1.An Experimental Study on Comedogenic effect of Various Korean Cosmetocs.

Varioua articles have been reported that extemal contactants were the cause Acne venenata. The daily use of cosmetics are tbe cause of Acne minor in adult wamen. To distinguish this from other type of Acne venenata due to extemal contactants Kligman had coined the term Acne cosmetica, which is a low grade persistant acneiform eruption with occasional papulopustulea in adult women. The authors have tested a wide range of available cosmetics(Total twenty including four kinds of cold cream species, two of cleansing cream species, three of nutrient cream species, six of lotion species, three of foundation make up species and two of foreign cosmetics) using the rabbit ear assay recently popularized by Kligman in order to evaluate comedogenic effects of various Korean ccsmetics. The results a.re as follows. 1. Among these twenty facial cosmetics, We observed no comedogenccicy in two facial cosmetics, mildly comcdogenecity in twelve facial cosmetics, moderately comedo- genecity in four facial cosmetics and severe comedogenecity in two facial cosmetics. 2. Histologic gradings were paralleled the findings of tbe naked eye in up to 65% 3. There were significant differences on comedogenecity between Korean cosmetics and foreign cosmetics which used by Kligman. 4. There were no significant diffrences between cold cream species, cleansing cream species. nutrient, cream species, lotion species and foundation make up species. 5, There were various differences among twenty kinds of every cosmetic specie.

5.EPICANTHOPLASTY USING MODIFIED UCHIDA'S METHOD .

In the majority of East Asian eyelids, the presence of a medial epicanthal fold is one of the unique anatomic characteristics. Roundness of medial canthal area and a narrow, short palpebral fissure are aesthetic consequences of this anatomic features. But, the idea of beauty has changed with time by influence of western culture. Many people are favorable to the wide open eye without epicanthal fold, which make improved facial balance. Double-eyelid operation is the most commonly performed procedure of aesthetic surgery field in Korea. However, many cases of successfully done double-eyelid operation results in only partial improvement by leaving medial epicanthal folds. We can get the improved results by utilizing epicanthopasty that lengthens the palpebral fissure, and thus producing the image of a larger, open eye. Consequently the requirements for epicanthoplasty has increased in modern time. The Uchida's split V-W plasty is one of the most frequently used methods. But, it has disadvantages of completely exposed scar, potential for dog-ear formation, and two stage procedure with some cases of double-eyelid operation. We modified the Uchida's method by moving the position of 2 triangular flaps more laterally and conjoining the incision line of double-eyelid operation with that of epicanthoplasty. The purposes of our modification are not complete elimination but relieving the epicanthal fold, hiding the minimal scar behind the new relieved the new relieved fold and preventing the dog-ear on the transitional zone. From January, 1995 to June, 1999, this method was applied to 93 patients who have medial epicanthal folds and performed with double-eyelid operation concomitantly. Age ranged From 12 to 47 years and the mean age was 23 years. Through the mean 15 months of follow up, the mean widening of the palpebral fissure width was 2.5mm and the mean reduction of the intercanthal distance was 5mm. The modified Uchida's method has advantages of simplicity in procedure, effectiveness in results, minimal scarring with hiding behind the new small fold, and one stage procedure with double-eyelid operation. We think it can be applied generally to almost of all double-eyelid operation except epicanthal fold is marked severe.

6.Psychiatry, Is It Now Okay?-Enlarging the Boundary of Psychiatry in the Neuroscience Era.

The authors, in this paper, addressed a variety of problems and difficulties which Korean psychiatrists should cope with. The surprising development of neurosciences, splitting of neuropsychiatry into neurology and psychiatry, easygoing attitude of psychiatrists, changes in the delivery system of health care and ill-balanced education of psychiary were listed as causes of or contributors to them. Social bias to psychiatry and regulations from outside are also considered as contributors. Psychiatric education, including medical school, residency training, continuing medical education and psychiatric textbooks, need to be changed in order to enlarge the boundary of psychiatry. Reestablishment of identity of psychiatry and psychiatrist is unavoidable, considering far-reaching new knowledge of neuroscience and gradually invisible borderzone between neurology and psychiatry. The other ways worth while to consider are : the expansion of psychiatrists' activities, development of medical behavioral science to a clinical specialty, creation of new psychiatric subspecialties, and additional training of psychiatric residencies in the primary medical care.

7.A Case of Marjllin's Ulcer.

Marjolin is ulcer may be defined briefly as a cancer arising in a bum scar. A 41-year-old female had a Marjolin is ulcer(12x10x3cm) on her right lower leg. This ulcerated tumor has been present for two and half months and the induction period sinnc her bum was 29 years. The pathologic diagn.osis was squamous cell carcinoma, grade 2. Roentgenograms of her right lower leg revealed several soft tissue masses densities which, in part, horizontally connected with the periosteum of the tibia. So she was treated by above-knee amputation with dissection of several, unenlarged inguinal nodes which showed no evidence of metastases. She developed large, single, fixed metastatic node in right inguinal area one month after amputation and dissection was done.

10.Diagnosis of Thoracic Lesions in Children by Fine Needle Aspiration Biopsy.

To elucidate the clinical avaiability of FNAB, we reviewed retrospectively medical records of 30 patients who had infiltrative pulmonary lesion or pulmonary nodule or mediastinal mass, and got percutaneous fine needle aspiration biopsy using Westcott needle, 20~22 gauge, at the department of Pediatrics, Hanyang university hospital from July, 1986 to June, 1993. The obtained results were as follows: 1) There were 30 patients, aged 6 months to 15 years. 2) There were 17 male patients and 13 female patients with sex ratio of 1.3:1 3) Among 30 cases, we discovered 16 cases of infiltrative lesions, 7 cases of pulmonary nodules and 7 cases of mediastinal mass lesions. In mediastinal there were 4 cases of anterior mediastinum and 3 cases of posterior mediastinum. 4) Overall success rate on fine needle aspiration biopsy was 60% (18/30). The success rate was 50% (8/16) in infiltrative lesions, 50% (8/15) in pulmonary nodules and 71% (5/7) in mediastinal mass lesions, respectively. 5) Eight infiltrative lesione were diagnosed as Pneumocystis carinii pneumonia. Five pulmonary nodules were diagnosed as aspergillosis (2 cases), tuberculosis (1 case), mucormycosis (1 case), lung metastasis of metastasis of neuroblastoma (1 case). Five mediastinal mass lesions were diagnosed as teratoma (2 cases), lymphoma (1 case), malignant neurogenic tumor (1 case), ganglioneuroblastoma (1 case). 6) The complications occured in 20% (6 cases) among 30 procedures. All cases were due to pneumothorax: 3 cases spontaneously resolved, and 3 cases needed chest tube insertion. There were no death related with this procedures.
