2.Development of B-eNSP (Baptist Electronic Nutritional Screening Program) Using Integrated Medical Information System and Clinical Efficiency.

PURPOSE: The aim of this study was to develop a baptist electronic Nutritional Screening Program (B-eNSP) that could be easily used to identify patients with nutrition at risk or malnutrition on their admission to hospital using an integrated medical information system and to evaluate validity, reliability and efficiency of B-eNSP. METHODS: From June 1 2009 to June 21 2009, we enrolled 496 patients for prospective research. We analyzed age, sex, weight, height, weight loss in the last 3 months, serum albumin, alteration of dietary intake, gastrointestinal symptom, functional capacity, diagnosis and its relationship to nutritional requirements, physical examination and Subjective Global Assessment (SGA). B-eNSP included body mass index (BMI), weight loss in the last 3 months, serum albumin. Each component was scored. Sensitivity and specificity were calculated to evaluate the validity of the B-eNSP. The receiver operating characteristic (ROC) curve was drawn by using B-eNSP to choose a cut-off value that maximizes sensitivity and specificity and Yoden Index. Comparison with SGA and the reliability of the B-eNSP was done using kappa statistics. RESULTS: The maximum Yoden Index was 0.866 and the cut-off value of the ROC curve was 2. A B-eNSP score higher than 2 was defined as nutrition at risk or malnutrition. Reliability of the B-eNSP was in accordance with SGA by kappa 0.845. CONCLUSION: The B-eNSP can be used efficiently to identify patients with nutrition at risk or malnutrition by simply using an integrated medical information system.

3.Early Clinical Outcome and Complications of Tension Free Vaginal Tape Procedure in Stress Incontinent Women.

PURPOSE: The aim of the study was to evaluate the clinical outcome and complications of tension free vaginal tape (TVT) procedure for the surgical treatment of female stress urinary incontinence. MATERIALS AND METHODS: Between April 1999 and May 2000, 41 women with stress urinary incontinence underwent TVT procedure. Preoperative evaluation included questionnaires study, physical examination, one hour pad test and urodynamic study. Postoperative clinical outcome, patient's satisfaction and complications were checked after 3-months. RESULTS: The mean age of patients was 51.6 years (range 40-76) and mean hospital stay 2.1 days (range 1-10). The mean follow-up period was 9.9 months (range 3-15). Thirty six patients (87.8%) were cured, 2 patients (4.9%) were improved. Two of 3 patients with failed operation who developed postoperative urge incontinence had grade III cystocele. Five bladder perforations and 1 obturator nerve injury occurred. The abdominal leak point pressure (LPP) increased from 50.6cmH20 to 110.8cmH2O, detrusor pressure of maximal flow rate (Pdet.Qmax) increased from 15.8cmH2O to 28.6cmH2O and maximal flow rate (Qmax) decreased from 32.8ml/s to 22.5ml/s after operation. CONCLUSIONS: The TVT procedure is an effective and feasible surgical treatment for female stress urinary incontinence. However, in patients with grade III cystocele, postoperative urge incontinence may develop after the procedure. Since TVT procedure involves securing the mid-urethra, urinary obstruction may also occur, necessitating the need for continued follow-up as well as a careful comparison with other sling procedure.

4.Metabolism of C(14)-acetate by cestodes.

The adult worm and plerocercoid larva(sparganum) of Diphyllobothrium mansoni and Moniezia expansa employed in this experiment. The adult worms were divided into three portions, i.e. immature, mature and gravid proglottids, and each proglottids were incubated in 50 cc or 250 cc volume of special incubation flasks with incubation medium consisting of 10 cc of 25 cc of Krebs-Ringer phosphate buffer (pH 7.4). The incubation medium was added C(14)-acetate and non-radioactive carrier Na-acetate so as to contain acetate concentration of 50 mg per cent. The worms were allowed to incubate for 5 hours in the Dubnoff metabolic shaking incubator at 38 C. After incubation period, the lactate and pyruvate appearance rate, total CO(2) production tate, the turnover rates were employed as pervious report(Seo et al., 1965). The quantitative analysis of C(14)-acetate utilized by the adult worm and plerocercoid larva of D. mansoni and M. expansa were compared and discussed in this report. According to these data of the experiment, it is impressed that the fatty acid such as acetate may play a role of major part of their metabolism in the adult worm and plerocercoid larva of D. mansoni , whereas minor part of acetate participated in the metabolism by M. expansa.

5.Epidural Morphine Spray for Pain Relief after Spine Surgery.

Adequate postoperative pain control is in highlight because it has been known that uncontrolled pain has adverse effects on postoperative cardiopulmonary, immunologic functions and in turn, possibly prolongs patient's recovery. One hundred and two patients who underwent spine surgery including laminectomy, fusion and fixation were given 10mg of morphine into operative field (exposed epidural area) just prior to wound closure for relief of postoperative pain. These patients were compaired with 51 patients who did not received epidural morphine. The median of the numerical rating scale of epidural morphine spray group (Group A) at 4, 12, 24 hours after operation revealed 2, 3, 5 respectively and no epidural morhine group (Grade B) revealed 4, 5 and 5. In conclusion, postoperative pain relief of group A is superior to that of group B.

6.Morphometric Analysis of the Collagen Changes Induced by Subcutaneous Injection of Interferon-gamma following Partial Unilateral Ureteral Obstruction in the Rat Kidney.

