1.A Clinical Study of the Trochanteric Fractures of the Femur

31 cases of the trochanteric fractures were treated at the Department of Orthopedic Surgery, Chosun University Hospital, from Jan. 1975 to Dec. 1978 and the following results were obtained. 1. The sex ratio was 3:2 (male to female). 18 cases were in man, with the peak age at the 4th decade and 5th decade and 13 cases were in woman over the 6th decade. 2. The most common causes of the trochanteric fractures were traffic accident or fall in man and slipping in woman. 3. The features of the trochanteric fractures were mostly unstable type (60%) and 16 cases in the right hip (52%) and 15 cases in the left hip (48%). 4. Dimon-Hughston method and Wayne-Country method were used to the unstable fractures and anatomical reduction to the stable fracture was maintained by inserting the S-P nail, Jewett nail and Mclaughlin nail. 5. Post-operative cast immobilization was applled until clinical union was oftained on the fracture site and early weight bearlng was carried out for the acceleration of the bony union. 6. Complications were found in 50% of the group treated by conservative method, while it occurred in 21.7% of the group treated surgically.

5.Relationship between Vimentin Expression and Progression of Uterine Cervix Epithelial Neoplasms.

Vimentin is an intermediate filament protein normally expressed in mesenchymal cells, but the evidence of the aberrant expression of vimentin in epithelial cancer cells suggests that the vimentin expression might be related to local invasiveness and metastatic potential. There have been a few previous studies on the vimentin expression in human cervical carcinogenesis using in vivo and in vitro models. We examined the immunohistochemical vimentin expression in various squamous epithelial neoplasms of the uterine cervix, including the cervical intraepithelial neoplasia group (n=25), the microinvasive squamous cell carcinoma group (n=15), the invasive squamous cell carcinoma group (n=15) and the metastatic squamous cell carcinoma group (n=8). Vimentin positivity was significantly higher in the invasive than in the intraepithelial group, and in the cases with lymph node metastasis than in those without metastasis, suggesting a relationship between the vimentin expression and progression of the uterine cervical epithelial tumors.

6.Carotid Doppler Ultrasound in Patients with Stenosis of the Intracranial Internal Carotid Artery.

PURPOSE: To investigate the Doppler ultrasonographic parameters of a carotid ultrasound (CUS) for the stenosis of the intracranial internal carotid artery (IICA). MATERIALS AND METHODS: The authors evaluated 15 patients with normal extracranial ICA, the common carotid artery (CCA), and stenosis of IICA (patient group). The control group consisted of 22 patients with normal findings on a magnetic resonance angiography (control group). All subjects were subjected to a CUS, whereas subjects from the patient group underwent a cerebral angiography. The following parameters were investigated: peak systolic velocity (PSV) and end diastolic velocity (EDV) in ICA, PSV in CCA, resistive index (RI) in ICA, and PSV ratio. Stenosis was measured following an angiography and was graded as follows: normal (0-24%), mild (25-49%), moderate (50-69%), and severe (70-99%). Next, the CUS findings, which reflected the IICA stenosis, were investigated. RESULTS: Of the 74 ICAs, 52 were normal (44 ICAs in the normal group and 8 ICAs in the patient group), 10 had mild stenosis, 8 had moderate stenosis, and severe 4 had severe stenosis. As the stenosis grade increased, PSV and EDV in ICA as well as PSV in CCA, decreased (p value < 0.05); however, the PSV ratio and RI remained unchanged. CONCLUSION: As stenosis of IICA increases, PSV and EDV of ICA and PSV of CCA decreases following a CUS.

8.The Clinical Analysis of 100 cases of Coronary artery Bypass Grafting with the Right Gastroepiploic artery.

BACKGROUND: In an effort t enhance long term patency of coronary bypass grafts, utilization of arterial conduits have been on an icrease. With the same objective, we have been using the right gastroepiploic artery (RGEA) in coronary artery bypass procedures since 1998. The current paper has been undertaken with the aim of assessing the apropriateness, problems, and short term results of using the RGEA as an arterial graft conduit by studying the postoperative clinical results of 100 patients than received coronary artery bypass grafting (CARG) with this artery. MATERIAL AND METHOD: Between May of 1998 and May of 1999, an analysis of the mortality, postoperative myocardial infarction, and the need for IABP insertion as a result of low cardiac output were made between 100 consecutive patients undergoing CABG with the RGEA. There was one postoperative death due to cerebral infarction. Postoperative complications/morbidity comprised myocardial infarction in 2, cerebral infarct in 3, reoperation due to bleeding in 1, mediastinitis in 1, and low cardiac output syndrome necessitating IABP in 3 patients. Complicatons related to harvesting of the arterial grafts were not experienced in any of the patients. CONCLUSION: The results of the current data show that utilization of the RGEA in CABG is not associated with increased mortality/morbidity and demonstrates satisfactory short term results suggesting the usefulnessof this conduit as an arterial graft.

9.Study on Pathogenicity of Borrelia burgdorferi Sensu Lato Isolated in Korea.

Spirochetes were isolated from the midgut of Ixodes persulcatus ticks captured at Chungju, Korea and identified as Borrelia afzelii strains by polymerase chain reaction. To determine the pathogenicity of the B. afzelii strains isolated in Korea, the microbiological and pathological features of Lyme disease were observed in C3H/He mice after intraperitoneal inoculation of the fresh isolate of B. afzelii strain. The results are summarized as follows 1) The Borrelia were detected in the tissues of heart, spleen, kidney, urinary bladder and knee joint within 7 days after inoculation of infection by dark field microscopic examination. The isolation rate from heart, urinary bladder and joint was significantly higher than the rate from spleen, kidney, and blood samples. 2) The Borrelia was detected in heart muscle by indirect immunofluorescent antibody test. 3) Antibody to the Borrelia was detected as early as one week after inoculation. 4) The marked tropism of the Borrelia was observed in myocardial, urinary tract and joint tissue. The main pathological features are inflammation in tissues of heart, kidney, joint and urinary bladder. From these results, the Borrelia afzelii strain isolated in Korea were determined as pathogenic strain.

10.Chronic Eosinophilic Pneumonia: A Case Report.

Chronic eosinophilic pneumonia is a rare diasease characterized by chronic infiltration of the lung with ~eosinophils, usually associated with peripheral eosinophilia. In 65% of cases, the chest rad Ogroaph shows typiical nonsegmental air-space consolidation confined to the outer third of the lung and in 25)/0 of case "photographic negative of pulmonary edema". Typical lung manifestations with 'peripheral eosinophiliSa' tahrcharacteristic of chronic eosinophilic pneumonia. In the remaining cases, radiographic findings are nonspecific and require lung biopsy for confirmation. We repot a case of chronic eosinophilic pneumonia in which chest radiograph and CT scans revealed bilateral patchy or diffuse opacity with nodules scattered throughout the lungs.
