids.

4.Clinical and Computed Tomography Evaluation of Plate and Screw on the Cervical Lateral Mass: A Modified Magerl's Technique.

OBJECTIVE: To determine the clinical and radiological safety of 15 consecutive patients managed with plate and screw fixation systems applied to the cervical lateral mass. METHODS: 15 patients who underwent posterior cervical and T1 arthrodesis were reviewed from Jan 2002 to Dec 2004. Posterior cervical screw and plate fixation was applied on the lateral mass of the cervical spine. The authors have tried lateral mass screw fixation using a modified Magerl's technique (20 degrees lateral and 20~30 degrees rostral screw trajectory) under preliminary radiological study. The average patient age was 39.73 degrees+/-11.00 years, and the average follow-up period was 9.73 degrees+/-6.77 months. Computed tomography scans taken after surgery were reviewed to confirm the attempted screw trajectory correct and safety. RESULTS: Three of 93 lateral mass screws were malpositioned but clinical damage was not noted. Two of 8 pedicle screws on the T1 vertebrae were not placed on the correct pedicle area. Screw and plate loosening was observed in one case but was not subjected to an additional procedure because of maintained screw position observed during follow-up periods. CONCLUSION: The results of this study indicate that lateral mass screw fixation using the Modified Magerl's technique on the cervical lateral mass may provide safe and effective application on the patients. In addition, the chance of incorrectly placed screws was higher in T1 pedicle screw fixation than in lateral mass screw fixation of the cervical area.

5.Scoliosis in a Nineteen Years Old Male: Prevalence Study.

OBJECTIVE: To evaluate the prevalence and the epidemiological findings of scoliosis in 19-year-old Korean males. METHODS: We applied chest radiographs for scoliosis screening from the examinations for conscription at Regional Military Manpower Administration in Seoul, Korea, from April 2009 to December 2009. A total 28,779 males, aged 19 years, were screened for scoliosis. Additional thoraco-lumbar radiographs were obtained for every examinee identified with scoliosis of more than 20 degrees using Cobb's angle. RESULTS: The prevalence of scoliosis of more than 10 degrees of Cobb's angle was 5.3%(1532 of 28,779 males). The most common category for these 19-year-old males was scoliosis of 10 to 19 degrees. The most common pattern of scoliosis was thoracic (57.6%), followed by thoracolumbar (32.1%), lumbar (7.0%), and double major curves (3.3%) scoliosis. CONCLUSIONS: Among 28,779 19-year-old Korean males, the prevalence of scoliosis of more than 10 degrees Cobb angle was 5.3%, and thoracic scoliosis was the most common.

6.Somatosensory Evoked Potentials in Aneurysmal Subarachnoid Hemorrhage with Delayed Cerebral Ischemic Deterioration.

Somatosensory evoked potentials(SEPs) reflect the functional integrity of the central neural pathways, and as such may be used to assess function that remains during variety of cerebral insults. To evaluate the utility of SEPs during cerebral ischemic deterioration due to aneurysmal subarachnoid hemorrhage, SEPs were measured in 43 patients with subarachnoid hemorrhage, among them 24 patients had no cerebral ischemic deterioration(group 1) and 19 patients had cerebral ischemic deterioration(group 2). In group 1, central conduction time(CCT) were measured on the day of admission(CCT1), the seventh day(CCT2), and the twenty-first day(CCT3) after aneurysmal rupture. In group 2, CCT were measured on the day of admission(CCT1), during(CCT2) and after resolution(CCT3) of cerebral ischemic deterioration. CCT2 of the both cerebral hemispheres were different significantly between two group(p=0.01). In group 2, there was stastically significantly difference between CCT1 and CCT2(p<0.01), and significant correlation between clinical grade and CCT was noted, especially in grade III, and IV. Therefore it is suggested that the central latencies of the SEPs are seemed to be a sensitive tools as indicators of the onset and extent of a cerebral ischemic deterioration due to aneurysmal subarachnoid hemorrhage.

7.The Outcome of Multiple Slit on Plaque with Plication Technique for the Treatment of Peyronie's Disease.

PURPOSE: To evaluate the postoperative outcome of the multiple slit on plaque plication technique for the treatment of Peyronie's disease. MATERIALS AND METHODS: We retrospectively evaluated 22 patients who underwent plaque incision with penile plication for the surgical treatment of Peyronie's disease, who had failed medical treatment between 2009 and 2014. Patients were grouped by preoperative degree of penile curvature into Group I: mild (n=5, 22.7%), Group II: moderate (n=11, 50.0%), and Group III: severe (n=6, 27.3%). After a thorough review of the medical records, we evaluated (a) the correction of the curvature; (b) sexual function; and (c) any penile shortening or other complications. RESULTS: The mean postoperative follow-up period was 39 months. Complete correction of the curvature was attained in 21 patients (95.5%). As an inevitable complication, minimal penile shortening (<1.5 cm) was reported by 14 patients (82.4%) but did not adversely affect sexual intercourse (0%), and all patients found the extent of penile shortening to be acceptable. Nineteen patients had good erectile function (International Index of Erectile Function >21). The most frequent complication was subcutaneous penile edema in three patients (13.6%), which was resolved within about 3 months following surgery. CONCLUSIONS: As a modified technique, multiple slit on plaque with plication is a simple, minimally-invasive and effective technique for correcting penile curvature regardless of curvature severity. The degree of penile curvature does not significantly predict the amount of penile length loss.

8.Changes of Antioxidant Capacity in Colorectal Cancer Patients.

