1.An Experimental Study on Comedogenic effect of Various Korean Cosmetocs.

Varioua articles have been reported that extemal contactants were the cause Acne venenata. The daily use of cosmetics are tbe cause of Acne minor in adult wamen. To distinguish this from other type of Acne venenata due to extemal contactants Kligman had coined the term Acne cosmetica, which is a low grade persistant acneiform eruption with occasional papulopustulea in adult women. The authors have tested a wide range of available cosmetics(Total twenty including four kinds of cold cream species, two of cleansing cream species, three of nutrient cream species, six of lotion species, three of foundation make up species and two of foreign cosmetics) using the rabbit ear assay recently popularized by Kligman in order to evaluate comedogenic effects of various Korean ccsmetics. The results a.re as follows. 1. Among these twenty facial cosmetics, We observed no comedogenccicy in two facial cosmetics, mildly comcdogenecity in twelve facial cosmetics, moderately comedo- genecity in four facial cosmetics and severe comedogenecity in two facial cosmetics. 2. Histologic gradings were paralleled the findings of tbe naked eye in up to 65% 3. There were significant differences on comedogenecity between Korean cosmetics and foreign cosmetics which used by Kligman. 4. There were no significant diffrences between cold cream species, cleansing cream species. nutrient, cream species, lotion species and foundation make up species. 5, There were various differences among twenty kinds of every cosmetic specie.

2.Epidermal Glycogen in the Diseases Showing Epidermal Hyperplasia.

In order to assess the epidermal glycogen in the epidermal hyperplastic disease, PAS(Periodic Acid Schiff) stain with or without diastase is used. Epidermal PAS positive reaction is significantly increased in psoriasis (90. 0%), verruca vulgaris (100%) and lichen simplex chronicus (100%) than control group (12, 5%) Epidermal PAS positive reaction is relatively increased in prurigo nodularis(66. 7 %) and chronic contact dermatitis (75%) Strong PAS positive reaction is noticed in 3 of 5 cases (60%) with verruca vulgaris, but none of 22 cases with psoriasis. It is suggested that epidermal glycogen epidermal hyperplasia and that the more glycogen accumulated. than control group (12. 5%) in 3 of 5 cases (60%) with verruca vulaccumulation is accompanied with the hyperplastic the epidermis is, the more glycogen accumulated.

3.Absorption, Excretion and Antioxidative Effect of Rebamipide on Reproductive Organ.

OBJECTIVE: Rebamipide is a propionic acid derivative that has an action of the inhibition of superoxide production and removal of hydroxyl radical with the sperm incubation and cryopreservation. In the present study, to investigate whether rebamipide is useful to treat male infertility and sterility, the author observed the antioxidative effects in patient with male infertility and also examined its absorption and distribution in rat genital organ. METHODS: To measure the distribution of rebamipide in reproductive organ in the rat, carbon indicated rebamipide, 14C-OPC-12759, was orally administered to 10 Spraque-Dawley rats and its organ concentration in serum, liver, kidney, stomach, duodenum, colon, urinary bladder, seminal vesicle, epididymis and testicle were measured each time after 0.5, 1, 2, 4, 8 and 24 hours by using HPLC fluorescent method. The concentrations in semen were measured by HPLC fluorescent method in a sample of 50 infertile males who took 900 mg of rebamipide daily for 3 months. To measure the antioxidative effect and fertility rate for 3 months, each month before and after the treatment, sperm motility, vitality, the oxygen free radical formation, level of peroxidation, fetilizing capacity of semen sample which were obtained from infertile male patients by masturbation after at least 48 hours abstinence were analyzed by computer assisted semen analyzer, eosin-nigrosin stain, chemiluminescence, thiobarbituric acid method and hypo-osmotic swelling test. Simultaneously in a sample that wanted baby, both pregnancy and delivery were researched. RESULTS: The 14C-OPC-12759 concentration in the body of white rats was highest in gastrointestinal organ like stomach, smal intestine and duodenum and followed by genital organ like seminal vesicle, testis and epididymis. The rebamipide concentration in semen of infertile males was 220.77+/-327.84 ng/mL (SD) which showed a large deviation but it was higher than serum which was 126+/-76 ng/mL (SD). In the infertile males, after the treatment with rebamipide, the level of seminal reactive oxygen species (ROS) and lipid peroxidation have significantly decreased in duration of the treatment (p<0.05) and sperm vitality and fertilizing capacity except sperm motility significantly improved on post treatment of 2~3 months (p<0.05). Out of the 41 cases who hoped for pregnancy, 15 cases (36.6%) became pregnant and 12 cases had childbrith, 2 cases had miscarriage and one case is ongoing. The side effect was observed in 1 case (2%) which experienced diarrhea but it was lost spontaneously. CONCLUSIONS: We conclude from this study that rebamipide showed relatively high tendancy of absorption and excretion in the genital organ. In infertile males who had elevated ROS in semen, by specifically inhibiting the cell damage from the antioxidation, a way to preserve sperm motility, vitality and fertilizing capacity was confirmed.

5.Drug Induced Cerebral Proerectile Effect in Anesthethetized Rats.

