3.Shade analysis of artificial teeth using shade scan(TM) system.

PURPOSE: The purpose of this study was to analyze and estimate whether each company may produce the color of artificial teeth as it stands, in the standard of vita classical shade guide using ShadeScan(TM) System. MATERIAL AND METHODS: we chose the products of 6 companies -EFUCERA, IVOCLAR, ENDURA, TRUBYTE, DURADENT, and DURACROSS- estimated the shade value of each fixed point(cervical, body, and incisal area) of artificial tooth, and verified the equality among the samples from the same company. RESULTS: First, the variation appeared significant at cervical and incisal area. It means that there were significant differences between cervical and incisal area although the most similar artificial teeth to the patients' natural teeth were provided. Second, the results in the body area showed that the variation between artificial and natural teeth was finite(p<0.05) in that area. CONCLUSION: it shows that the reproduction of colors of artificial teeth might be successful regarding the body is the most important part for a determination of the color of artificial teeth. However, more complements are necessary for the better reproduction of the color of artificial teeth between cervical and incisal area.

4.Control Study on Temperament and Character Traits, Resilience, and Predictors of Military Maladjusted Soldiers.

OBJECTIVES: The purpose of this study was to reveal psychological traits, risk factors, and predictors of the military maladjusted group. In addition, we attempted to compare the correlation of temperament, character profile, and resilience between maladjusted and adjusted groups. METHODS: A total of 166 maladjusted soldiers who joined the green camp of three divisions and 125 adjusted soldiers participated in the survey from July to December 2013. As self-report questionnaire methods, Beck Anxiety Inventory, Beck Depression Inventory (BDI), and the Korean version of the Connor-Davidson Resilience Scale, Temperament and Character Inventory (TCI) were used. T-test, binary logistic regression analysis, Pearson's correlation, and multiple regression analysis were used as analytical methods. RESULTS: The maladjusted group showed a high level of anxiety, depression, and Harm Avoidance and a low level of resilience, Reward Dependence (RD), Persistence, Self-Directedness, and Cooperativeness compared with the adjusted group. BDI and RD were found to be statistically significant for prediction of military maladjustment. Higher correlation between resilience and TCI was observed for the maladjusted group, but not the adjusted group. CONCLUSION: We suggest that a high depression level and a low RD level should be included as predictors among target symptoms for development of psychological treatment for the maladjusted group. In addition, we suggest that the level of resilience can affect correlation of resilience with TCI.

5.Using dental virtual patients with dynamic occlusion in esthetic restoration of anterior teeth: case reports

Recently, a method of fabricating an esthetic anterior fixed prosthesis by integrating data such as three-dimensional facial scan and jaw motion to form a virtual patient with dynamic occlusion has been introduced. This enables smooth communication with patients during the diagnosis process, improves the predictability of esthetic prosthetic treatment, and lowers the possibility of occlusal adjustment. In this case report, a virtual patient with dynamic occlusion was created in which the results of the treatment were simulated, and esthetic maxillary anterior fixed prosthesis was fabricated. With the aid of the virtual patient, the final restorations were satisfactory both in terms of esthetic and function.
6.Effect of high glucose on the prostaglandin E2 production in human gingival fibroblasts and periodontal ligament cells.

The purpose of this study was to evaluate the effect of high glucose on prostaglandin E2 production in human gingival fibroblasts and periodontal ligament cells in vitro. In control group, the cells(5x10(4) cells/ml) were cultured with Dulbecco's Modified Eagle's Medium contained with 10% fetal bovine serum, 45mg/dl glucose. In experimental groups, glucose was added to the above culture condition at the final glucose concentrations of 100mg/dl(Test group 1), 200mg/dl (Test group 2) and 400mg/dl(Test group 3). Then each group was tested for the cell proliferation rate, protein levels, and prostaglandin E2 production at 1/2, 1, 2, 5 days. The results were as follows ; 1. As glucose concentration increased, cell proliferation rate decreased significantly at 1, 2, 5 days in human gingival fibroblasts and periodontal ligament cells(P<0.01). 2. In human gingival fibroblasts, test group 2 and 3 showed significantly decreased protein levels as compared to control group at 5 days(P<0.01). 3. In human periodontal ligament cells, as glucose concentration increased, protein levels decreased significantly at 2 days and 5 days(P<0.01). 4. Prostaglandin E2 production in human gingival fibroblasts and human periodontal ligament cells significantly increased as glucose concentration increased(P<0.01). The results at 5 days showed obvious difference as compared to those at 2 days. From the above results, high glucose appeared to affect cellular activities including cell proliferation rate, protein levels and enhance prostaglandin E2 production. It was assumed that prostaglandin E2 production by high glucose enhances inflammatory reaction and has a toxic effect on human gingival fibroblasts and human periodontal ligament cells. This study suggests that periodontal disease in diabetic patient is related to prostaglandin E2 production.

