1.An Ultrastructural Morphometric study of Follicular Center Lymphocytes of the palatine Tonsil: Reinvestigation of the Lukes-Collins' Classification of Malignant lymphoma.

The model of lymphocyte differentiation described for Lukes-Collins' classification of non-Hodgkin's lymphoma is related particularly to morphological alterations of nuclei of follicular center lymphocytes by antigenic stimulation. The authors carried out ultrastructural and morphometric studies to investigate morphological alterations during lymphocyte transformation on the nuclear profiles of follicular center, parafollicular and mantle zone lymphocytes in ten tonsillectomy cases of chronic hypertrophic tonsillitis. The nuclear parameters measured included nuclear area, contour index, frequency invagination and cleft, depth of invagination and nuclear diameters. Follicular centers contained a mixed population of lymphocytes consisting of untransformed (type 1), partially transformed (type 2) and fully transformed (type 3) lymphocytes. During lymphocyte transformation in both follicular, and parafollicular and mantle zones, the nuclei had a gradual and progressive increase in size. The nuclear contour index of type 2 nuclei of both follicular and parafollicular and mantle zones tended to be higher than those of type 1 and 3, indicating a greater degree of irregularity and variability of nuclear profiles. Invaginated and cleaved lymphocytes were not confined to me transformed lymphocytes. A considerable portion of lymphocytes had invaginations and clefts in parafollicular and mantle zone as well as follicular center. No difference on the depth of invagination was noted in type 1, type 2 and type 3 lymphocytes. The results indicate that some promise of the Lukes-Collins conepts of follicular center cells and the process of lymphocyte transformation in follicular centers may be necessary to revise

3.The in Vitro Proliferative Properties of Fibroblasts Originating from Upper and Lower Dermis of Psoriatic Skin Lesions.

To investigate the heterogeneity of fibroblast proliferation rate depending on the depth of dermis in the psoriatic skin lesions, fibroblasts were obtained from the upper and lower dermis of the forearm skin lesions in 9 psoriatir, patients and of the comparable sites in 7 healthy persons respectively, and were cultured. The in vitro proliferation rates of the fibroblasts were calculated by measuring the efficiency af cell attachment, cell numbers on varying days of culture and the population doubling time during the 3rd passage of subculture. The results were as follows: 1, The efficiencies of cell attachment at 24 hours after seeding were not statistically different between the upper derrnal fibroblasts and the lower ones in both psoriatic skin lesions and normal skins. 2. By rneasuring the cell numbers and the population doubling times, the prolifeatior rate of the upper der mal fibroblasts was greater than that of the lower ones in both psoriatic skin lesions and normal skins and that the fibroblastic proliferation rate was significantly inereased in the psoriatic skin lesion compared to the normal skin.

4.Operative Treatment in Fractures of the Humeral Shaft: Comparison of the Clinical Results of Flexible Intramedullary Nailing Versus Interlocking Intramedullary Nailing

The purpose of this paper is to compare the results, complications, advantages and disadvantages of treatment by flexible intramedullary nail with those by interlocked intramedullary nail in humeral shaft fractures. The authors analyzed 42 cases of humeral shaft fractures in patients who were treated by intramedullary nailing. Nineteen patients were treated with flexible I-M nail and twenty three patients were treated with interlocked I-M nail. Fourteen cases(74%) treated with flexible I-M nail were obtained primary bone union, and twenty one cases(91%) treated with interlocked I-M nail were obtained primary bone union. Five cases treated with flexible I-M nail and 2 cases treated with interlocked I-M nail failed union and the most common cause of nonunion was distraction. Flexible I-M nailing does not solve rotational deformity and could't obtain a high union rate due to distraction. Therefore, we recommend that adequate external support is considered to prevent distraction using the flexible I-M nail. Although we obtained a satisfactory union rate using by Seidel interlocked I-M nail, we must observe the distal spreading screw loosening during the follow up period. In bones with small diameter, sufficient reaming may be necessary, because the distal spreading screw can't spread well.

