3.Incidental gallbladder opacification after intravascular contrast infusion.

Opacification of the gallbladder after parenteral administration of contrast material usually indicates diminished renal function but can occur with normal renal function. The authors recently experienced 13 cases of such vicarious excretion of contrast media. Among 13 patients. 9 patients had renal disease unilateral ureteral stone(n=3), staghorn calculi(n=2), acute renal failure from sepsis(n=1), renal contusion(n=2) and unilateral renal artery involvement by dissecting aortic aneurysm(n=1). Of these 9 patients, 3 patients showed abnormal serum creatinine levels at the time of presentation or shortly thereafter. In 6 patients, injection of a large amount of contrast media was possibly the additional cause of vicarius excretion. There was no detectable cause in the remaining 4 patients. Heterotopic excretion of contrast media is clearly a complex phenomenon, the reason for which it is difficult to establish with certainty in each individual case. Anyhow, vicarius excretion of contrast media occurs more frequently than previously thought, and it can occur not only with abnormal renal function but with normal function also.

4.A Case of Granulomatous Reaction to Tattoo Pigment.

A 48-year-old woman, who had had tattoo on her eyebrows, was seen with pruitic erythematous indurated plaques on the tattooing sites. The skin react ion was begun to observe about 4 weeks after getting tattoo. Skin biopsy specimens from the indurated lesions showed granulomatous reactions. Intradermal tests by means of multiple pricks and patch test were done on her low back with the same tattoo dye and also 4 others available in Korea. Serial punch biopsy specimens were obtained from the sites of intradermal test at 2nd, 7th, 28th, and 56th day after intradermal test. Granulomatous reactions were found in the spiecimens taken at the 28th and 56th day.

8.Clinical Application of Hepatic Arterial Embolization

Transcatheter arterial embolization was applied in 7 cases of hepatic disease including hemobilia of various causes and malignant tumors at Department of Radiology, Seoul National University Hospital during recent several years. The embolic materials were autologous blood clot and Gelfoam particle. Successful control of bleeding or devascularization of tumor vessel was made in 6 of the 7 patients. There was no serious complications; however, post embolization syndrome was found including pain, fever, nausea and vomiting in all cases and subsided after a few days. Transcatheter hepatic arterial embolization is suggested to be a safe and effective treatment in control of hemobilia and devascularization of malignant hepatic tumors.

9.The clinical Results of Graf Instrumentation in Unstable Lumbar Spinal Disordors

Various kinds of spinal instruments have been developed for the treatment of lumbar spinal disordors. Recently, the Graf instrument as soft stabilizer has been introduced in treating lumbar spinal disordors associated with instability. To determine the reliability of Graf instrument providung spinal stability we have analysed 19 cases of unstable lumbar degenerative disease treated with adequate decompression and Graf instrumentation between May 1991 and March 1992. There were 15 females and 4 males. Average age at operation was 50.8 years (Range, 35 to 70). Minimum follow up was 14 months. The main surgical indication was serious limitation of daily activity caused by intractable symptoms and signs with spinal instability. Clinical assesment based on authors modified criteria revealed satisfactory (Exellent/Good) in 16 cases (84.2%). The Graf instrument has many advantages over rigid implant such as enough stability without arthrodesis after extensive surgical decompression, rapid rehabilitation, less operative risk and preservation of spinal motion. These results suggests Graf instrument instead of rigid implant is highly recommendable in managing unstable lumbar disordors.

10.Clinical Application of Orthofix External Fixator

Orthofix, a unilateral external fixator is simple and easy to use and it's design is also convenient to reduce and maintain the fracture site. In addition, wide range of application is an another advantage because compression and distraction is possible in the central body. Authors reviewed 56 operations in 50 patients for whom Orthofix was applied for 4 years from march 1988 to february 1992 in Chonnam University Hospital. There were 46 males and 4 females. Twenty five patients(50%) were between 20 to 40 years of age. The indication was open fracture of the long bone in 27 cases, infected nonunion in 17 cases, bony defect in 4 cases, arthrodiastasis in 3 cases, arthrodesis in 2 cases and others in 3 cases. 1. There were 23 cases treated with Orthofix only until completion of treatment, 17 cases necessary to another methods of treatment, 14 cases maintaining Orthofix until now and 2 cases miscellaneous. 2. The duration of maintenance of Orthofix until completion of treatment in 20 cases of open fracture of long bone or infected nonunion was 9.5 months in average. 3. The bone union time averaged 9.5 months in above 20 cases and there was a little difference of bone union time between open fractures and infected nonunions. 4. Result of long term follow-up in 20 cases open fracture of long bone or infected are as follows; 12 cases(60%) have angulation less than 10° or length inequality less than 10mm, 14 cases(70%) have showed no limitation in range of motion or mild limitation of joint above and below the fracture site. 5. Complications, directly related to Orthofix, developed in 17 cases. There were 15 cases of pin tract infection and 2 cases of fixation loosening. Orthofix was considered as one of the excellent external fixation system, for it is simple and easy to use and could provide rigid fixation for difficult conditions in Orthopedic field such as open fracture, infected nonunion, bony defect and arthrodiatasis.
