1.The Accuracy of Self-Reported Weight and Height.

The use of self reported height and weight is very common in epidemiologic studies and clinical settings. In this study we assessed the accuracy of selfreported weight, height and body mass index by comparing with measured data. Data were obtained from 1065 persons(657 men, 358 women) who visited a health care center for routine medical check-up during the period September 1994-February 1995. The correlation between reported and measured height and weight was very strong. There were some discrepancies between two data, however, and men and women differ ed somewhat in their pattern of misreporting. Weight was overestimated by 0.27kg in men and underestimated by 0.09kg in wo men whereas height was overestimated in men(0.88cm) and women(2.15cm). These misreported data resulted in an underestimation of body mass index and low sensitivity for diagnosing overweight in men and women. The size of error was larger in women than in men. So it suggests that the self reported height and weight data could be used as a continuous variable with little error, but misclassification could be occured when the self reported data were used as a categorical variable. Errors in self reported weight and height, BMI were related to a person's over-weight status. The more people overweighed the more they underestimated their weight. Educational level and age, exercise, marri age had no relationship with the accuracy of self reported data. In conclusion, the use of self reported weight and height in epidemiologic studies and clinical settings could result underestimation of obesity, especially in obese female group.

2.The Relationship between Empowerment and Performance of Infection Control by Emergency Department Nurses.

PURPOSE: The purpose of this study was to identify the relationship between empowerment and performance of infection control by emergency department nurses. METHOD: Participants were 190 nurses working in emergency departments in 14 hospitals located in Busan. Descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficients with SPSS PC+ WIN 12.0 were used to analyze the data. RESULTS: There were significant differences in empowerment by years in ED (emergency departments) and position. There was a significant differences in performance of infection control by position. There were significant differences in empowerment by infection control-related characteristics and by regular conference for infection control. There was a significant and positive relationship between empowerment and performance of infection control in ED nurses. CONCLUSION: The results of this study indicate that improvement in the level of ED nurse's empowerment would lead to an increase in the performances of infection control.

3.An Immunohistochemical Study of the Relationships between Estrogen and Progesterone Receptors and Proliferating Cell Nuclear Antigen in Endometrial Hyperplasia and Adenocarcinoma.

Estrogen and progesterone receptors exist in the epithelial and stromal cells of the endometrium. Proliferative disorders of the endometrium may be associated with autocrine and paracrine actions of estrogen and progesterone in epithelial and stromal cells. This study was performed to evaluate the differences estrogen and progesterone receptor(ER/PR) expression in the epithelial and stromal cells of endometrial hyperplasias and adenocarcinomas using immunohistochemical methods. Immunohistochemical analysis of proliferating cell nuclear antigen(PCNA) was done to evaluate a possible correlation between PCNA and hormone receptor expression. Evaluation was based on samples from 31 simple hyperplasias, 30 complex hyperplasias, and 32 adenocarcinomas. The immunohistochemical expression of ER, PR and PCNA in epithelial and stromal cells were examined according to a scoring system based on the percentage of positive cells and the staining intensity. The results were as follows; 1) The expression of ER and PR in epithelial cells showed a graded, significant decreases in simple hyperplasia, complex hyperplasia and endometrial carcinoma, in that order(ER: P=0.008, PR: P= 0.026). 2) PR expression in the stromal cells showed a significant decrease between hyperplasia and adenocarcinoma(P=0.003). The difference in ER expression was not significant. 3) In stromal cells, the decrease in PR expression was more prominent than the decrease in ER expression when complex hyperplasia was compared to simple hyperplasia. 4) The PCNA expression in simple and complex hyperplasia and adenocarcinoma was not higher than the expression of PCNA in nomal proliferative endometrium. There was no significant difference in PCNA expression between simple and complex hyperplasia and adenocarcinoma(P=0.073). 5) A negative correlation between PCNA and ER/PR expression was not demonstrated in simple and complex hyperplasia, or in adenocarcinoma. Endometrial hyperplasia and adenocarcinoma are probably related to a paracrine action of estrogen and progesterone in epithelial and stromal cells. A progressive loss of PR expression in stromal cells may induce abnormal proliferation of endometrium due to a disrupted hormonal balance.

5.Applicability of Genes of Cancer-associated Testis Antigens in Diagnosis of Cancer.

