1.A Clinical Comparison between One-Plane Bilateral Frame and Two-Plane Bilateral Frame of External Fixation in the Treatment of Open Tibial Shaft Fractures

Treatment of open tibial shaft fracture is difficult, especially when there are severe soft tissue damage. Various complications are noted. The aims of treatment for the open fractures are to promote primary wound healing and union of the fractured bone. We have employed external skeletal fixation devices those are Hoffmann apparatus and pin & resin external fixator for the treatment of 34 cases of open tibial shaft fractures from March, 1981 to May, 1984. Each fixation method was divided into two type, one-plane bilateral frame (180' frame) and two-plane bilateral frame(120') Following results were obtained. 1. Open tibial shaft fracture was most commonly occured in age of high activity of male. 2. External fixation was very useful as a primary treatment method in open tibial shaft fracture. 3. The most common complication was pin tract infection. 4. Mean duration of fixation was 12.6 weeks. After removal of extemal fixation, 8 cases of delayed or nonunion were noted. In addition to bone graft, secondary open reduction and internal fixation with plate and screws (7 cases), only bone graft (1 case) were carried out. Good results were obtained. 5. There was no significant difference in bone union time between 180' frame and 120' frame. The complications were more frequently occured in 120' frame than in 180' frame.

2.3 Cases of Melanosis Coli.

Colonic lesions by irritant laxative abuse are devided into melanosis coli, cathartic colon, soap or chemical colitis. Melanosis coli is the brownish or black discoloration of the colon because of the accumulation of lipofuscin pigment in macrophages located in lamina propria and associated with anthraquinone containing laxative abuse. The site of this lesion is more common in cecum and proximal colon, but whole colon can be involved. This lesion is occurred between 4 months and 13 months from initiation of drug medication, and the lesion is benign because the pigments disappear by withdrawning the laxatives. A number of cases were reported in our country, all of the cases were associated with prolonged administration of anthraquinone type laxatives. We presents two cases of melanosis eoli that had administrated aloe to treat the chronic constipation during long terms, and one case of melanosis coli that had not administrated laxatives or a specific drug with chronic constipation. All of three cases were confirmed by colonoscopy.

3.The significance of fundus photographs during health mass screening.

BACKGROUND: Fundus photographs performed during health check-up were reviewed to determine the usefulness in mass screening. METHODS: Subjects consistine of 3017 people who had undergone physical health check up at the Health Clinic, Dong-Eui Medical Center in 1997 and whose fundus photographs of both eyes were taken showing clearly visible posterior pole and optic disc were chosen as subjects. Medical records which included cases of reported close exam and causes, actual cases of close exam and final diagnoses. RESULTS: Of the 3017 people, reported close exam was performed in 665 (22.0%). Among them glaucoma was suspected in 460 retinal hemorrhagic lesion in 63, retinal degenerative lesion in 67 and others in 75. There were 187 actual cases(28.1%) of close exam including glaucoma suspect in 79, retinal hemorrhagic lesion 47, retinal degenerative lesion 34 and others 27. Among 187 actual cases of close exam, 140 people (1.3%) were eventually diagnosed as glaucoma suspect (60), retinal hemorrhagic lesion (38), retinal degenerative lesion (21) and others (21). CONCLUSIONS: Fundus photographs are useful in mass screening, but performing close exam to enhance the usefulness of the fundus photographs is necessary.

4.Morphometric Analysis of the Collagen Changes Induced by Subcutaneous Injection of Interferon-gamma following Partial Unilateral Ureteral Obstruction in the Rat Kidney.

PURPOSE: Interfenon-gamma(IFN-gamma) has been tried to decrease fibrosis in liver cirrhosis in the human and animals. To decrease collagen content in the partially obstructed kidney, we examined collagen changes by IFN-gamma after creating partial ureteral obstruction in the rat. We applied IFN-gammato the partially obstructed kidney and examined collagen contents in the renal cortex by morphometric analysis. MATERIALS AND METHODS: In a total of 50 Sprague Dawley adult male rats(300-350gm), we created partial unilateral ureteral obstruction(PUUO). We divided rats into 5 groups(n=10 in each group): normal control, sham operation group, sham operation with IFN-gamma 4 weeks, PUUO 4 weeks, PUUO with IFN-gamma 4 weeks. IFN-gamma(LG Chemical, Korea) 100,000 IU was subcutaneously injected daily for 4 weeks from operation day 1 in the treated groups. The rats were sacrificed on the expected day and renal cortex was prepared for Masson`s trichrome special staining. With the image analyser, we calculated the relative ratio of collagen to renal cortex area randomly in each kidney. RESULTS: Relative percent of collagen content of the normal control, sham-operation, and sham-operation with IFN-gamma treated groups were 2.42+/-0.53%(n=10), 2.62+/-0.53%(n=10), and 2.22+/-0.65%(n=10), respectively. While relative percent of collagen content of PUUO group, and PUUO with IFN-gamma treated group were 12.25+/-2.41%(n=5) and 7.14+/-3.43%(n=9) respectively. There was significant difference between those of last two groups.(p<0.05, Wilcoxon-rank sum test) CONCLUSIONS: We applied interferon-gamma to the partially obstructed kidney and examined collagen contents in the kidney by morphometric analysis. From the above data, IFN-gammaseems to have a decreasing effect upon collagen content in the partially obstructed rat kidney.

5.Long-term Followup Results of Desmopressin-based Combined Drug Therapy in the Nocturnal Enuretic Children.