PURPOSE: Free radicals are defined as any species capable of independent existence that contains one or more unpaired electrons and they have the effects on carcinogenesis and tumor progression by causing mutations on genetic structure or suppressing repair of mutated DNA. This study was aimed to identify changes of antioxidant capacity in carcinogenesis and tumor progression of colorectal cancer (CRC). METHODS: We studied the level of serum antioxidant capacity using ABTS technique for 34 CRC patients who were operated between July 1997 and January 1998 at department of surgery, Taegu Catholic Medical Center and compared with 38 persons who had normal value of liver function during the same period. RESULTS: CRC patients showed decreased serum antioxidant capacity level compared to that of control group (CRC male 1.23 0.15 mmol/L, CRC female 1.11 0.13 mmol/L, control male 1.40 0.15, control female 1.35 0.11 mmol/L). Changes of antioxidant capacity levels were not correlated with stages, even though the marginal difference between T-stages (T1/2 1.23 0.10 mmol/L, T3/4 1.16 0.15 mmol/L). CONCLUSIONS: Free radicals may be the causative agent of colorectal carcinogenesis and can be associated with early phase of carcinogenesis rather than tumor progression.

9.Differential Diagnosis between Malignant and Benign Breast Diseases Using Localized Proton Magnetic Resonance.

Proton magnetic rcsonance spectroscopy (1H MRS) has demontrated its abilities to detect an increase of choline containing compounds (Cho) in various brain tumors and prostatic cancer tissues. Based on preclinical works done by other authors using multinuclei MRS, it is reasonable to assume malignant breast tumors will have elevated level of Cho compared to that of normal tissues and benign breast lesions. Several challenges must be met to obtain clinically useful 1H breast spectrum. Good water and fat suppression, Bo homogeneity are required to detect low level metabolic signals like choline if any. In this study, we investigated the clinical utility of 1H MRS with simultaneous suppression of water and fat signals, using breast imaging surface coil for evaluating breast cancer with small lesions. All studies were performed using a GE signa MRI unit (1.5 T, Ver 5.5) and 2-channel breast coil (GE). Water suppression was achieved by chemical selective saturation, and fat signal was attenuated using inversion recovery sequence. Spectroscopic data were acquired with PRESS sequence. Twenty-three patients, age 14-75, were examined. Eleven of these patients presented with invasive ductal carcinoma. The remaining patients presented with benign processes including fibroadenoma, fibrocystic change, galactocele, adenosis, ductal ectasia and dystrophic calcification. The size of lesions were variable (8-90 mm in diameter). Choline at 3.25 ppm was visible in the spectra of all cancer patients, while invisible in the spectra of all benign lesions except a lesion of dystrophic calcification. We concluded that in vivo detection of choline containing compounds in breast carcinomas using proton magnetic resonance spectroscopy demonstrated its potential as a noninvasive tool for differential diagnosis of malignant and benign breast lesions larger than 7 mm in diameter.

10.The Effect of Photodynamic Therapy on Colon Cancer Cell Line.

PURPOSE: Photodynamic therapy (PDT) is a relatively new technology for an alternative modality in the treatment of colorectal cancers. This study was conducted to identify the appropriate dosage and energy level for the photosensitizer as well as effect of PDT on colon cancer cells. METHODS: Colon cancer cell line, COLO 205 (American Type Culture Collection, ATCC) was obtained from Korea Cell Line Bank (KCLB, Seoul, Korea). Cells were cultured on RPMI 1640 medium with 10% fetal calf serum, penicillin, and gentamicin. Cells were incubated at 37 C in a 5% CO2 air environment. Photosense (sulphonated aluminum phthalocyanine, AlPcS4, NIOPIK, State Research Center, Moscow, Russia) was used for the photosensitizer and Fireplace video-3 (Biospec, General Physics Institute, Moscow, Russia, 500 mW/cm2, 670 nm) was used for the light source. 1 104 cells were incubated in 96 well plates with different concentrations of aluminum phthalocyanine: 0.1, 0.3, 0.5 and 1micrometer for 24 hours then photoirradiation was performed at either 24 or 48 J/cm2. The time variations of the viabilities of cells of the four study groups and were measured by using MTT assay according to time were compared to those of the three control groups: control (no treatment), control (AlPcS4, no light), control (light, no AlPcS4), and the study groups (PDT) at one hour, 24 hours, 48 hours and 72 hours after PDT. RESULTS: At, one hour after PDT, the viability of the cells was not changed in the control groups. Viabilities of 117, 40, 35, and 23% in the 24 J group and 76, 31, 52, and 48% in 48 J group were observed, respectively in order of increasing concentration with the value of 87~103% for the control group. 24 hours later, viability of control groups were not changed, By 24 hour after PDT, the viabilities of the control groups had not changed, but those of the 24 J/cm2 and 48 J/cm2 PDT study groups had decreased significantly to 62, 17, 16, and 18% and 24, 15, 13, and 13%, respectively (P=0.00). By 48 hours viability of the 24 J/cm2 and 48 J/cm2 PDT study groups were also significantly decreased being 103, 26, 13, and 13% and 50, 8, 8, and 9%, respectively (P=0.00). By 72 hours, viabilities were 84, 21, 21, and 30 % and 33, 20, 33, and 15%, respectively (P=0.00). CONCLUSIONS: The PDT groups showed a marked cytotoxic effect compared to the control groups, and the effect appeared just after PDT and peaked in 48 hours. The minimum required concentration of the photosensitizer for effective cytotoxicity was at 0.3 micrometer either 24 or 48 J/cm2.