Genes of cancer-associated testis antigens (CTAs) are expressed in various cancer tissues. In order to use CTAs as cancer diagnosis marker, we developed molecular method for detection of CTAs transcripts in tissue. In order to know the applicability of DNA of cancer-associated testis antigens (CTAs) on cancer diagnosis, molecular diagnostic methods for detection of gene expression of melanoma antigen gene (MAGE), GAGE, and B melanoma antigen (BAGE) was studied. After comparing DNA sequences of CTAs, S1/AS1 and S2/AS2, GAGE-S/ GAGE-AS, and BAGE-S/BAGE-AS primers were designed for the detection of MAGEs, GAGEs and BAGEs, respectively. The gene expression of CTAs in cancer cell lines, head and neck cancer tissues, ovary cancer tissue, and peritoneal cells of gastric cancer patients were investigated by reverse transcription-polymerase chain reaction (RT-PCR) using these primers. The MAGEs, GAGEs and BAGE genes were expressed in 8/8 (100%), 5/8 (62.5%) and 1/8 (12.5%) of head and neck cancer tissues, respectively. The gene expression of MAGEs were also detected in 8/10 (80%) of ovary cancer tissues and in 9/10 (90%) of peritoneal cells of gastric cancer patients in RT-PCR test using S1/AS1 primers. The results of this study suggest that molecular diagnosis method using CTAs genes, especially RT-PCR using S1/AS1 primer combination, is useful for diagnosis of cancer and it will be used for the prediction of cancer progression or regression and metastasis in future.

6.Aggressive Osteoblastoma: A case report

Osteoblastoma is a rare tumor which forms osteoid and bone with highly vascular stroma, account for less than 1 percent of all bone tumors. The lesion had been though to be controlled with curettage and local excision. In recent years, osteoblastomas have been described as aggressive osteoblastoma or malignant osteoblastoma, separated from the innocuous behavior of usual osteoblastoma. It represents the histologic characteristics of the presence of epithelioid osteoblasts, trabecular rather than lace-like osteoid, low mitotic rate with no atypical form with prominent giant cells of osteoclastic type. The differential diagnosis between aggressibe osteoblastoma and osteogenic sarcoma is extremely difficult, but its peculiar histologic pattern and its different clinical and radiologic features and better prognosis are helpful. We report here a case of aggressive osteoblastoma of 17 years old female patient with 3 years and 4 months follow-up period and review the literature.

7.Whole Exome Sequencing in a Korean Child with Joubert Syndrome-related Disorders.

Joubert syndrome and Joubert syndrome-related disorders (JSRDs) are rare autosomal recessive or X-linked disorders characterized by cerebellar vermis hypoplasia and a brain stem malformation, which presents as the “molar tooth sign” in magnetic resonance imaging (MRI). JSRDs are a group of clinically heterogeneous conditions that exhibit neurological manifestations and multiple organ involvement. JSRDs are also genetically heterogeneous, and approximately 20 causative genes that account for 45% of JSRDs have been identified. A 7-yr-old boy visited Wonkwang University Sanbon Hospital with the following presentations: no ocular fixation, ataxia, growth retardation, and hypotonia. Physical examination revealed facial dysmorphism, spindle shaped fingers, and height (99 cm) and weight (13 kg) below the third percentile. Ophthalmic examination revealed retinal dystrophy. A diagnosis of JSRDs was made based on clinical and brain MRI findings. We found two heterozygous variants c.2945 G>T; p.Arg982Met (G>T) and c.2216dupA; p.Phe740Valfs*2 (dupA) in AHI1, and a heterozygous c.3973C>T; p.Arg1325Trp (C>T) variant in KIF7 by whole exome sequencing (WES). Genetic analysis on the proband's father revealed that he had both AHI1 variants, but did not have the KIF7 variant, which was inconsistent with autosomal recessive inheritance. Therefore, the G>T variant and C>T variant were presumed to be of “uncertain significance.” Furthermore, one novel dupA variant was interpreted as “pathogenic,” while the second allele was not detected. Caution should be exercised while interpreting the significance of variants detected by WES. In addition, the involvement of genes other than the 20 known ones will require further investigation to elucidate the pathogenesis of JSRDs.

8.Clinical Observations of 66 Endoscopic Gastric Polypectomies.