7.Cardiac Arrest due to Recurrent Ventricular Fibrillation Triggered by Unifocal Ventricular Premature Complexes in a Silent Myocardial Infarction.

A 51-year-old male patient was referred for a sudden out-of-hospital cardiac arrest. Upon arrival, he was conscious and had no chest pain complaints. There was no abnormality in initial electrocardiographic and echocardiographic examinations. However, episodes of recurrent ventricular fibrillation (VF) were documented on rhythm monitoring. Each VF episode was triggered by an isolated monomorphic ventricular premature complex (VPC). Suspecting idiopathic VF, emergency radiofrequency catheter ablation was planned for the VPCs. However, when coronary angiography was performed to exclude silent ischemia, the results showed a total occlusion of the right coronary artery posterolateral branch, which is thought to supply the left ventricular inferior and septal wall. After successful reperfusion, VF episodes and the triggering VPCs disappeared. We are documenting this case to emphasize the potential for silent myocardial infarction to cause out-of-hospital sudden cardiac arrest even in a patient without any symptom or sign of acute coronary syndrome.

8.Histological Observation of Bone Reaction with Relation to Surface Treatment of Titanium.

The biocompatibility of the titanium has been estabilished through various experimental studies such as cell culture toxicity test, pyrogen test, mutagen test and others. In order to confirm biocompatibility after fabrication of titanium and to clarify the difference between the bone reaction after insertion of the lathed titanium rods and the bone reaction after insertion of the finished and polished rods, both rods were implanted into the proximal femur of a rabbit. Histologic reactions in the bone were observed according to the ASTM standards at the intervals of 6 weeks, 12 weeks and 26 weeks after implantation. The result were as follows In 6 weeks after implantation of lathed titanium rods, inflammatory reactions, such as minimal degree infiltration of polymorphonuclear leukocytes and lymphocytes were observed in all cases. This was thought to be caused by surgical trauma. However, inflammatory cell infiltration was not seen after implantation of polished and finished rods in all cases. The cellular infiltration and the histologic reaction of the bone after implantation of lathed group were significantly more pronounced than those after implantation of the finished group. In 12 weekt after implantation of lathed rods, two of four cases revealed a minimal degree of cellular infiliration and histologic reaction seemed to be more pronounced in the lathed group, but they were not significant statistically.

10.A Case of Moyamoya Syndrome Associated with Hereditary Spherocytosis.

Moyamoya is a chronic cerebrovascular disease characterized by progressive stenosis or occlusion of the terminal parts of both intermal carotid arteries with telangiectatic vascular network of collateral circulation at the base of the brain and leptomeningeal arteries. The etiology and pathophysiology of this disease are still unknown. Although the idiopathic presentattion is the commonest, moyamoya disease has also been reported in several hereditary or acquired clinical conditions including neurofibromatosis, sickle cell anemia, tuberculous meningitis, atherosclerosis, and following radiation therapy to the head. The term moyamoya disease should be reserved for those cases in which the characteristic angiogrphic pattern is idiopathic; moyamoya syndrome is used when the underlying condition is known. We have experienced a case of coexistence of moyamoya syndrome and hereditary spherocytosis in a 6-year-8-month-old girl who presented with right-sided hemiparesis and pallor. A cerebral angiogram revealed occlusion of proximal portion of left middle cerebral artery and abnormal collateral network. The peripheral blood smear and osmotic fragility test disclosed hereditary spherocytosis. To our knowledge, the coexistence of moyamoya syndrome and hereditary spherocytosis has not been documented. We report here the case and the brief review of related literatures. Further studies are needed to clarify the intimate relationship between the two diseases.
