3.Treatment of Stress Urinary Incontinence with Laparoscopic Bladder Neck Suspension using Extracorporeal Needle Compared with Laparoscopic Bladder Neck Suspension and Raz`s Operations.

PURPOSE: Laparoscopic bladder neck suspension is an effective treatment for genuine stress urinary incontinence. To decrease the operating time, associated morbidity and hospital stay, we introduced laparoscopic bladder neck suspension using extracorporeal needle. We assessed the feasibility of this new technique, and compared with simple laparoscopic bladder neck suspension and transvaginal Raz procedure. MATERIALS AND METHODS: Eleven patients underwent a laparoscopic bladder neck suspension using extracorporeal needle were compared with 16 patients underwent simple laparoscopic bladder neck suspension and 66 patients treated with transvaginal Raz procedure for genuine stress urinary incontinence. All patients were assessed for their age, duration of symptom, operative time, length of hospital stay, postoperative residual urine volume, complications, and success rate. RESULTS: The result of present method was cure in 82% and improvement in 18% of patients. The overall success rate of three methods were similar. Average duration of postoperative admission was 3.4 days and average residual urine volume at discharge was 12.1 ml (simple laparoscopic Burch procedure: 5.1 days and 22.1 ml; Raz procedure: 7.7 days and 37.5 ml). Operative time of laparoscopic bladder neck suspension using extracorporeal needle was shorter than the simple laparoscopic bladder neck suspension and transvaginal Raz procedure (55 vs. 125 and 102 minutes). CONCLUSION: Laparoscopic bladder neck suspension using extracorporeal needle is a good technique for treatment of genuine stress urinary incontinence. However, additional long term follow-up study will be required.

5.Retropsoas peritoneal recess in CT.

Retropsoas space below the level of kidney has been suggested as a portion of inferior extensions of perirenal and anterior and posterior pararenal spaces. With this being true, the space may play an important role in disease extension. A study was performed to verify the existence of retropsoas peritoneal recess by means of identifying the extension of bowel loops into this space. Abdominal CT scans of 146 cases evaluated retrospectively revealed extension of 5 small bowel and 7 large bowel loops (6 descending and 1 ascending colons)(n=12/8.2%) into the retropsoas space verifying its existence. Since pathologic collection within the retropsoas space might be falsely inter preted as a retroperitoneal pathology and percutaneous uroradiologic intervention could result in intraperitoneal injury or contamination without the knowledge on the existence of this space, observation of this space is essential in CT scans.

6.Evaluation of Tumor Invasion in Gastric Carcinoma with CT Using Water as an Oral Contrast Agent in Prone Position.

PURPOSE: To evaluate the accuracy of CT using water as an oral contrast material in a prone position in determining the depth of tumor invasion in gastric cancer patients. MATERIALS AND METHODS: Thirty three patients(19 male, 14 female) with surgically confirmed gastric cancer were studied. We performed CT in a prone position after ingestion of 1 liter of pure water. CT findings were classified into 4 groups by the morphologic appearances of infiltrates in the perigastric fat plane :normal perigastric fat(SO), fine mottled densities(S1), irregular aggregated or linear densities(S2) and direct extension and invasion of tumor into contiguous structures(S3). Also we prospectively compared the CT staging with pathologic T staging according to the TNM systems. RESULTS: The overall accuracy of CT staging in determining the pathologic T factor was 69.6%. As we regarded T1 and T2 lesions as one group on CT, the accuracy of CT staging was increased to 80.2% because of a limitation of CT for distinguishing T1 from T2 lesions. CONCLUSION: Prone position CT using water as an oral contrast agent is quite accurate in determining the T staging of gastric carcinoma.

9.Egg-laying capacity of Clonorchis sinensis in guinea pig and rabbit.

Thirteen guinea pigs and 23 rabbits were challenged with 300 metacercariae of Clonorchis sinensis each and the recovery of adult worms and egg counts in feces were compared at 11-13 weeks post-infection in guinea pigs and at 7-9 weeks post-infection in rabbits. The recovery rate of adult worm was 23.5 percent in guinea pigs and 20.l-30.5 percent in rabbits. The mean E.P.G. of these animals in above period of infection was 106 in guinea pigs and 121 in rabbits. When one hundredth of E.P.G. was expressed as X and the number of adult worms as Y, the relationships between X and Y were as follows; Y=0.8X+10.8 in guinea pigs and Y=0.7X+6 in rabbits.
