3.Comparative Study for METHODSfor Diagnosing Onychomycosis.

BACKGROUND: Onychomycosis is very common nail problem, so an in xpensive, quick and sensitive test is essential for screening nail specimens. Recently, there is a report of new method for diagnosing onychomycosis u.;ing KOH treated nail clippings which vri then crushed and finally stained with periodic acid-Schifft(PAS) stain (KONCPA). OBJECTIVE: We evaluate the susefulness of the new methods using KOH treated nail clippings and nail debris which were then crushed and finally stained with FS (KONPA) or chlorazol black E(KONBE), for the diagriosis of onychomycosis. METHODS: We compare different methods for diagnosing onychonycisis such as KOH stains, fungal cultures, histologic evaluation, SEM, KONCPA, KONBE, and KOHJPA. RESULTS: KONPA was proved to be more effective indentifying uiigal hyphae in comparison with conventional KOH nail scraping preparation, fungal culture, and FONBE. The positive rates of each method were 74%, 46%, 43%, and 63%, respectively. Also, KENPA proved to be more rapid and easy to perform in cotnparison to the histologic evaluation of tiail clippings and SEM. CONCLUSION: KONPA is a sensitive, quick, and readily available teled for use in clinical settings in cases that are highy siispected to be onychomycosis clinicaly, but show negative results using conventional methods

5.A Case of Oil Granuloma Developed after Application of Squalene on Both Eyelids.

Oil granuloma develops mainly after injection of oil substances such as paraffin, silicone, etc, but there have been reports of oil granuloma after topical application of lipid substance to the body. A 22-year-old woman developed oil granuloma after the application of squalene on both lower eye-lids. Histopathologic findings ahowed acanthosis and dyskeratosis of the hair follicles. Lipid suketances staining with oil-red-O stain were found mainly on the hair follicles. and partly on the epidermis. We report a case of oil grinulorna caused by squalerie that may have penetrated through the hair follicles and partly through the epidermis resulting in chronic granulous changes in the dermis.

7.A Case of Congenital Hypoprothrombinemia.

Congenital hypoprothrombinemia is a rare congenital coagulation defect. The clinical signs are manifestation of generalized tendency such as; mucosal bleeding, hypermenorrhea and post tooth extraction hemorrage. It is associated with prolongation of PT and PTT with normal thrombin time and decreased serum prothrombin level. A case with congenital hypoprothrombinemia was experienced by the authors. A 36 days old male baby was admitted with palor skin and vomiting started one day before admission. Right side brain parenchymal hemorrhage and left shift of lateral ventricle were on brain CT scan. Prologation of prothrombin time and partial prothrombin time with decreased serum prothrombin level were resulted. Serum factor I, V, VII, VIII, IX and X were within normal level. We report a case congenital hypoprothrombinemia with a brief review of relaed literatures.

8.A Case of Allergic Contact Dermatitis to Nitrofurazone Ointment.

Nitrofurazone is a well known sensitizer and is widely used particularly in surgical department for the treatment of secondary infection, but the reports of all ergic contact dermatitis to nitrofurazone are rare in Korean literature. We experienced a case of allergic eontact dermatitis to 0.2% nitrofurazone in 31-year-old female after dressing the grafted wound on the left index finger with the gauze embedded with 0.2% nitrofurazone olintmnt. The patch test results revealed a strong positve reaction to 0.2% nitrofurazone ointment after 48 hours.

9.Immunohistochemical Study on Expression of CD34 in Basal Cell Carcinomas and Trichoepitheliomas.

BACKGROUND: The differentiation between basal cell carcinoma(BCC) and trichoepithelioma(TE) is sometimes difficult clinically and histologically, and their differentiation is important since their treatment and prognosis are sometimes different. OBJECTIVE: Our purpose was to investigate whether there was a difference in CD34 staining patterns in the stromas (immediate and distant stromas from the tumor lobules) of BCC and TE, since the histopathologic characteristics of the stromas are one of the most important features to differentiate the two tumor. METHOD: We perfomed immunoperoxidase staining(modified ABC technique) by using a monoclonal anti CD34 antibody(QBEND10, IgG1) on the formalin-fixed, paraffin-embedded biopsy specimens of 11 BCC as and 10 TEs. RESULTS: 1. In the immediate strcimas, spindle-shaped cells were stained in 4 out of ll cases of BCC and in 9 out of 10 cases of TE. However, the staining patterns observed in the 4 cases of BCC were all loosely scattered, week staining, while those of the 9 cases of TE were all densely compact, strong staining. CD34 was not expressed in one case of TE. 2. In the distant stromas, all cases of BCC and TE showed staining of loosely scattered spindle-shaped cells, and there was no difference in staining patterns of the two tumors. 3. Papillary mesenchymed bodies were observed in 8 cases of TE and in none of BCC, and they expressed CD34 focally. CONCLUSION: CD34 sta ining patterns of the immediate peritumoral stromas of BCCs and TEs were different and could differentiate the two tumors.