5.A Case of Pearly Penile Papules.

Pearly penile papules are small, smooth, dome-shaped, grayish to skin-colored papules, that are arranged in one or several rows. These are commonly located circumferentially on the corona and sulcus of the glans penis. A 36-year-old male patient had had asymptomatic numerous pearly smooth 1 * 1mm -sized dome-shaped papules for several months. A Histopathological examination revealed an increased number of fibroblasts on the papillary dermis, vascular proliferation and a mild lymphocytic infiltration. We diagnosed the condition as pearly penile papules. No treatment other than reassurance was given. We report, herein, a case of pearly penile papules.

7.Time-sequential Development of Multiple Squamous Cell Carcinomas on Palms and Soles.

Squamous cell carcinoma of the skin arises mostly in the head and neck regions, less frequently in the rest of the body, and rarely in the palms and soles. We report a case of a 6S-year-old man who had had time-sequential development of multiple squamous cell carcinomas on his palms and soles for the past 12 years. These lesions were, in order of time, ulcerative nodules, ulcerative hyperkeratotic papules, hyperkeratotic plaques, maceratied plaques, dark discolored patchs, and hyperkeratotic papules. They were treated by total excision and cryotherpy apart from the last squatnous cell carcinoma in 1994.

8.The Immunohistochemical Study of bcl-2 and p53 Expression of Pilomatricoma.

BACKGROUND: The pathogenesis of pilomatricoma is unknown. Several reports have suggested that apoptosis may play a role in the pathogenesis of pilomatricoma. As the bcl-2 and p53 are well-known regulators of apoptosis, a disturbance in either bcl-2 or p53 could compromise a proper apoptotic response and relate to the development of the tumor. OBJECTIVE: The purpose of this study was to evaluate the expression of bcl-2 and p53 proteins in pilomatricoma. METHODS: Paraffin blocks of 14 cases of pilomatricoma were stained immunohistochemically by using anti-bcl-2 and anti-p53 antibodies. RESULTS: 1. Histologically, SCs were found in all 14 pilomatricomas, while BCs and TCs were found each in 9 tumors. 2. All the SCs found in 14 tumors and all the TCs found in 9 tumors were unstained for both bcl-2 and p53. 3. The expression of bcl-2 in 9 tumors was variable. Basal BCs showed reactivity from negativity(-) to strong positivity(+++) and suprabasal BCs showed reativity from negativity(-) to moderately strong(++) positivity. Both basal BCs and suprabasal BCs showed negativity for bcl-2 in 3 tumors and similar positive staining in 1 tumor, while basal BCs showed stonger positivity than suprabasal BCs in 5 tumors. 4. The expression of p53 in 9 tumors was also variable. Both basal BCs and suprabasal BCs showed reactivity from negativity(-) to moderately strong positivity(++). Both basal BCs and suprabasal BCs showed negativity for p53 in 1 tumor and similar positive staining in 6 tumors, while suprabasal BCs showed stronger positivity than basal BCs in 2 tumors. CONCLUSION: The above immunohistochemical findings showed that the expression of bcl-2 and p53 was variable and suggest that both bcl-2 and p53 have a limited role in the control of apoptosis in pilomatricoma.

10.A case of Primary Serous Papillary Carcinoma of the Peritoneum..

Although the peritoneum is rare site for a primary neoplasm, certain malignant neoplasms may arise from it. A case of 63-year-old woman who had a serous papillary carcinoma of peritoneal origin is reported. Extraovarian peritoneal serous papillary carcinoma was characterized by ascites, malignant washings, and omental involvement with bulky infiltration and/or multiple tumor nodules. The symptoms caused by diffuse spreading of the neoplasm over the peritoneum are the most important manifestation for clinical diagnosis of malignant primary neoplasm of peritoneum, especially serous papillary carcinoma. This behaving tumor was at least partially responded to therapy. In this report, we describe a case of serous papillary carcinoma of peritoneum carring on proper management with brief review.
