2.CYTOTOXICITY OF DENTAL CAST BASE METAL ALLOYS ON HUMAN ORAL KERATINOCYTES.

Although many studies on the cytotoxicity of the dental cast metal alloys and their components have been carried out, the results are rather conflicting because of the different type of cells used and the various experimental procedures taken. Recently, a number of scientists have claimed that it would be preferable to focus on the use of cells from relevant specific location of the human bodies. Consequently, the primary cultured oral keratinocyte derived from oral mucous along with nickel chloride and several of widely used dental cast base metal alloys(two-Ni-Cr alloys and one Co-Cr alloy)in domestic were selected for this study, from which 1) The amounts of released metal ions were determined using atomic absorption spectrometry, 2) The cytotoxicity of nickel chloride and dental cast base metal alloys was evaluated via MTT assay, and finally, 3) The amounts of released metal ions and the cytotoxicity of nickel chloride were correlated with the cytotoxicity of dental cast base metal alloys And, the results were summarized as follows ; 1. Nickel ion from Ni-Cr alloys and Cobalt ion from Co-Cr alloys resulted in maximum releasing rate during first 24 hours, followed by a decrease in releasing rate with time. Chromium ion were found to be minimal in all alloys. 2. In cytotoxic test, with 40muM, 80muM of nickel chloride, there were observed an increase in the relative cell number compared to control samples after 24 hours. With 160muM, there was found to be no difference in the relative cell number with control, except that 48 hour showed a increase in relative cell number. With 320muM, the relative cell number remained constant and decreased after 48 hours, and with 640muM, a continuing decrease in relative cell number was observed throughout test period. 3. The sensitivity of primary cultured oral epithelium to nickel was lower compared to the cells used in other studies. 4. CB-80 Soft and Regalloy showed no cytotoxicity to primary cultured oral epithelium and New crown resulted in a slight cytotoxicity. In conclusion, it was shown that the primary cultured oral keratinocytes could be applied successfully as testing cells in cytotoxicity test. Futhermore, the dental cast base metal alloys used in this study were found to be biocompatible.

6.4 Cases of Steakhouse Syndrome of the Esophagus.

Acute food impaction of the esophagus is not an unusual problem on an emergency ward. It has been termed the Steakhouse syndrome or the Backyard barbecue syndrome, indicating that meet is the obstrueting bolus in the majority of cases. Most esophageal stenoaes are located distally, and this ie also the area where most food impations occur. The obstruction quickly becomes apparent to the patient when further swallowing of food or liquid is impossible. Various obstructive lesions or motility disorders of the esophagus have been implicated as etiological factors in acute food impaction. Common,disorders include reflux esophagitis with stricture, hiatal hernia, abnormal rings and postoperative strictures at anastomotic sites. Malignant stenoses have been very rarely described. Recently we experienced of 4 patients who suffered from acute dysphagia after they had ingestion of food such as beef, pork, chicken, and dog meat, repectively. All of them were diagnased as steakhouse syndrome due to acute food impaction of the esophageal after examinations of esophagogram and endoscopy. They had hiatal hernia, esophageal cancer, reflux esophagitis with stricture, and abnormal ring as underlying diseases, repectively. All of them were treated with endoseopic removal of foreign body by a Dormia basket.

7.A Case of Meningioma in Temporo-occipital Lobe without Dural Attachment in a 14-yer-old Girl: Case Report.

Meningiomas are thought to arise from the arachnoid cap or meningothelial cells. This frequent apposition to the dura mater probably accounts for the usual attachment of the neoplasm to this layer. We report a case of intraparenchymal meningioma without dural attachment in a 14-year-old girl. A survey of the literature reveals that the majority of cases of meningiomas without dural attachment occur either in children or below the tentorium, and most of them located in the ventricle, the cranial vault. Intraparenchymal meningioma without dural attachment, however, are extremely rare. The uncommon locations of meningiomas at sites distant from the duramater is believed to reflect the rare occurrence of arachnoid cap cells in the Virchow-Robin spaces along the cerebral vasculature or in the pial layers distant from the dura mater.

8.Clinical Parameters Associated with Vasospasm in Early-Operated Aneurysmal Subarachnoid Hemorrhage.

Much controversies still exist with respect to the risk factors influencing symptomatic cerebral vasospasm. In our study, the patient group had some unique characteristics, ie.:42.5%(65) of the 153 patients were aged 60 years or older(mean;55.6 years) and consequently the proportion of Hunt-Hess grade 3-5 was higher than grade 1-2(57.5% vs 42.5%). The authors analyzed the relationship between vasospasm and Hunt & Hess grade, Fisher grade, hydrocehalus, age & hypertension among these 153 early operative cases after a 3 year follow up study. The incidence of vasospasm was significantly higher in the Hunt-Hess grade III, IV, V and the Fisher grade III, IV, compared to grade I & II, and also in preoperative hydrocehalus but it was insignificant in old and hypertensive patients. These results suggest initial severe direct insults and complicating preoperative hydrocephalus increase the chance of postoperative symptomatic vasospasm in this older aged, poorer grade study group.

9.Stabilization of Fractured Spine with Cotrel

Twenty-five patients with unstable fracture or fracture-dislocation of the thoracolumbar and lumbar spines were treated with Cotrel-Dubousset instrumentation. Internal fixation was done in two ways; long rodding in seventeen patients and short rodding in eight. Short segment posterolateral or posterior autogenous iliac bone graft was done in all cases. The amount of correction of the collapsed anterior vertebral body height and the local kyphotic angle at the immediate post-operation were similar between the two groups, but loss of correction at last follow-up was more severe in the short rodding group. Instrument failure was also more common in the short rodding group. In conclusion, it was found that short segment stabilization of spine fractures with Cotrel-Dubousset instrument can effectively reduce fracture but can not maintain reduction until fusion. Therefore, long rodding was reconfirmed to be the better method of stabilization of the thoracolumbar and lumbar spines fractures.
