3.Experience of Posterior Urethral Valves with Vesicoureteral Reflux.

A retrospective review was undertaken of 5 pediatric cases of posterior urethral valve with vesicoureteral reflux. The clinical data were summarized as follows : 1. At the time of diagnosis, patient age was recorded as 2 months, 3, 4, 13, and 14 years. Three cases presented with high fever, one with urinary dribbling and one with acute epididymitis. 2. Voiding cystourethrogram was abnormal in all cases, showing thickened, trabeculated bladder wall, dilatation of posterior urethra and vesicoureteral reflux. Vesicoureteral reflux was bilateral in 3 cases, unilateral in 2 cases and its degree was grade R-V/V in 8 renal units, grade E/V in one renal unit. 3. Initially transurethral valve ablation was performed in all cases, of which 2 had concomitant antireflux surgery and 3 had delayed ureteral reconstruction between one and 11 months after valve ablation. 4. Follow up duration was between 13 and 53months. In 4 cases, reflux disappeared and the pyelographic appearance markedly improved as regards pelvocalyceal and ureteral dilatation with improvement of general status at followup. But, in one case that had demonstrated bilateral high grade vesicoureteral reflux with sever ureteral dilatation and poor renal function, voiding symptom and radiologic appearance improved but left vesicoureteral reflux persisted by 13 months after reconstructive surgery.

4.Studies on the histopathological changes in the human testis after vasectomy.

To determine the effect of vasectomy on the human testis, author compared testicular biopsy specimens from 27 healthy men undergoing vasectomy reversal with those from 21 healthy fertile volunteers. The data of the control group, those normal persons, were obtained from the other study. The qualitative and morphometric quantitative analyses were performed for the data obtained. The average age of the post-vasectomy group was 35.3+/-4.6 years old (29-48 years old). The mean duration of vasal obstruction was 48+/-34 months (3-144 months). The mean volume of testis was 19.0+/-3.3ml (12-25ml) for the right side and 18.7+/-3.5ml (15-25ml) for the left side at the time of vasovasostomy. Through the observation of the testicular biopsy, the decreased spermatogenesis was revealed in all 27 cases. Atrophic seminiferous tubules were observed in 24 cases (89.4%) and focal interstitial fibrosis was revealed in 7 cases (25.9%). seventeen cases (63.0%) showed Leydig cell hyperplasia. Maturation arrest and germ cell aplasia were not observed. Morphometric quantitative analyses revealed a significant increase in the mean thickness of seminiferous tubular walls to 16.0+/-2.1um (10.7-19.4um). Further, there were significant increases in the mean diameter of cross sectional seminiferous tubule to 223+/-19.0um (183-258um) and in the mean area to 37,300+/-6,200um2 (27,800-48,200um2). The mean numbers of mature spermatids and Sertoli cells were reduced significantly to 2418 (10-40) and 14+/-3 (8-22) respectively per tubular cross section. Person correlation were analyzed to find out whether age and duration of vasal obstruction had statistically significant correlations with each variables used to show the changes of testicular tissue in this study. While the tubular wall thickness. the cross sectional tubular area and the number of mature spermatids and Sertoli cells were not significantly correlated with the age and the duration of vasal obstruction (p>0.01), seminiferous tubular diameter showed significantly negative correlations with the age and the duration of vasal obstruction (p<0.01).The tubular diameter decreased as the age and the duration of vasal obstruction increased. It can be concluded that there would be significant histologic changes in the human testis after vasectomy.

5.The Analysis of Methylenetetrahydrofolate Reductase Mutation in Recurrent Spontaneous Abortion Associated with Hyperhomocysteinemia.

