Bowens disease is generally regarded as a premalignant dermatosis. If untreated, 3% to 5% of patients may develop squamous cell carcinoma. However, sebaciou carcinoma arising from Bowens disease is very rare. We presented a case of quarnous cell carcinoma and sebaceous circ s disease in a 68 year-old male. He had multiple bowenoid skin lesi nsties. A bean-sized nodule as developed on the bowenoid lesion of the he had a large yellow crust.ed exudative tumor on the Rt. thigh. We took a biopsy specimen of these three discrete lesions. The nu lipid stain of frozen section revealed Bowens disease, squarnous concllnoma arising from Bowen on the trunk and extremi Rt. lower abdomen. Almost of routine histology and carcinoma, and sebaceous carcinoma, respectively.

3.CYTOTOXICITY OF DENTAL CAST BASE METAL ALLOYS ON HUMAN ORAL KERATINOCYTES.

Although many studies on the cytotoxicity of the dental cast metal alloys and their components have been carried out, the results are rather conflicting because of the different type of cells used and the various experimental procedures taken. Recently, a number of scientists have claimed that it would be preferable to focus on the use of cells from relevant specific location of the human bodies. Consequently, the primary cultured oral keratinocyte derived from oral mucous along with nickel chloride and several of widely used dental cast base metal alloys(two-Ni-Cr alloys and one Co-Cr alloy)in domestic were selected for this study, from which 1) The amounts of released metal ions were determined using atomic absorption spectrometry, 2) The cytotoxicity of nickel chloride and dental cast base metal alloys was evaluated via MTT assay, and finally, 3) The amounts of released metal ions and the cytotoxicity of nickel chloride were correlated with the cytotoxicity of dental cast base metal alloys And, the results were summarized as follows ; 1. Nickel ion from Ni-Cr alloys and Cobalt ion from Co-Cr alloys resulted in maximum releasing rate during first 24 hours, followed by a decrease in releasing rate with time. Chromium ion were found to be minimal in all alloys. 2. In cytotoxic test, with 40muM, 80muM of nickel chloride, there were observed an increase in the relative cell number compared to control samples after 24 hours. With 160muM, there was found to be no difference in the relative cell number with control, except that 48 hour showed a increase in relative cell number. With 320muM, the relative cell number remained constant and decreased after 48 hours, and with 640muM, a continuing decrease in relative cell number was observed throughout test period. 3. The sensitivity of primary cultured oral epithelium to nickel was lower compared to the cells used in other studies. 4. CB-80 Soft and Regalloy showed no cytotoxicity to primary cultured oral epithelium and New crown resulted in a slight cytotoxicity. In conclusion, it was shown that the primary cultured oral keratinocytes could be applied successfully as testing cells in cytotoxicity test. Futhermore, the dental cast base metal alloys used in this study were found to be biocompatible.

4.Cognitive Function in Older Patients with Diabetes Mellitus.

BACKGROUND: The relationship between type II diabetes and congnitive function has been explored in several studies. But the result was controversial. The purpose of the study was to determine whether diabetes mellitus in older patients(>or=65 years) is assiciated with cognitive dysfunction and to discover the related factors with th cognitive dysfunction. METHOD: Twenty patients with type 2 diabetes and twenty subjects with non-diabetes are studied with cognitive function test at Hwachun health center and county hospital. Cognitive function was assessed using Mini-Mental State Examination and Clock Drawing test and the score was analyzed. The diabetes groups were subdivided with duration of diabetes, HbAlc, chronic diabetic complications and then analyzed. RESULTS: A total of 6(30%) diabetic subjects scored below 24 on mini-mental state examination, compared with 3(15%) of controls(p=0.2560). The mean socres were 20.1+/-2.7 and 19.0+/-3.5 respectively. Clock drawing test demonstrated that 13(65%) diabetic subjects inconectly placed the numbers and hands, compared with 7(35%) of controls(p=0.0578>0.05). The duration of diabetes mellitus, HbAlc, chronic diabetic com- plications were not associated with cognitive dysfun- ction among diabetic subjects. CONCLUSIONS: The study showed that type 2 diabetes mellitus in the elderly is not associated with cognitive dysfunction, but further study should be done.

10.Morphological Classification of Left Superior Vena Cava and Analysis of Associated Cardiac Anomalies.

The persistent left superior vena cava (LSVC) is not rare cardiovascular developmental anomaly occurring both in association with congenital heart disease and as an isolated anomaly of no hemodynamic importance. We have studied 73 cases of the LSCV out of 1,060 cases of congenital heart disease catheterized at Yonsei Cardiovascular Center. We conducted the study with a view point of position of the heart and abdominal organs and segmental analysis of the underlying congenital heart disease. We also analysed the associated extracardiac vascular anomalies. The following results were obtained: 1) The incidence of this anomaly among congenital heart disease was 6.9% and 41 cases(56.2%) had cyanosis. 2) We observed 20 cases(27.3%) with the malposition of the heart and 17 cases(23.3%) with malposition of the abdominal organs. The ventricular loops revealed D-loop in 60 cases, L-loop in 7 cases and in the remaining 6 cases, it was uncertain. 3) With a view point of type of LSVC by Lucas & Krabill, type A was in 50 cases(68.5%), type D in 14 cases(19.2%), type B in 5 cases(6.8%) and type C in 4 cases(5.5%). 4) Associated cardiovascular anomalies were as follows: ventricular septal defect; 42 cases(57.5%), atrial septal defect; 33 cases(45.2%), patent ductus arteriosus; 27 cases(36.9%), and tetralogy of Fallot; 18 cases(24.7%). In conclusion, LSVC usually has no hemodynamic importance, but this cardiac anomaly is frequently combined with complex intracardiac anomalies. Therefore, it is important to making accurate diagnosis and successful management for preventing the risk of it.