PURPOSE: Interfenon-gamma(IFN-gamma) has been tried to decrease fibrosis in liver cirrhosis in the human and animals. To decrease collagen content in the partially obstructed kidney, we examined collagen changes by IFN-gamma after creating partial ureteral obstruction in the rat. We applied IFN-gammato the partially obstructed kidney and examined collagen contents in the renal cortex by morphometric analysis. MATERIALS AND METHODS: In a total of 50 Sprague Dawley adult male rats(300-350gm), we created partial unilateral ureteral obstruction(PUUO). We divided rats into 5 groups(n=10 in each group): normal control, sham operation group, sham operation with IFN-gamma 4 weeks, PUUO 4 weeks, PUUO with IFN-gamma 4 weeks. IFN-gamma(LG Chemical, Korea) 100,000 IU was subcutaneously injected daily for 4 weeks from operation day 1 in the treated groups. The rats were sacrificed on the expected day and renal cortex was prepared for Masson`s trichrome special staining. With the image analyser, we calculated the relative ratio of collagen to renal cortex area randomly in each kidney. RESULTS: Relative percent of collagen content of the normal control, sham-operation, and sham-operation with IFN-gamma treated groups were 2.42+/-0.53%(n=10), 2.62+/-0.53%(n=10), and 2.22+/-0.65%(n=10), respectively. While relative percent of collagen content of PUUO group, and PUUO with IFN-gamma treated group were 12.25+/-2.41%(n=5) and 7.14+/-3.43%(n=9) respectively. There was significant difference between those of last two groups.(p<0.05, Wilcoxon-rank sum test) CONCLUSIONS: We applied interferon-gamma to the partially obstructed kidney and examined collagen contents in the kidney by morphometric analysis. From the above data, IFN-gammaseems to have a decreasing effect upon collagen content in the partially obstructed rat kidney.

7.Long-term Followup Results of Desmopressin-based Combined Drug Therapy in the Nocturnal Enuretic Children.

PURPOSE: This study was designed to evaluate the effectiveness of desmopressin-based combined drug therapy in nocturnal enuretic children who had been followed at least 6 months. MATERIALS AND METHODS: Thirthy eight patients(5 to 16 years old) with 4 or more wet nights per week were studied. We used desmopressin-based combined drug therapy(imipramine or/and oxybutynin chloride) depending upon associated daytime voiding symptom. Patient response to treatment was categorized as complete(0 to 1 wet night per month), near complete(2 to 4 wet nights per month), intermediate(5 to 7 wet nights per month) and poor(more than 7 wet nights per month). We evaluated the results at the initial 6 week and at the last follow-up visit on medication. RESULTS: After 6 weeks, 13(34.2%) patients were complete responder, 13(34.2%) near complete responder, 7(18.4%) intermediate responder and 5(13.1%) poor responder, respectively. The response rate in this period was 86.8%. At the last follow-up visit on medication, 20(52.6%) patients were classified as complete responder, 13(34.2%) as near complete responder, 4(10.5%) as intermediate responder and 1(2.6%) as poor responder. Therefore, overall response rate was 97.3%. There was no obvious side effect that made discontinue drug therapy. CONCLUSIONS: From the above data, we think that longterm desmopressin-based combined drug therapy is the highly successful and safe treatment method for enuretic children.

8.Microsurgical Single Tubular Epididymovasostomy Assessment In Era of Intracytoplasmic Sperm Injection.

In the situations that intracytoplasmic sperm injection (ICSI) has revolutionized the treatment of male infertility, there appeared some needs to evaluate the role of microsurgical single tubular epididymovasostomy (EV). The outcome of microsurgical single tubular EV for obstructive azoospermia patients was retrospectively reviewed in 61 patients who had been followed for more than 18 months postoperatively. The overall patency rate followed surgery was 69% (42/61 patients), and live-birth rate was 31% (19/61 patients). Of these 19 men 13 were able to impregnate their partners; 6 of the 19 live-birth were achieved with the assistance of conventional IVF. Previously known prognostic factors-etiology, preoperative testis biopsy findings and intraoperative presence of sperm or motile sperm- all did not affect the patency and pregnancy rate. The live-birth rate of ICSI has been reported about 20~40%. Our results suggest that microsurgical single tubular EV is the preferential method to treat obstructive azoospermia patients concerning the merits of natural pregnancy and high patency rate. ICSI would be applied to the patients with patent anastomoses who have failed to achieve natural pregnancy. We think that microsurgical single tubular EV in obstructive azoospermia patients should be performed more actively by well-trained urologists in more expanded indications.

10.The Correlation Between the Smoking and Oxygen Free Radicals in Men.

BACKGROUND: This study aimed to empirically investigate the determinants on oxygen free radical degree which causes biological damage focusing on smoking habit and health behaviors in daily life. METHODS: In this cross-sectional study, the subjects were adult males who submitted free radical tests among the examinees of the comprehensive medical testings at an university hospital in Suwon from 2008 to 2010. Total 497 samples were used in analyses for clarifying the relations of free radical degree to age, smoking, obesity, and other health behaviors. RESULTS: First, the degree of oxygen free radical increases by 0.164 as the age group increases (P<0.05). Second, the degree of oxygen free radical increases by 0.547 as the smoking habit increases (P<0.001). Third, the degree of oxygen free radical increases by 0.087 as the exposure to hazardous factors in workplace increases (P<0.001). CONCLUSIONS: The findings of the study imply that the smoking habit has more great influence on the increase of oxygen free radical in body than any other health behaviors. The causality of free radical to smoking and health behavior was not identified due to the limit of the cross-sectional study so it needs to be studied prospectively in the future.