PURPOSE: To examine the centrally elicited erectile effects of alpha-MSH, oxytocin, prostagladin E1, and anabolic steroids after intracerebroventricular administration. MATERIALS AND METHODS: We used anesthetized male Sprague-Dawley rats. After intracerebroventricular administration of normal saline (NS), alpha-melanocyte stimulating hormone (alpha-MSH), oxytocin acetate (OT), prostaglandin E1 (PGE1), methylprednisolone (MP), testosterone enanthate (TE) or 17beta-estradiol (E2) under stereotaxis, the intracavernosal pressure (ICP), systolic femoral artery pressure (FAP), heart rate (HR), time to first response, duration, and number of erectile responses and adverse reactions were evaluated for 60 minutes. To show whether erections were centrally elicited, the above criteria were re-evaluated after a bilateral pelvic neurotomy and bilateral orchiectomy. RESULTS: A cerebral proerectile effect was elicited only by alpha-MSH and OT with no significant changes of FAP or HR. With PGE1, significant changes in FAP and HR were observed. The ICP/FAP ratio was highest (0.49 0.03) with alpha-MSH. The mean time latency was shortest with OT (20.6 5.6 min). The mean duration was longest in alpha-MSH (11.6 8.7 min). The number of responses was highest with OT (3.6 0.7). Adverse reactions, such as stretching, yawning and ejaculation, were simultaneously observed during increases in ICP. In the cases of a bilateral pelvic neurotomy or bilateral orchiectomy, these elicited erectile responses disappeared. CONCLUSIONS: alpha-MSH had the most potent proerectile effect compared to OT and PGE1 as assessed by highest intracavernosal pressure as well as duration of erectile response. The results suggest that testosterone and the pelvic nerve were essential for the central proerectile response.

The classic clinical manifestations of acute suppurative cholangitis were first described by Charcot in 1877 as a triad of fever(and chill),jaundice and right upper quadrant pain. In 1959,Reynolds and Dargan characterized acute suppurative cholangitis as a distinct clinical entity manifested by a clinical pentad of Charcot,s three signs plus shock and central nervous system depression.We have clinical analysis of acute suppurative cholangitis,218 cases who were admitted in Chonnam Hospital from Jan.1989 to Dec.1995.All cases were treated conservatively initially,and then delayed operation was performed when the patients were improved from the septic condition by cholangitis.

8.Serum CA 125 Levels in Patients with Impaired Renal Function.

Measurement of the serum CA 125 level as a tumor marker in patients with epithelial ovarian cancer has been widely used to monitor disease status and predict survival of patient. While a number of benign gynecologic as well as benign or malignant non-gynecologic conditions are associated with elevations of serum CA 125 levels, the established normal range describes a healthy population of women. Because the metabolism and clearance of CA 125 is not well understood and mild or moderate degrees of renal impairment frequently occurs in ovarian cancer patients during treatment or course of disease, it is valuable to investigate the effect of impaired renal function on serum level of CA 125. Eighty-nine women on hemodialysis who had no other definite cause to elevate serum CA 125 level were selected at random. The age of the patients ranged from 19 to 83 and renal disease was secondary in most cases to diabetes mellitus, hypertension or glomeru-lonephritis. The creatinine clearance was less than 10cc/min for all patients. (continue)

9.Post-treatment Follow up of Primary Breast Cancer (I), Survey among members of The Korean Breast Cancer Society.

Periodic examination of patients after potentially curative treatment for breast cancer is routine practice. The objectives of such follow up are 1) surveillance for cancer recurrence 2) monitoring for toxicities related to therapy 3) maximizing overall health and quality of life for cancer survivors. However, recent literatures suggest that routine intensive follow up studies for asymptomatic breast cancer patients after primary treatments, showed no survival benefit. Also, there is controversy between aggressive and minimal policy for breast cancer patients follow up after primary treatment still exist. We administrated a mailed survey to The Korea Breast Cancer Society (KBSC) members (N=136,1996) to study the methods of preoperative evaluation and post-treatment follow up, as practicing in Korea and determine whether an accepted standard exists, or whether difference in practice occurred, base on physicians factors (years of physician experience, age of physician, number of breast cancer treated annually, practice type). The survey response yield was 45.59% (62/136), and mean age of the respondent was 43.7 years (range 33-67). There was roughly equal distribution of respondents by region (capital/noncapital area:27/31), and practice type (university hospital/general hospital: 37/21). The average practice in breast disease treatment of respondents (most of them were surgeon: N=57) was 8 years, and breast-conserving rate of respondents in 1996 was 210.2%. The considerable factors in follow up practice were 1) stage, 2) symptoms, 3) age, 4) operation methods, 5) demand of patient, 6) education level of patient in orders. Most frequently used tests for intial work up and base line follow up study for breast cancer patient were history, physical examination, mammography, breast US, chest X-ray, bone scan, FNA, open biopsy, CBC/ ESR, SMA, CEA, CA15-3 respectively. In this study, we proposed the model of breast cancer follow up which is practicing among the KBCS members and also we concluded as follows 1) This follow up model is more aggressive than other foreign cancer center follow up protocol 2) The KBCS members survey for non-breast cancer second malignancy less frequently than others 3) It seems to need concensus conference for Korea guideline model of breast cancer follow up.

10.Chronic Eosinophilic Pneumonia: A Case Report.

Chronic eosinophilic pneumonia is a rare diasease characterized by chronic infiltration of the lung with ~eosinophils, usually associated with peripheral eosinophilia. In 65% of cases, the chest rad Ogroaph shows typiical nonsegmental air-space consolidation confined to the outer third of the lung and in 25)/0 of case "photographic negative of pulmonary edema". Typical lung manifestations with 'peripheral eosinophiliSa' tahrcharacteristic of chronic eosinophilic pneumonia. In the remaining cases, radiographic findings are nonspecific and require lung biopsy for confirmation. We repot a case of chronic eosinophilic pneumonia in which chest radiograph and CT scans revealed bilateral patchy or diffuse opacity with nodules scattered throughout the lungs.
