3.A Case of Sjogren-Larsson Syndrome.

Sjogren-Larsson Syndrome is a rare disorder chracterized by congenital icthyosis, spastic diplegia or tetraplegia and mental retardation. The inheritance is autosomal recessive, due to deficiency of alcohol dehydrogenase activity of fatty acid matabolism. We have experienced a case of Sjogren-Larsson Syndrome in a 16 month-old male children who have dried thick skin, developmental delay, and spastic diplegia. He also has glistening spot on macula area with irregular high voltage slow wave (2-3Hz) on electroencephalography. From Biopsy finding, Lamella ictyosis with acanthosis and hyperkeratosis in epithelium is shown. He was much improved clinically for skin lesion and developmental status with etretinate supplement. We report the case with brief review of related literatures.

Pulmonary Embolism, one of the causes of maternal death, is a life threatening disease that needs early and accurate diagnosis. We have exprerienced a case of a fatal pulmonary embolism which was diagnosed by lung perfusion scan on the postoperative 1 day after cesarean delivery and was managed with heparin therapy. We present this case with a brief review of literatures.

9.Dose Response of Fentanyl Cough Reflex through Peripheral Venous Catheter.

BACKGROUND: We observed fentanyl known as centrally-acting antitussive agents provoke a cough response in some patients at induction of anesthesia. This may be of clinical importance. METHOD: 121 patients (ASA class I) were assigned randomly to 4 groups. Each group was given different doses of fentanyll Group 1 (n=30); 0.5ug/kg, Group 2 (n=30); 1ug/kg, Group 3 (n=33); 2ug/kg, Group 4 (n=28); 4 g/kgl, within 1 second through a peripheral venous cannula before induction of anesthesia. All patients were observed carefully in order to detect a cough response and any side effects. RESULT: The incidences of FCR (Fentanyl Cough Response) were 0% in Group 1, 10.0% in Group 2, 30.3% in Group 3, and 39.3% in Group 4. The ED50 of FCR was 4.25ug/kg. The mean onset-time from the end of fentanyl administration to the beginning of coughing was 12.5 seconds. FCR was decreased with aging, but not affected by weight, height, or smoking. Other serious side effects were not accompanied. CONCLUSION: Fentanyl can evoke the pulmonary chemoreflex dose-dependently and the ED50 was 4.25 g/kg.

10.Comparison of the Effectiveness of the Renogram, the Serial Renal Scan and the Diuretic Half Time according to the Renal Function for Interpreting a Diuretic DTPA Scan following Pyeloplasty.

PURPOSE: The diuretic renal scan has been considered to be a good tool for evaluating the postoperative renal function in the patients with ureteropelvic junction obstruction (UPJO). We evaluated the diagnostic accuracy of the renogram, the serial renal scan and the diuretic half time according to the differential renal function (DRF) on the diuretic renal scan. MATERIALS AND METHODS: We reviewed the diuretic renal scans of 27 pediatric patients (25 males and 2 females) who underwent pyeloplasty due to unilateral UPJO. Diuretic renal scanning was done using the 99mTc- DTPA (diethylenetriaminepentaacetic acid) standardized protocol. We compared the diagnostic accuracy of the renogram, the serial renal scan and the diuretic half time according to the DRF (group I: above 40%, group II: below 40%). RESULTS: The mean age of the patients was 48.0 59.2 months (range: 2-210 months). The diuretic half time, renogram, serial renal scan and each combination demonstrated statistical significance for the interpretation of ureteral obstruction (generalized estimation equation, p=0.003). The interpretation of the diuretic renal scan was not affected by the DRF (McNemar test, p=0.598). The false positive rate and specificity of the renogram, serial renal scan and diuretic half time were 22.2%, 29.6%, 59.3% and 77.8%, 70.4%, 40.7%, respectively. In addition, the false positive rate and specificity of a combination of the diuretic half time and renogram, the diuretic half time and serial renal scan, the renogram and serial renal scan, the diuretic half time and renogram, and serial renal scan were 22.2%, 25.9%, 14.8%, 14.8%, and 77.8%, 74.1%, 85.2%, 85.2%, respectively. CONCLUSIONS: The diuretic half time, renogram and serial renal scan were individually correlated with the postoperative status of the UPJO. However, a combination of various factors of the renal scan should be considered for evaluating the postoperative status of UPJO.