PURPOSE: This study was designed to evaluate the effectiveness of desmopressin-based combined drug therapy in nocturnal enuretic children who had been followed at least 6 months. MATERIALS AND METHODS: Thirthy eight patients(5 to 16 years old) with 4 or more wet nights per week were studied. We used desmopressin-based combined drug therapy(imipramine or/and oxybutynin chloride) depending upon associated daytime voiding symptom. Patient response to treatment was categorized as complete(0 to 1 wet night per month), near complete(2 to 4 wet nights per month), intermediate(5 to 7 wet nights per month) and poor(more than 7 wet nights per month). We evaluated the results at the initial 6 week and at the last follow-up visit on medication. RESULTS: After 6 weeks, 13(34.2%) patients were complete responder, 13(34.2%) near complete responder, 7(18.4%) intermediate responder and 5(13.1%) poor responder, respectively. The response rate in this period was 86.8%. At the last follow-up visit on medication, 20(52.6%) patients were classified as complete responder, 13(34.2%) as near complete responder, 4(10.5%) as intermediate responder and 1(2.6%) as poor responder. Therefore, overall response rate was 97.3%. There was no obvious side effect that made discontinue drug therapy. CONCLUSIONS: From the above data, we think that longterm desmopressin-based combined drug therapy is the highly successful and safe treatment method for enuretic children.

6.Results of immediate loading for implant restoration in partially edentulous patients: a 6-month preliminary prospective study using SinusQuick(TM) EB implant system.

STATEMENT OF PROBLEM: Many dental clinicians are concerned about immediate loading of inserted implants. However, there have been few clinical studies surveying the success rates of immediate loading, based on Korean implant systems. PURPOSE: The aim of this study was to evaluate the outcome of immediate functional loading of the implant (SinusQuick(TM) EB, Neobiotech Co., Seoul, Korea) in partially edentulous maxilla or mandible. MATERIAL AND METHODS: Total 15 implants were placed. Within 2 weeks after implant insertion, provisional implant-supported fixed partial dentures were delivered to the patients. Quantitatively, marginal bone loss was measured at the time of immediate loading, after 3-months of continued loading and at the last follow-up. The mean follow-up period was 4.8 months. RESULTS: Mean marginal bone loss from implant surgery to early loading, 3-months follow-up and last follow-up was 0.03 +/- 0.07 mm, 0.16 +/- 0.17 mm and 0.29 +/- 0.19 mm. No implant failed up to 6 months after insertion, resulting in a 100% survival rate. CONCLUSION: Immediate loading exhibited high success rate in partial edentulism for up to 6 months. Well-controlled long term clinical studies with large sample size are necessary to confirm this finding.

7.Ureterocystoplasty: Experiences in Two Cases of Posterior Urethral Valve.

Ureterocystoplasty is a known useful method for the management of selected cases with dilated ureters, high pressure and poorly compliant bladders. It precludes the potential complications of enterocystoplasty. In two cases of posterior urethral valve with urinary incontinence, bilateral dilated ureters, and poorly compliant bladders, we performed ureterocystoplasties using dilated lower end of the ureter after transuretero-ureterostomy and ureteral reimplantation. Both patients were almost continent day and night. The bladder capacities and compliances were improved. There was no deterioration of renal functions. In conclusion, we suggest that the ureterocystoplasty using dilated distal ureter after transureteroureterostomy is an appropriate procedure in selected cases of posterior urethral valve.

9.A Case of Acute Dacryocystitis Diagnosed after Surgical Treatment of Orbital Cellulitis and Orbital Abscess

PURPOSE: We report a case of acute dacryocystitis diagnosed with abscess and rupture of lacrimal sac and fistula to posterior orbit during the operation. CASE SUMMARY: A 71-year-old woman visited our clinic with edema and pain in the eyelid from three days ago. For past four months, there was viscous of the left eye and tears. The patient had severe conjunctival chemosis and hyperemia, compared with the left eyelid edema and redness. Orbital CT scan showed orbital cellulitis, which was followed by systemic antibiotics and steroid therapy. On the 4th day of therapy, orbital abscess formation was observed in orbit MRI and surgical drainage was planned. During surgery, we found rupture of the posterior part of lacrimal sac and fistula to posterior orbit. Pseudomonas aeruginosa was identified in the bacterial cultures, and after the administration of appropriate antibiotics, the disease showed improved progress, and then additional dacryocystorhinostomy was performed. CONCLUSIONS: In our case, acute dacryocystitis rarely spread in orbit, which may lead to delayed diagnosis, orbital cellulitis and abscess, resulting in serious complications of vision threat. So, we think that it is necessary to consider surgical treatment more actively in the stage of chronic dacryocysitis.

10.Role of ultrasound in management of liver abscess

Ultrasonograms of proven liver abscesses in 44 patients were analysed. In ulltrasonography prior to otherdiagnostic approach, 38 patients(86.4%) were accurately diagnosed as liver abscess with correlation of clinicalsymptoms. The outline of abscess is smoothly defined. oval and round in 19 patients, irregularly and ill-definedin 25 patients. Only in 5 patients echogenic capsules are demonstrated. The internal echogenecity is variable;they are predominantly echo-free in 14, predominantly low-echo in 25 and predominantly echogenic, similar to solidtumor in 2 patients . 36 patients who were treated by medication only and combined needle aspiration had follow-up ultrasonography within 3 weeks since initial treatment. 33 patients were ultrasonically and clinically improved.Only one patients died of sepsis after needle aspiration and 2 patients were discharged without any sign ofimprovement.