Advance in fiberoptic endoscopy have improved diagnostic capabilities and management in patients with gastric polyps and increased experience with endoscopic polypectomy offers the most simple and safe method in removal of gastric polyps. Sixty six endoscopic gastric polypectomies was performed in 59 patients who visited Chonnam National University Hospital from 1980 to 1989 The results obtained were as follows. 1) The most patients were in the seventh decade followed by fifth, fourth decade. The ratio of male to female was 1: 1.95. 2) The common clinical symptoms with which patients presented were epigastric discomfort (54. 2%), epigastric pain (44.1%), indigestion (16.9%), nausea and vomiting (13.6%) and hematemesis (3.4%). The associated diseases of gastric polyp were chronic superficial gastritis (28.8%), chronic atrophic gastritis (13.6%), benign gastric ulcer, gastric caecer, cancer of ampulla of Vater. 3) The number of patients with single gastric polyp wa 47 (79.7%), and that of multiple gastric polyps was 12 (20.3%). The most common location of gastric polyps was gastric antrum (66.6%) follawed by gastric body (27.3%) and gastric fundus (6.1%). 4) The removed polyps were mostly 1.0 cm to 2.0 cm in size and in the gross findings by Yamadas classification, type IV (48.5%) was most common. 5) Histogical examinations revealed that 49.6% of remoyed polyps were hyperplastic polys and 18. 7% of those were adenomatous polyps. Only one case of adenomatous polyp had contaied focally malignant change of mucosa. 6) Almost all cases were in the absenee of significant complications, but three patients showed bleeding at the site of polypectomy, which controlled by conservative means.

9.Effect of the Type and Positon of Scoliosis on the Static Balance of Adolescents.

PURPOSE: This paper provides basic clinical data on the treatment of scoliosis patients by analyzing the effects of the type and position of scoliosis on the static balance using Tetrax on adolescents who have balance disorders as a consequence of structural changes, such as scoliosis. METHODS: A total of 110 adolescents were divided into 6 groups according to the radiographs and 60 adolescents were sampled, 10 each for each group. The static balance was measured and analyzed on the existence of sight using Tetrax. RESULTS: The changes followed by existence of sight in static balance group showed a significant difference statistically in Stability index in all groups (p < 0.001). The changes in the static balance in each group in terms of the stability index were significant in all groups, where the experiments were performed under the eyes opened and eyes-closed condition (p < 0.01)(p < 0.001). The post-hoc comparison revealed the stability index to be the highest in the eyes-opened condition in group III, but not in group IV. In the eyes-closed condition, group III showed the highest stability index of all the groups. CONCLUSION: Adolescents with lumbar scoliosis had a lower static balance in single scoliosis with the eyes-closed condition or had little movement compensation. In addition, unlike the other normal scoliosis, back scoliosis has negative effects on the posture because it causes an increase in the disturbance of posture. Therefore, future studied will be needed to examine the imbalance of posture in people suffering from back pain.

10.Effect of Preemptive Analgesia on the Post-operative Pain.

BACKGROUND: Preemptive analgesia may prevent nociceptive inputs generated during surgery from sensitizing central neurons and, therefore, may reduce post-operative pain. But, preemptive analgesia has been shown to decrease postinjury pain in animals, studies in human have provided controversial results. We studied whether intravenous morphine infusion before induction could affect post-operative pain and analgesic demands, when compared with intravenous morphine infusion after the closure of the peritoneum. METHODS: Female patients scheduled for total abdominal hysterectomy were randomly assigned to one of two groups of prospectively studied in a double-blind manner. Group I (n=23) and II (n=20) received intravenous morphine (0.1 mg/kg) before induction of anesthesia and after the closure of the peritoneum, respectively. Either group had continuous infusion of morphine (1.5 mg/hr) immediately after i.v. bolus morphine. Postoperative pain relief was provided with i.v. morphine from a PCA system (Medex Walkmed). Postoperative visual analogue pain scores (VAS), analgesics requirements and side effects were examined and compared between groups for 2 postoperative days. RESULTS: VAS were significantly less in group I (3.3+/-0.4) than in group II (5.3+/-0.5) 2hrs after surgery (p<0.01). Patient-cantrolled morphine cumulative consumption in group I was significantly less than in group II for 24hours (21.9 mg vs 35.3 mg) and 48hours (37.4 mg vs 55.0 mg) after operation (p<0.01). CONCLUSIONS: Preemptive analgesia with intravenous morphine reduces postoperative pain and analgesic requirements. Lower postoperative analgesic requirements in preemptive analgesic group indicate that intravenous morphine prevents development of injury-induced peripheral or central sensitization.