OBJECTIVE : To analyze the methylenetetrahydrofolate reductase (MTHFR) mutation in recurrent spontaneous abortion associated with hyperhomocysteinemia. MATERIAL AND METHOD: The blood Sample of habitual aborter with high fasting homocysteine level was tested by PCR-RFLP method. RESULTS: The patient was found to be a homozygosity for MTHFR gene mutation that was confirmed by the finding which is consistent with the mutation at the nucleotide 677 C to T, Corresponding to Ala to Val. CONCLUSIONS: Hyperhomocysteinemia due to MTHFR mutation is a cause of recurrent spontaneous abortion. Therefore, the MTHFR mutation should be examined in the workup of recurrent spontaneous abortion showing hyperhomocysteinemia.

6.Pseudoaneurysm of the Facial Artery after the Injection of Botox.

Facial artery pseudoaneurysm is a rare lesion that can be mistaken for an epidermal cyst or other lesions. We present here the case of an iatrogenic pseudoaneurysm of the facial artery branch that was caused by Botox injection. Botox is frequently used for cosmetic and therapeutic purposes in any part of the body. We treated this lesion by performing aneurysmectomy, and the histopathologic analysis showed myxoid degeneration.

7.A Case of pustulosis Palmaris et Plantaris.

A case of 42 years old male with pustulosis palmaris et plantaris was reported. The skin lesions were characterized by several vesicles, pustule, scaly crusts with a erythematous infiltrated base on the both palms and lateral side of the fingers. The physical examination showed dental caries on the left second molar teeth. The pus culture on the Sabouraud's media and the blood agar showed no growing organism. The gram stain of the lakes of pustules reveald many WBC but no organism. He was treated with oxytetracycline, DDS and wet dressing with potassium permanganate solution for 2 weeks and favorable effect was noted.

8.A prediction of bony interference between proximal and distal segment of the mandible with integrated 3d solid model and dental cast in orthognathic surgery.

Three-dimensional solid model has not been widely used in surgical prediction of orthognathic surgery because freque from occlusal restorations or prosthesis limited the usefulness of simulated surgery involving occlusion. We prepared three-dimensional(3D) solid model from CT data and integrated the 3D solid model with dental cast using a face-bow transfer technique combined with skeletal reference measurement and confirmation with cephalometric radiographs. With this simple and easy method, it was possible to predict bony interference between the proximal and distal segment of the mandible so that we can prevent condylar displacement after sagittal split ramus osteotomy of the mandible with prominent asymmetry. The method error was within 2mm and it seemed to be useful in preoperative planning for maxillofacial surgery with maxillo-mandibular occlusal change

9.CT Arthrographic Finding in Shoulder Instability.

The purpose of this study is to evaluate the morphologic findings of the labrum and capsuloligamentous structures in the normal and unstable shoulders and to compare the effectiveness of double contrast arthrographic findings which were taken with supine and prone position. We reviewed the computerized tomographic arthrogram in 9 cases of normal shoulder and l3 cases of shoulder instability. We evaluated the shape of the labrum, the attachment of anterior capsule and the amount of capsular laxity. Capsular laxity was compared each others in normal, the unidirectional instability and the multidirectional instability. Among these materials, we simultaneously performed CT arthrogram with supine and prone position and evaluated which is more reliable and accurate. Scapular inclination was ahout 25 degrees in the supine position and ahout 60 degrees in the prone position. Normal glenoid labrum varied in size and shape, so its lack of a classic appearance might not indicate a labral tear. Variations existed in the anterior capsular attachment onto the glenoid. The anterior capsule was used to attach far away from the glenoid rim. but it might not indicate an anterior instability. With the prone position, the labral lesion took more excellent visualization due to the contrast coating on the glenoid labrum in both anterior and posterior. Anterior capsular expansion was seen well with prone position in anterior instability. In summary, for proper interpretation of the instability, awareness of the morphologic variations of the labrum and capsuloligamentous structure is important. CT double contrast arthrography with prone position has made a significant contribution to improving and easing the documentation of variety of anatomic abnormalities. Now we recommend the prone position in CT arthrogram for imaging glenoid labrum pathology in shoulder instability.
